Sweat is an essential protection system for the body, but its failure can result in pathologic conditions, including several skin diseases, such as palmoplantar pustulosis (PPP). As reduced intraepidermal E-cadherin expression in skin lesions was confirmed in PPP skin lesions, a role for interleukin (IL)-1-rich sweat in PPP has been proposed, and IL-1 has been implicated in the altered E-cadherin expression observed in both cultured keratinocytes and mice epidermis. For further investigation, live imaging of sweat perspiration on a mouse toe-pad under two-photon excitation microscopy was performed using a novel fluorescent dye cocktail (which we named JSAC). Finally, intraepidermal vesicle formation which is the main cause of PPP pathogenesis was successfully induced using our "LASER-snipe" technique with JSAC. "LASER-snipe" is a type of laser ablation technique that uses two-photon absorption of fluorescent material to destroy a few acrosyringium cells at a pinpoint location in three-dimensional space of living tissue to cause eccrine sweat leakage. These observatory techniques and this mouse model may be useful not only in live imaging for physiological phenomena in vivo such as PPP pathomechanism investigation, but also for the field of functional physiological morphology.

Nose reconstruction is challenging given the three-dimensional structure and free edge, and various methods have been reported. In general, local flaps provide cosmetic outcomes that are better than those following skin grafts, but there are no published comparative studies on Asians. To determine whether local flaps or skin grafts may optimally be used to reconstruct external nasal defects among Asians. We retrospectively collected data on patients who underwent external nasal tumour resection and reconstruction by 14 plastic surgeons in eight Japanese institutes from 2009 to 2021. The cosmetic results were scored by 14 surgeons using anonymized preoperative and six-month postoperative photographs. Scores for each reconstruction method were statistically evaluated. In total, 86 cases were enrolled; 57 received local flaps and 29 received skin grafts. Most local flaps showed better outcomes compared to skin grafts, but this was not the case for nasolabial and forehead flaps. Notably, local flaps placed in the nasal ala tended to be less successful than flaps placed elsewhere; only the bilobed flap scored better than skin grafts. The defect site did not affect the results of skin grafts. For Asians requiring nasal reconstruction, local flaps provide better cosmetic outcomes than skin grafts, except for those in the nasal ala. Skin grafts may be a good alternative when the bilobed flap is unavailable for the nasal ala.

80歳男性。左臀部皮下腫瘍の精査目的で当科へ紹介となった。所見では血液生化学検査では特に異常はみられなかったが、骨盤部MRI検査では前仙骨部から左臀部にT2強調像で高信号、T1強調像で低信号の、内部は均一な125×68×60mm大の多房性嚢胞性の腫瘤が認められた。治療として経仙骨的アプローチで腫瘍摘出術を行ったところ、病理組織学的に前仙骨部epidermoid cystと診断された。しかし、術後6年で腫瘍が再発し、再度摘出術が行われるも、残存腫瘍の再増大を繰り返した。以後、3回目手術では仙骨離断が行われたが、瘢痕と癒着で切除不十分となり、最終的に5回目の手術時にフェノール法で嚢腫壁を焼灼し、腫瘍は再増大なく経過している。

58歳男。40歳頃から胸部正中の皮下結節を自覚し、徐々に増大していたが放置していた。57歳時に背部感染性粉瘤の加療目的で近医整形外科を受診した際に胸部正中病変の針生検が行われ、悪性腫瘍が疑われたため当科に紹介された。胸部正中の胸骨尾側端に5cm大、弾性硬で皮膚と癒着し皮下との可動性良好な腫瘤を認めた。生検を行い、神経分化を伴う基底細胞癌が疑われたため拡大切除術を施行した。切除標本の病理組織所見は、病変と表皮との連続性がないことや、ロゼット様構造を認めたことなどが基底細胞癌の特徴とは一致せず、確定診断には至らなかった。他施設へ病理コンサルテーションを行い、SCAND(神経内分泌分化を伴う汗腺癌)との鑑別を要したが、CK20陽性の非腫瘍性Merkel細胞を認めたことから最終的に毛芽腫と診断された。

Nuclear IL-33 levels are high at the epidermal edges of skin wounds and facilitate wound healing. However, IL-33-mediated regulation of keratinocyte (KC) biology during wound healing remains poorly understood. During skin-wound healing, KC migration and re-epithelialization are mediated predominantly by EGFR signaling activation and depend on the function of signal transducer and activator of transcription 3 (STAT3). We found that migrating KCs at the leading edges of mouse skin wounds exhibited concomitant induction and nuclear colocalization of IL-33 and phosphorylated STAT3. In cultured human KCs, activation of EGFR signaling caused rapid elevation of nuclear IL-33, which directly interacts with phosphorylated STAT3, promoting STAT3 activation. In vitro KC migration and wound-healing assays revealed that high nuclear IL-33 levels were required for KC migration and wound closure. KC mobility associated with a lack of suprabasal epidermal keratins and extracellular matrix degradation mediated by matrix metalloproteinases (MMPs) control cell migration at the intracellular and extracellular levels, respectively. In EGFR-activated KCs, nuclear IL-33 mediated keratin 1 and 10 downregulation and MMP9 upregulation by promoting STAT3 activation and limited MMP1, MMP3, and MMP10 induction by suppressing NF-κB transactivation. Thus, epidermal nuclear IL-33 is involved in KC migration and wound closure by regulating the STAT3 and NF-κB pathways.

Trehalose is the nonreducing disaccharide of glucose, evolutionarily conserved in invertebrates. The living skin equivalent (LSE) is an organotypic coculture containing keratinocytes cultivated on fibroblast-populated dermal substitutes. We demonstrated that human primary fibroblasts treated with highly concentrated trehalose promote significantly extensive spread of the epidermal layer of LSE without any deleterious effects. The RNA-seq analysis of trehalose-treated 2D and 3D fibroblasts at early time points revealed the involvement of the CDKN1A pathway, the knockdown of which significantly suppressed the upregulation of DPT, ANGPT2, VEGFA, EREG, and FGF2. The trehalose-treated fibroblasts were positive for senescence-associated β-galactosidase. Finally, transplantation of the dermal substitute with trehalose-treated fibroblasts accelerated wound closure and increased capillary formation significantly in the experimental mouse wounds in vivo, which was canceled by the CDKN1A knockdown. These data indicate that high-concentration trehalose can induce the senescence-like state in fibroblasts via CDKN1A/p21, which may be therapeutically useful for optimal wound repair.

Various epidermal growth factor receptor (EGFR) ligands are highly expressed in the epidermis of psoriasis lesions, and abnormal EGFR activation appears to be involved in the pathogenesis of psoriasis. However, how EGFR signaling contributes to the development of psoriasis is unclear. Interleukin (IL)-17A, a critical effector of the IL-23/IL-17A pathway, increases the expression of psoriasis signature genes in keratinocytes and plays an essential role in the pathogenesis of psoriasis by inducing IκBζ, a critical transcriptional regulator in psoriasis. In this study, we stimulated primary human keratinocytes with IL-17A and various EGFR ligands to investigate whether EGFR ligands regulate the expression of psoriasis signature genes. In cultured normal human keratinocytes and a living skin equivalent, EGFR ligands did not induce psoriasis-related gene expression, but significantly enhanced the IL-17A-mediated induction of various psoriasis signature genes, including antimicrobial peptides, cytokines, and chemokines. This was dependent on an EGFR activation-mediated synergistic increase in IL-17A-induced IκBζ expression and was partially mediated by the EGFR-dependent upregulation of Bcl3. Therefore, EGFR ligands can act as synergistic agents of IL-17A signaling by stimulating the epidermal production of psoriasis signature genes in psoriasis lesions. This study reveals a potential mechanism by which EGFR signaling contributes to the pathogenesis of psoriasis. This article is protected by copyright. All rights reserved.

Atopic dermatitis (AD) is an inflammatory skin disease characterized by skin barrier dysfunction. Although T helper type 2 cytokines downregulate the expression of epidermal barrier proteins, the signaling mechanism underlying these effects remains unclear. IL-33, a chromatin-associated cytokine, is highly expressed in the nuclei of epidermal keratinocytes in AD skin; however, it is unclear whether this expression promotes the development of AD. TSLP, an epithelial cells-derived pro‒T helper type 2 cytokine, is elevated in the epidermis of patients with AD. TSLP affects the pathogenesis of AD by activating T helper type 2 responses and impairing epidermal barrier integrity. In this study, we stimulated postconfluent human keratinocytes and living skin equivalent with TSLP to investigate the role of nuclear IL-33 in TSLP-induced epidermal barrier defects. We observed that TSLP reduced the levels of FLG, hBD2, S100A7, and claudin-1, which required nuclear IL-33 expression. Similar to the T helper type 2 cytokines IL-4, IL-13, and IL-31, TSLP was shown to upregulate IL-33 expression and triggered the formation of nuclear IL-33/phosphorylated signal transducer and activator of transcription 3 complex, which bound to the FLG promoter, thereby inhibiting transcription. Moreover, nuclear IL-33 acted as a cofactor of signal transducer and activator of transcription 3 in the TSLP-induced transcriptional repression of hBD2, S100A7, and claudin-1. Therefore, epidermal nuclear IL-33 may be a key regulator of TSLP-mediated epidermal barrier dysfunction.

IL-33, a chromatin-associated multifunctional cytokine, is implicated in the pathogenesis of atopic dermatitis (AD), an inflammatory skin disorder characterized by skin barrier dysfunction. IL-33 accumulates in the nuclei of epidermal keratinocytes (KCs) in AD lesions. However, it is unclear whether nuclear IL-33 directly contributes to the pathogenesis of AD. IL-31, a pruritogenic cytokine primarily produced by T helper type 2 cells, is elevated in AD lesions and promotes AD development by suppressing KC differentiation and inducing itching. In this study, we investigated the involvement of nuclear IL-33 in IL-31‒mediated suppression of KC differentiation. In monolayer cultures and living skin equivalent, IL-31 increased the expression of full-length IL-33 and the phosphorylation of signal transducer and activator of transcription 3 (STAT3) in the nuclei of human KCs, which in turn downregulated the expression of differentiation markers. We found that IL-31 and IL-4/IL-13 use very similar mechanisms to inhibit KC differentiation: nuclear IL-33 combines with phosphorylated STAT3 and functions as a STAT3 transcription cofactor, promoting phosphorylated STAT3 binding to the FLG promoter to inhibit its transcription; moreover, the nuclear IL-33/phosphorylated STAT3 complex drives the downregulation of keratin 1 and keratin 10 by reducing the availability of the transcription factor RunX1. Therefore, nuclear IL-33 plays an important role in IL-31‒mediated differentiation suppression by regulating STAT3 activation in human KCs.

The previous reports show that IL-33 is highly detected in the nucleus of epidermal keratinocytes in AD lesions compared with that in unaffected or normal skin. However, it is unclear whether intracellular IL-33 directly contributes to the pathogenesis of AD. T helper type 2 cytokines IL-4 and IL-13 that are elevated in AD lesions suppress keratinocyte differentiation to impair skin barrier function. We investigated whether intracellular IL-33 is involved in IL-4 and IL-13 function. In monolayer culture and living skin equivalent analyses, IL-4 and IL-13 increased the expression of full-length IL-33 in the nucleus of keratinocytes by activating the MAPK/extracellular signal‒regulated kinase kinase/extracellular signal‒regulated kinase signaling pathway, which is necessary for the inhibition of differentiation markers FLG, LOR, keratin 1, and keratin 10. The nuclear IL-33 functions as a transcription cofactor of signal transducer and activator of transcription 3, increasing the binding of phosphorylated signal transducer and activator of transcription 3 to FLG promoter, thereby inhibiting its transcription, and it inhibits the expression of transcription factor RUNX1 by signal transducer and activator of transcription 3 and signal transducer and activator of transcription 6, thereby downregulating LOR, keratin 1, and keratin 10. Thus, the elevated nuclear IL-33 in the epidermis of AD lesions may be involved in the pathogenesis of AD by inhibiting keratinocyte differentiation and skin barrier function.

55歳男性。ネコに右示指を咬まれたが放置し受傷後5日より排膿を認めた。前医にて抗生剤内服・静脈投与、切開術、創内の掻爬術を行ったが増悪した。切断を勧められたが患指温存の強い希望があり、紹介受診となった。血液検査、単純X線所見より化膿性腱鞘炎および指骨骨髄炎と診断した。デブリードマン、不良肉芽掻爬、壊死した深指屈筋・伸筋腱の切除、可能な限りの遊離骨片の除去を行い、末節骨と中節骨をキルシュナー鋼線で固定した。細菌培養からPasteurella multocida、Peptostreptococcus anaerobius、グラム陽性桿菌を検出したため、術後、2週間のペニシリン製剤静脈投与、1日2回の生理食塩水による創部洗浄を行った。キルシュナー鋼線は3週間目に自然抜去され、約1ヵ月で炎症は改善し、創部は上皮化したが偽関節の残存を認めた。偽関節の治療は1年6ヵ月後に行ったが、移植腸骨の完全生着に6ヵ月以上を要し、指の短縮、感覚障害は残存している。

The protractor-based goniometer is the standard instrument used to measure finger range of motion (ROM). However, the method is often complicated to apply and places a burden on the investigator. Here, we developed a new method for finger ROM measurement using a smartphone. This study was performed to determine the reliability and convenience of this new method. The ROM in 1007 finger joints was measured by both the standard and new method and the data were analyzed using the intraclass correlation coefficient (ICC). The smartphone ICC score was high (0.927), and the average measurement time per joint was 49% lower with the smartphone compared to the goniometer. The results indicated that the smartphone-based measurement method had the same reliability as the conventional goniometer, in addition to an excellent measurement time.

The objective was to prepare guidelines to perform the current optimum treatment by organizing effective and efficient treatments of hemangiomas and vascular malformations, confirming the safety and systematizing treatment, employing evidence-based medicine techniques and aimed at improvement of the outcomes. Clinical questions (CQ) were decided based on the important clinical issues. For document retrieval, key words for published work searches were set for each CQ, and work published from 1980 to the end of September 2014 was searched in PubMed, Cochrane Library and Japana Centra Revuo Medicina databases. The strengths of evidence and recommendations acquired by systematic reviews were determined following the Medical Information Network Distribution System technique. A total of 33 CQ were used to compile recommendations and the subjects included efficacy of resection, sclerotherapy/embolization, drug therapy, laser therapy, radiotherapy and other conservative treatment, differences in appropriate treatment due to the location of lesions and among symptoms, appropriate timing of treatment and tests, and pathological diagnosis deciding the diagnosis. Thus, the Japanese Clinical Practice Guidelines for Vascular Anomalies 2017 have been prepared as the evidence-based guidelines for the management of vascular anomalies.

In atopic diseases, the epithelium releases cytokines and chemokines that initiate skin inflammation. Atopic dermatitis (AD) is characterized by a disrupted epidermal barrier and is triggered or exacerbated by environmental stimuli such as house dust mite (HDM) allergens. The proinflammatory cytokine interleukin 33 (IL-33) plays an important role in the pathogenesis of AD, but how IL-33 production in keratinocytes is elicited by HDM is unknown. To that end, here we stimulated monolayer-cultured human keratinocytes and human living skin equivalents with Dermatophagoides pteronyssinus HDM extract to investigate its effects on IL-33 production from keratinocytes. The HDM extract induced intracellular expression of IL-33 and modulated its processing and maturation, triggering rapid IL-33 release from keratinocytes. Group 1 HDM allergen but not group 2 HDM allergen elicited IL-33 production. An ATP assay of keratinocyte culture supernatants revealed an acute and transient accumulation of extracellular ATP immediately after the HDM extract stimulation. Using the broad-spectrum P2 antagonist suramin, the specific purinergic receptor P2Y2 (P2RY2) antagonist AR-C118925XX, and P2RY2-specific siRNA, we discovered that the HDM extract-induced IL-33 expression was mainly dependent on extracellular ATP/P2Y2 signaling mediated by transactivation of epidermal growth factor receptor, followed by activation of the ERK kinase signaling pathway. Moreover, HDM extract–induced release of 25-kDa IL-33 from the keratinocytes depended on an extracellular ATP/P2 signaling–mediated intracellular Ca2+ increase. Our study demonstrates the new mechanism controlling the induction and maturation of keratinocyte-produced IL-33 by HDM allergens, an innate immune process that might play a role in AD development or severity.

60歳、女性。右後頭部の青色斑内に黒色ドーム状結節と赤色扁平結節を認めた。赤色結節では真皮全層～皮下にメラニンを含有しない大型の異型細胞が増殖しており、免疫組織染色で腫瘍細胞はHMB-45、Melan-A陽性であった。一方、黒色結節では真皮全層性にメラニン顆粒を多量に含有する異形の乏しい細胞が密に増殖していた。以上より、cellular blue nevusに生じたamelanoticな結節を呈するmalignant blue nevusと診断した。術後2年で肺、骨に多発転移を生じ死亡した。(著者抄録)

37歳女。当科受診の約4ヵ月前から陰部にそう痒感を伴う結節を自覚していた。結節は徐々に増加し、疼痛を伴うようになったため近医婦人科を受診した。陰部ヘルペスと診断され、抗ウイルス薬の外用・内服や抗真菌薬の外用が行ったが改善せず、同院皮膚科を受診したところ外陰部多発粉瘤と診断され、手術目的で当科に紹介された。剃毛を行い詳細に診察したところ、左小陰唇に10本程度の毛を含む数ミリ大の瘻孔を認めた。手術は局所麻酔下に多発粉瘤切除と小陰唇の瘻孔摘出術を同時に行い、病理組織所見から大陰唇多発粉瘤および小陰唇毛巣洞と診断した。術後経過は良好で、4ヵ月後の現在まで再発は認めていない。

6ヵ月女児。出生時から左上眼瞼外側に毛の束を認めていた。両親が治療を希望し、近医小児科を経て当科紹介となった。左上眼瞼に3mm大の皮膚陥凹と10本以上の発毛を認めた。同様の症例について文献検索したところ眼窩内dermoid cystを合併している症例の報告があったためMRIを施行したが、眼窩周囲に腫瘍性病変はみられず、異所性睫毛と診断して摘出術を施行した。全身麻酔下に、毛根および陥凹部周囲を含めるように紡錘形にデザインした。上眼瞼結膜を反転すると瞼板外側に陥凹を認め、毛を牽引すると結膜側の陥凹が強調された。デザインに沿って皮膚切開し、毛根周囲を剥離していくと、瞼板外側方向に向かう索状物を認めた。索状物の終末は瞼板とは癒着しておらず、眼輪筋を一部含めて一塊に摘出したところ、瞼板外側の陥凹は消失した。摘出標本の病理組織所見は、多数の皮脂腺を有する成熟した毛包が密集し、その周囲に立毛筋は認めなかった。索状物は線維性組織で構成されていた。術後6ヵ月の現在まで再発は認めていない。

73歳女性。71歳時より右坐骨部に褥瘡が生じ、前医にてデブリードマン、局所陰圧閉鎖療法などを受けるも難治性を示し、手術目的で当科へ紹介となった。入院時、所見では右坐骨部に5cm大の皮膚潰瘍がみられ、頭側には2cmのポケットが認められた。創部の状態は比較的良好であったため、デブリードマンと後大腿皮弁による一期的再建術を計画した。その際、マーキングとしてシリンジで吸った塩化メチルロザリニン(ビオクタニンブルー)エタノール液2%を27G針で刺青を行った。皮弁は問題なく生着したが、術後14日目にビオクタニンブルーエタノール液2%の刺青よる脂肪壊死をきたした。術後16日目に潰瘍の切除縫縮が行われたが、創離開したため、術後20日目より抗MRSA薬を投与しつつ、局所陰圧閉鎖療法を13日間継続し、術後33日目に再度切除縫縮した。その結果、2回目の縫縮術後は褥瘡、潰瘍の再発はみられていない。

Palmoplantar pustulosis (PPP) is a chronic skin disease characterized by infectious-like pustules on the palms and soles. These pustules are thought to be sterile because bacterial cultures obtained from the pustules are negative. However, culture methods are limited in their ability to identify all bacteria on the skin. We hypothesized that the “sterile” pustules of PPP do not lack bacteria, but rather contain a microbiome. To test this hypothesis, we identified bacteria in “sterile” pustules using non-culture methods. We conducted Sanger and 16S rRNA sequencing using primers specific to the V1-V2 region in PPP-pustulovesicles (PVs) (n = 43) and pompholyx vesicle fluids (n = 15). Sanger sequencing identified some Staphylococcus, Propionibacterium, Streptococcus and Pyrinomonas species in PPP-PVs but failed to identify any bacteria in most of the pompholyx vesicles. 16S rRNA sequencing of PPP-PVs indicated the presence of a microbiome that included various phyla, including Firmicutes, Proteobacteria, Actinobacteria and Bacteroidetes. At the genus level, smokers had higher levels of Staphylococcus in PPP-PVs compared with non-smokers. These results indicate that a microbiome exists in “sterile” pustules of PPP and that PPP smokers had higher levels of Staphylococcus in pustules. It is therefore necessary to reconsider the pathogenesis of PPP from the perspective of the microbiome.

形成外科医にとって局所皮弁術は、皮膚・皮下縫合や植皮術に次いで習得すべき必須の技術である。現在までに局所皮弁から穿通枝皮弁、遊離皮弁に至るまで数多くの皮弁が報告され、その基本原理や応用についての成書も数多く出版されている。したがって、初めて皮弁術を行う術者にとっても、敷居はそれほど高くないと言えよう。しかし、簡便な方法に見えるからこそ基本をしっかり理解していないと、思わぬピットフォールに陥ることもある。本稿では、皮弁初心者が最初に使用する機会が多いと思われる、局所皮弁の基本形のtransposition flapとrotation flapについて、そのデザインから臨床応用までをわかりやすく解説する。(著者抄録)

Epidermal keratinocytes initiate skin inflammation by activating immune cells. The skin barrier is disrupted in atopic dermatitis (AD) and epidermal keratinocytes can be exposed to environmental stimuli, such as house dust mite (HDM) allergens. We showed previously that HDM allergens activate the NLRP3 inflammasome of keratinocytes, thereby releasing pro-inflammatory cytokines. Heparinoid is an effective moisturizer for atopic dry skin. However, a recent report showed that heparinoid treatment can improve inflammation of lichen planus. Therefore, we hypothesized that it acts on epidermal keratinocytes not only as a moisturizer, but also as a suppressant of the triggers of skin inflammation. We found that HDM allergen-induced interleukin (IL)-1β release from keratinocytes was inhibited significantly by heparinoid pretreatment without affecting cell viability. However, heparinoid did not affect caspase-1 release, suggesting that heparinoid did not affect HDM allergen-induced inflammasome activation. Heparinoid treatment not only decreased intracellular levels of pro-IL-1β, but also suppressed IL-1β messenger RNA (mRNA) expression in keratinocytes. Among the intracellular signalling pathways, the activation of extracellular signal-regulated kinase and p38 pathways, which are required for IL-1β expression in keratinocytes, was inhibited by heparinoid treatment. The inhibitory effect of heparinoid on IL-1β mRNA expression was also confirmed with living skin equivalents. Our results demonstrated that heparinoid suppresses the initiation of keratinocyte-mediated skin inflammation.

IL-22 induces STAT3 phosphorylation and mediates psoriasis-related gene expression. However, the signaling mechanism leading from pSTAT3 to the expression of these genes remains unclear. We focused on Bcl-3, which is induced by STAT3 activation and mediates gene expression. In cultured human epidermal keratinocytes, IL-22 increased Bcl-3, which was translocated to the nucleus with p50 via STAT3 activation. The increases in CXCL8, S100As and human β-defensin 2 mRNA expression caused by IL-22 were abolished by siRNA against Bcl-3. Although CCL20 expression was also augmented by IL-22, the knockdown of Bcl-3 increased its level. Moreover, the combination of IL-22 and IL-17A enhanced Bcl-3 production, IL-22-induced gene expression, and the expression of other psoriasis-related genes, including those encoding IL-17C, IL-19, and IL-36γ. The expression of these genes (except for CCL20) was also suppressed by the knockdown of Bcl-3. Bcl-3 overexpression induced CXCL8 and HBD2 expression but not S100As expression. We also compared Bcl-3 expression between psoriatic skin lesions and normal skin. Immunostaining revealed strong signals for Bcl-3 and p50 in the nucleus of epidermal keratinocytes from psoriatic skin. The IL-22-STAT3-Bcl-3 pathway may be important in the pathogenesis of psoriasis.

32歳男。全身倦怠感、左胸部の鈍痛を主訴とした。3歳時よりペースメーカー埋め込み手術を複数回受けており、29歳時の肺動脈弁置換時に縦隔洞炎(MRSA)を起こして入退院を繰り返していた。紹介受診時には38度台の発熱と全身倦怠感、左胸部から正中にかけて発赤腫脹と圧痛があった。胸部レントゲン、胸部単純CTでは左第3肋骨から腹部に至る多数の未使用のペースメーカーリードと左第4、5肋骨付近の膿瘍形成を認めた。FDG-PET/CTにて皮弁下部から腹部のリードに沿って炎症性病変が存在したことから、小児期に留置された複数のペースメーカーリードが、成人期の弁置換時に起こった縦隔洞炎を契機に感染源となり、一連の病態を引き起こしたと考えられた。段階的にリードとリード端末を摘出することで治癒に至り、術後4年経過現在、感染の再燃は認めていない。本症例では感染部位の特定にFDG-PET/CTが有用であった。

症例は60歳男性で、右臀部の痛みが出現し、急速に増悪した。CTおよびMRI検査で骨盤内に多房性嚢胞性腫瘤を認めた。腫瘍マーカーではSCC抗原上昇、CEA軽度上昇を認めた。骨盤部MRIでは、前仙骨部に一部T2WIで低信号の充実性成分を伴う7×9×10cm大の多房性嚢胞性の腫瘤を認め、それに連続して右臀部脂肪内にも小結節の集籏を認めた。PET-CTでは、嚢胞内の充実性成分に一致して高度のFDG集積亢進を認め、仙骨右側では骨浸潤を疑わせる骨融解像と結節状の集積亢進を認めた。CTガイド下針生検では有棘細胞癌を疑った。疼痛緩和と腫瘍縮小効果を期待して放射線治療を行った。疼痛は改善し、2ヵ月後のMRIで悪性腫瘍部分(仙骨浸潤部)の縮小を認めた。S3を残して全摘可能と判断し、手術を行った。病理組織学的所見で、仙骨部奇形腫内から発生し仙骨に浸潤した有棘細胞癌と診断した。現在術後1年4ヵ月で、明らかな再発所見はなく、腹部および仙骨部のヘルニア等の合併症も認めていない。

We previously reported that the early vesicle of the palmoplantar pustulosis (PPP) vesicle originated from eccrine sweat in the acrosyringium and that the PPP vesicle contains the antimicrobial peptide human cathelicidin-18/LL-37. The concentration of LL-37 was sufficient to induce the subsequent inflammation in lesions and human keratinocytes, and the PPP vesicles contained additional small fragments of human cathelicidin-18, of approximately 7 kDa, which have not been identified. The aim of the present study was to clarify the additional processed forms found in PPP vesicles and their physiological effects on normal keratinocytes and sweat gland cells. Lesional PPP vesicles were collected from PPP patients, and endogenous human cathelicidin-18/LL-37 was depleted using a LL-37 antibody affinity column. A designed recombinant human cathelicidin-18 peptide was prepared and incubated with the depleted PPP vesicle fluid to confirm the additional processed form. In-gel digestion analysis and protein sequencing confirmed the additional form as TLN58. TLN-58 up-regulated IL-17C, IL-8, IL-23, IL-1 alpha, and IL-1 beta mRNA and protein expression in normal human keratinocytes and also showed antibacterial activity against Staphylococcus aureus, Staphylococcus epidermidis, and group A Streptococcus species, similar to LL-37. This additional form could be involved in the continued inflammation in PPP lesions.

51歳女。糸球体腎炎による腎不全のため血液透析導入中、左側腹部の皮膚悪性黒色腫に対する切除術を受け、その後転居のため当院紹介となった。初診時、左乳房外側に10mm大の結節を認め、摘出術を行い、病理診断は悪性黒色腫のリンパ節転移であった。左腋窩リンパ節郭清を追加し、術後はインターフェロンβの局所注射を行った。約5年後に左胸部の皮膚転移を認め、切除術、インターフェロンβの局所注射を行った。以後定期的に経過をみているが、再発や遠位の所見はない。また、本例は希望により定期的に血清5-S-CD値の測定を行っており、10年9ヵ月の間に測定した値について検討を行った。測定回数は37回、平均値は57nmol/lであった。各測定値を紫外線曝露の多い5〜10月と紫外線曝露の少ない11〜4月で比較したところ、血清5-S-CD値は5〜10月では上昇し、11〜4月では下降する傾向がみられた。透析期間と血清5-S-CD値の関係をみたところ、ある程度の相関がみられ、11〜4月に限ると更に強い相関がみられた。リンパ節摘出および郭清時の前後では血清5-S-CD値は下降していたが、約5年後のリンパ節摘出時の前後では血清5-S-CDは上昇していた。

33歳女。主訴は脂肪吸引後の皮膚潰瘍と圧痛・灼熱感であった。美容外科クリニックで腹部・腰部の脂肪吸引術を受けた。術後2週目より臍部の吸引カニューレ挿入部・背部の硬膜外カテーテル挿入部より排膿が持続し、ドレナージを受けた。膿から非定型抗酸菌が検出され、レボフロキサシン・ミノマイシンを投与されたが、排膿と痛みが持続し、術後7ヵ月で紹介受診した。CTで腹部〜腰部の全周にわたり脂肪織内に多発する軟部陰影を認め、非定型抗酸菌症による慢性多発性膿瘍と診断した。術前にCTとエコーにより膿瘍の範囲を確認し、すべての膿瘍腔および瘻孔を開放することを目的としてデブリードマンを行い、術後16日目にさらに10ヶ所ほど補助切開を加え、膿瘍腔を一塊に摘出して開放創とした。2回の手術とも術後抗生剤としてシプロフロキサシン静脈投与を2週間行い、抗酸菌培養陰性後はミノマイシン内服に変更した。経過良好で術後48日目に退院し、術後1年で再発を認めていない。

55歳男。約11年前に左下顎犬歯部歯肉に潰瘍が出現した。白板症の診断で経過観察となった。1ヵ月前より同部に出血、疼痛を認め、悪性腫瘍を疑われた。左下顎4〜7相当部に47×30mmの腫瘤を認め、一部頬粘膜に浸潤を認め、組織学的に扁平上皮癌であった。左頸部に20mm大、右頸部に10mm大のリンパ節をそれぞれ一個触知した。左頬部皮膚を含めた下顎区域切除による腫瘍切除と左全頸部郭清術を施行した。下顎欠損に対してはチタンプレートによる再建を行った。術後12時間目頃より徐々に皮弁辺縁より出血が増加し、皮弁うっ血も認められた。開創したところ吻合部に血栓が認められた。静脈を検索している間に動脈も吻合部に血栓を形成したため、胸背動脈を頸横動脈へ再吻合し、胸背静脈は頸横静脈に再吻合した。術後11ヵ月の現在、再発を認めず、摂食可能で言語明瞭度も比較的良好な状態に回復している。

39歳、女性。初診の3ヵ月前から左膝外側に圧痛を伴う常色のしこりに気づき、2週間前から色調変化と自発痛を生じた。初診時15mm大の表面赤色調で軽度隆起する硬結を認め、生検によりEHEと診断した。局麻下に20mm離して拡大切除、全層植皮術を施行した。2.5ヵ月後、CTで鼠径および外腸骨リンパ節の腫脹を認め、鼠径部リンパ節生検にて転移と診断した。鼠径および骨盤内リンパ節郭清術施行し、外腸骨リンパ節に転移を認めたが、総腸骨リンパ節には転移を認めなかった。摘出した転移リンパ節から薬剤感受性試験行い、Ifosfamid(IFM)の腫瘍増殖抑制率が84.4%と高感受性であったため、IFM、Doxorubicin hydrochloride(DXR)による化学療法を5クール行い、最終術後3年11ヵ月再発転移を認めていない。皮膚に生じるEHEは稀で、若干の文献的考察を加えて報告する。(著者抄録)

Kasabach-Merritt症候群を併発したtufted angiomaの3例を経験した。症例1は生後9日男児で、出生時より右側胸部に紅斑を伴う皮下硬結を認めた。病理組織検査所見よりtufted angiomaと診断した。プレドニゾロン、プロプラノロールを投与した血小板数は安定せず、選択的動脈塞栓術を行った。しかし、腫瘍は再増大し、放射線電子線治療を行った。症例2は2ヵ月女児で、生後47日目に右大腿に紅斑が出現し、徐々に硬結部が隆起した。病理組織所見よりtufted angiomaと診断した。プレドニゾロンにより血小板数は徐々に増加した。CTで腫瘍の縮小傾向を認めた。症例3は生後2日女児で、出生時より左大腿後面に暗赤色の硬い硬結を認めた。臨床症状と病理所見よりtufted angiomaと診断した。プレドニゾロンにより血小板数は急速に増加した。腫瘍は再燃なく消退傾向を認めた。

症例1は21歳女性で、仙骨部左側の腫瘤が増大し、近医にて動静脈奇形(AVM)を疑われ紹介受診した。同部に40×30mm大の拍動ある軟腫瘤を触知し、血管造影で左内腸骨動脈の枝を栄養動脈とする血管塊を認めAVMと診断した。放射線科に流入動脈の経血管的塞栓術を依頼し、2日後に経血管的流出静脈制御下にて経皮的硬化療法を行った。2回の硬化療法でAVMは著しく縮小し、皮下と臀筋内の残存AVMを切除した。術後6年余経過で再発は認めていない。症例2は30歳女性で、10年前より左臀部に有痛性腫瘤を自覚し、他院にてAVMを疑われ紹介受診した。左臀部に140×120mm大の拍動ある腫瘤を触知し、血管造影・造影3D-CTで内腸骨動脈の枝を栄養動脈とする血管塊を認めAVMと診断した。症例1同様の硬化療法を4回行うも効果に乏しく、大腿後面の流出静脈末端部を外科的に結紮し、無水エタノールを病巣内に注入した。治療後1年経過でAVMの消失を確認した。

20生日,男児.生下時より右足関節外側に紅色腫瘤を認め,血管腫疑いにて紹介され受診した.右足関節に30mmの内部が紅色調で,周囲が常色〜淡青色で多毛を伴う皮膚腫瘤を認めた.病理組織像では,真皮全層にわたって組織球様細胞と泡沫細胞が浸潤し,Touton型巨細胞が混在していた.免疫組織染色にて腫瘍細胞はビメンチン,CD68,α1アンチトリプシン陽性,S100蛋白陰性で,黄色腫細胞と考えられた.生後10ヵ月の時点で著明に増大したが,1歳10ヵ月には淡褐色軟腫瘤となり縮小傾向がみられた.2歳10ヵ月時では浸潤を触れる橙色局面となり,3歳10ヵ月時ではほとんど浸潤を触れなくなった.本邦報告例中最大級の腫瘤像を呈した単発型若年性黄色肉芽腫の1例であるとともに,全切除をすることなく長期間経過観察できた症例である.巨大若年性黄色肉芽腫の場合でも自然消退を期待して経過観察することが大切である.(著者抄録)

皮膚悪性腫瘍の化学療法は、いまだ確立された方法は少なく、特に悪性黒色腫以外の悪性腫瘍では症例報告や少数例の報告が多く、多施設での大規模な研究報告はない。しかし、本邦でも術後補助療法から進行期症例に対する集学的治療の1つとして様々な薬剤、方法が用いられている。抗癌剤治療にあたっては各腫瘍に対して有効な薬剤やレジメンを知ることは重要であるが、治療開始前には患者の病状と全身状態を把握し、抗癌剤治療に対する患者の同意を得ることが大切である。そして、治療が始まってからも全身状態や副作用をチェックし、一定の期間で抗癌剤の効果を評価し、治療継続か否かを決定しなければならない。本稿では抗癌剤の種類や機序などの基本的な知識と、実際の使用方法について解説する。(著者抄録)

82歳、女性の鼻部から右内眼角に及ぶmorpheaformの基底細胞癌(BCC)を報告した。病巣部切除後、全層植皮術を行った。術後2年4ヵ月以後は来院がなく、97歳、14年半ぶりの再診時には、再発腫瘍が眼瞼ならびに眼窩に浸潤していた。CT画像上、眼窩脂肪織や周囲骨組織への浸潤所見を認めた。軽度認知症を伴う超高齢者故に、眼窩内容除去術を行わずに、姑息的治療で経過を追っているが、最近のCT、MRI画像上では、脳硬膜、右海綿静脈洞、斜台、ほかへの浸潤所見を認めている。眼周囲のBCCでは、再発が少なからず起こるので、密な間隔で、長期的な追跡が必要である。(著者抄録)

当科で手術治療を行った毛細血管奇形患者42例を調査した。年齢は10ヵ月から81歳で、手術部位は頭頸部37例、上肢1例、下肢4例であった。手術治療はレーザー治療抵抗性や隆起・増殖性変化や組織肥大を呈した症例に対して行った。病変切除後に直接縫合が困難な場合には、全層植皮術が再建方法の主体となっていた。毛細血管奇形患者は色素レーザー治療により良好な結果が得られるようになったが、治療抵抗性の症例、隆起・増殖性変化、組織肥大をきたすものがわずかながら存在する。毛細血管奇形のレーザー治療においては定期的な評価を行い、手術治療の必要性も検討する必要があると考えられた。(著者抄録)

10ヵ月女児。出生時より胸壁・腹壁破裂がみられ、心臓心尖部が脱出しており、心臓還納術と胸腹壁再建術を施行された。その際、心臓を固定するため欠損する心外膜、胸骨下部をゴアテックスシート(A)で被覆し、左右の前胸部皮弁を伸展して閉創された。生後10ヵ月時に胸部正中創が離開し、Aが露出して難治性潰瘍となり当科紹介された。全身麻酔下に潰瘍周囲の菲薄した皮膚を切除し、A周囲の肉芽組織を切除した後、下腹壁動脈からの傍臍穿通枝が含まれるように7×15cmの局所皮弁を右外腹斜筋上に作成し、欠損部に移動し縫合した。術後2日目より部分壊死を生じ小瘻孔を形成し、外来通院で処置を継続したものの創部培養でMRSAが検出され、潰瘍部が徐々に拡大して術後56日に再入院となった。潰瘍底にAが露出しており、全身麻酔下に手術を施行してAを摘出したところ、心臓側に薄い線維性皮膜が新生し、心外膜としての支持性が確認できたため新しいAは挿入しなかった。上方茎の腹直筋皮弁3×4cmを作成して潰瘍を被覆し、その際に胸壁を補強するために大きく採取した腹直筋筋膜前鞘を潰瘍底周囲の外腹斜筋と大胸筋筋膜周囲組織に縫合した。術後経過は良好で、術後10日に軽快退院し、8ヵ月経過の現在、再発は認めていない。

32歳男。6年前より左上背部の皮下腫瘤を自覚し、近医でリンパ管奇形(LM)を疑われた。腫瘤は13×8cm、弾性軟で、表面に水疱様の丘疹・結節が集簇しており、下床との可動性は良好であった。血液生化学検査で異常はなく、CTで分葉状の扁平な軟部陰影を認め、MRIではT1強調像で低信号、T2強調像で高信号を示す多房性嚢胞性病変の像であった。LMと考え、間質内へのポリドカノール局注を計3回行ったが変化はなく、受診が一旦中断された。初診1年半後の再診時、大きさは18×12×3cmと増大し、弾性硬で、表面の丘疹・結節は増加して毛細血管拡張を伴っていた。確定診断を兼ねて切除術を施行し、病理組織所見で真皮から皮下脂肪織にかけて線維性中隔に囲まれた大量の粘液状基質を認め、その中に異型性のない紡錘形細胞、星芒状線維を認めた。粘液はヒアルロン酸であった。免疫染色ではS-100陰性、D2-40陽性のリンパ管は少数で、superficial angiomyxomaと診断した。術後7ヵ月経過して再発はない。

Nodal lymphangiogenesis promotes distant lymph node (LN) metastasis in experimental cancer models. However, the role of nodal lymphangiogenesis hi distant metastasis and in the overall survival of cancer patients remains unknown. Therefore, we investigated mechanisms that might facilitate regional and distant LN metastasis in extramammary Paget's disease (EMPD). We retrospectively analyzed the impact of tumor-induced lymphatic vessel activation on the survival of 116 patients, the largest cohort with EMPD studied to date. Nodal lymphangiogenesis was significantly increased in metastatic, compared with tumor-free, LNs (P = 0.022). increased lymphatic invasion within regional LNs was significantly associated with distant metastasis in LN (P = 0.047) and organs (P = 0.003). Thus, invasion within regional LNs is a powerful indicator of systemic tumor spread and reduced patient survival in EMPD (P = 0.0004). Lymphatic vessels associated with tumors expressed stromal cell-derived factor-1 (SDF-1), whereas CXCR4 was expressed on invasive Paget cells undergoing epithelial-mesenchymal transition (EMT)-like process. A431 cells overexpressing Snail expressed increased levels of CXCR4 in the presence of transforming growth factor-beta 1. Haptotactic migration assays confirmed that Snail-induced EMT-like process promotes tumor cell motility via the CXCR4-SDF-1 axis. Sinusoidal lymphatic endothelial cells and macrophages expressed SDF-1 in subcapsular sinuses of lymph nodes before Paget cell arrival. 硬化療法を施行した手指の血管奇形7例7手(男2例、女5例、年齢5〜21歳)について検討した。Venous malformation(VM)6例(切除後再発3例)およびArteriovenous malformation(AVM)1例であった。手術方法は、内用液を吸引した後に硬化剤0.2〜0.5ccを注入し病変内に約1分留置して再度内用液を吸引する操作を病変部が硬くなるまで2〜3回繰り返した。使用薬剤は10%オレイン酸モノエタノールアミン(EO)を2例に、無水エタノール(Et)を5例に1%ポリドカノール(PO)を1例に造影剤で希釈して用いた。VMの6例中4例は著明に縮小し、やや縮小が2例で、AVM例は疼痛が消失しやや縮小を認めた。合併症はEO使用の1例に皮膚の部分壊死、Et使用の2例に一過性の皮膚知覚異常を認めた。硬化療法は摘出困難例や術後再発例にも有効であった。使用薬剤は1回の注入量が0.5mlを超えない範囲で数回に分けて行うことで合併症を軽減できると考えた。

50歳,男性.愛媛県松山市内の山中で12月に3週間ほど野宿をしていて,両下腿凍傷を受傷した.初診時凍傷の亜急性期であり,高度脱水による腎不全も認めた.入院し全身管理と並行してプロスタグランディン製剤の静注,スルファジアジン銀クリーム塗布により保存的治療を行った.組織の壊死は骨まで及び,広範囲な骨髄炎を併発したため,入院22日目に両膝下での切断を行った.術後はリハビリにより両義足,T字杖歩行で長距離歩行も可能になり,早期に退院した.比較的温暖な地域においても条件によっては重度の凍傷が発生することがあり,凍傷の病態,治療の知識が必要であると認識させられた症例であった.(著者抄録)

症例は男児で、2歳時に右眼球を損傷し、ただちに当院眼科で眼球内容除去術が行われ、義眼台としてシリコンボールが挿入された。8歳時の定期診察で義眼台の露出が認められ、義眼床の再建目的で当科に紹介された。CT検査で義眼台周囲の強膜を含む軟組織が著明に肥厚し後退しているのが認められ、人工物を用いた再建方法では再露出の危険性が高いと考えられた。また、若年であるため手術侵襲を出来るだけ少なくすることを考慮し、再建方法は強膜内への真皮脂肪移植術を選択した。手術は全身麻酔下に行い、術後1ヵ月のMRIで真皮脂肪の生着が確認された。術後4ヵ月のMRIで移植片の萎縮を認め、義眼台の大きさが若干縮小していたが、臨床的には十分な容量が保たれており、術後1年6ヵ月のMRIで萎縮の進行は認めなかった。術後2年の現在、義眼床は十分な広さで柔らかい状態を保っており、義眼の装着状態も良好である。

50例の唇顎口蓋裂患者に行った,骨鋭匙を用いた低侵襲腸骨海綿骨採取法(低侵襲法)の手術手技について述べた.また,それ以前に行っていた腸骨稜を大きく開窓する方法(従来法)32例と術後入院期間,術後補助なしで歩行可能となるまでの期間,鎮痛薬使用回数を比較した結果,いずれも低侵襲法が優れていた.従来法の合併症として肥厚性瘢痕,大腿部の知覚鈍麻,腸骨の再骨折などが認められたが,低侵襲法では瘢痕以外は認められなかった.本法では,小範囲とはいえ,腸骨稜骨端軟骨に侵襲が及ぶため,成長に伴う骨変形が危惧されるが,最長6年の経過観察期間では腸骨の変形は軽度であった.本法は唇顎裂口蓋裂患者の二次的海綿骨移植術に有用と考えた(著者抄録)

26年間に経験したin situ例をのぞく皮膚悪性黒色腫の頭部・顔面例11例とその他の95例を対象に比較検討した.過去には5cmの切除縁で切除したこともあったが,最近の約10年間では,比較的狭い切除を行った.stage II以上では所属リンパ節郭清術を行った.また,原則としてstage II以上の症例には,化学療法または免疫療法を行った.頭部・顔面例では,上記の切除範囲の根治治療を行ったもの(根治群)が6例で,局所麻酔または全身麻酔下に単純切除+化学療法などの姑息的治療を行ったもの(姑息群)が5例であった.その他例では,初診時に遠隔転移を認めたstage IVの6例をのぞいたstage IIIまでの89例中,根治群が82例で,姑息群が7例であった.頭部・顔面例では,その他例にくらべ腫瘍死率が低く,他病死率および不明率が高かった

頸部に発生する先天的皮膚瘻としては,正中頸瘻・側頸瘻(嚢腫・嚢胞を含む)がよく知られているが,今回,これらの疾患とはやや異なる胸骨上窩とその近傍に発生した先天性皮膚瘻(胸骨上窩皮膚瘻)の2症例(1歳5ヵ月女児2例)を経験した.この自験例2例を呈示するとともに,これまでの日本での胸骨上窩皮膚瘻の報告例(自験例を含む20症例)の分析,疾患の名称,鑑別疾患などについて考察した.胸骨上窩皮膚瘻は日本においては稀であるが,発生学的・病理組織学的に胸骨上窩の先天性皮膚瘻を独立した1疾患とみなすべきか否かについて今後検討する必要があると考えられた

Two rare cases of congenital skin fistula on the suprasternal area were described, followed by an analysis of similar disorders in the Japanese populace at large. Histologically, each fistula was surrounded by stratified squamous epithelium associated with a hair follicle and/or sebaceous gland. Fistulas on the suprasternal area do not seem to be rarer than expected, although the reported cases in the literature were remarkably limited. Whether or not these skin fistulas constitute an independent disorder from lateral or median cervical fistulas, cutaneous abnormalities with sternal clefts, or dermoid fistulas on the suprasternal notch, is a question that remains open to discussion.

51歳女.約2年前に左上顎癌(T4N0M0)の診断で左眼瞼・眼窩内容を含む左上顎全摘術を施行され,術後は疑顎を使用しガーゼで眼窩部の欠損をカバーして生活していたが,上顎・眼窩の充填と義眼床形成術を希望して紹介来院した.第1回手術では右遊離腹直筋皮弁移植により上顎から眼窩の充填を行うとともに眼窩皮膚・鼻腔内側壁・口蓋の再建を行った.手術9ヵ月後に移植した腹直筋皮弁内にポケットを作成し全層植皮術による義眼床形成術を施行したが,術後1ヵ月後に感染のため義眼床内コンフォーマーの留置が困難となり植皮片が高度拘縮をきたし,義眼の装着が不可能となった.第2回手術から9ヵ月後に全層植皮術に加え,上下眼瞼結膜側へ耳介舟状窩よりの皮膚軟骨複合組織移植を行い,再度義眼床の形成術を施行した.術後約2年の現在,腹直筋皮弁が萎縮し頬部陥凹による非対称性は認めが,義眼床と義眼の装着状態は良好で眼帯を使用せずに色の薄いサングラスを装用して社会生活に復帰している

上肢に発生した悪性黒色腫(MM)21例,扁平上皮癌(SCC)33例,基底細胞癌(BCC)1例,悪性軟部腫瘍(MSTT)13例,大腸原発の平滑筋肉腫皮膚転移1例について分析,検討した.MMは手部,特に爪部に多く認められ,切除1例,切除+植皮6例,指切断を13例に施行した.5例が腫瘍死した.SSCの平均年齢は73.1歳であり,切除を10例,切除+植皮を16例,切除+皮弁1例,指切断を5例,前腕切断を1例に施行し,予後は良好であった.BBCは71歳男性の前腕に発生しており,切除縫合を行った.MSTTは40〜50歳代に多く,前腕部〜上腕に多くみられた.内訳は,悪性線維性組織球腫6例,隆起性皮膚線維肉腫4例,脂肪肉腫2例,平滑筋肉腫1例で,切除を5例,切除+植皮6例,切除+皮弁を1例,切除生検を1例に行った.他病死1例を除き,全例生存中である

75歳、男性。陰茎癌術後、陰嚢にリンパ浮腫を認めていた。陰嚢の腫脹、外尿道口周囲の硬結を主訴に当科紹介受診。初診時、陰嚢表面には出血および紫斑を混じていた。硬結部の病理組織は、angiosarcomaの所見であった。治療は放射線療法を選択し、計45Gy照射後、陰嚢の潰瘍、紫斑は完全に消退した。Angiosarcomaは体中どこにでも生じうるが、外陰部に発生したangiosarcomaは報告例が少なく極めて稀である。陰嚢に発生した誘因として、術後リンパ浮腫を生じていたことからStewart-Treves症候群として発症した可能性と、転倒後に陰嚢の腫脹、硬結を生じていたことから外傷を契機に発症した可能性が考えられた。(著者抄録)

A 64-year-old woman developed plaques on her trunk and extremities that began developing at the age of 54 years. She consulted a dermatologist and was treated by topical steroid ointment on the diagnosis of discoid lupus erythematosus. An ulcerated tumor had developed on a previously existing plaque on the right lower leg and was first noticed 2 months before we saw her. The tumor was diagnosed as a squamous cell carcinoma (SCC) by biopsy. The tumor was removed with a surgical margin of 1-2 cm. Histopathology of the plaque showed remarkable hyperkeratosis and papillomatosis with band-like lymphocytic infiltration, which was diagnosed as hypertrophic lichen planus (HLP). Although it is a rare disease, we should carefully follow up patients with HLP because SCC can develop on the lesion.

A 43-year-old woman had noticed a brownish spot on her left forehead for 10 years, and a crust on it for 1 year. A bluish macule, 10×15 mm, was observed. Preauricular and neck lymph node swelling was not detected. Biopsy revealed basal cell carcinoma (BCC), and the lesion was resected with a 5 mm margin. Histologically, a micronodular pattern of BCC was revealed, but no vessel or nerve invasion was seen. Residual cancer was suspected, and 1 year and 4 months after the first surgery, we found recurrence histologically. We performed wide resection and skin graft, and no residual cancer was detected histologically. We found no recurrence or metastasis for one year and 3 months after the second surgery, but the patient did not return for follow-up during the next 5 years. She suddenly came back to us presenting a left cheek tumor. We found an elastic hard tumor, 40×45 mm, on her left parotid area and diagnosed metastatic BCC histologically. We excised the tumor together with the masseter muscle, periosteum of mandible and parotid gland
however, residual cancer was suspected around the jaw joint, so we added radiation therapy. A histological micronodular pattern of BCC belongs to a category of clinical infiltrative subtypes and is recognized as aggressive BCC with a poor prognosis. We think that this patient has provided us with some very instructive lessons.

苺状血管腫に対する早期短パルス色素レーザー治療について報告した。対象は2000年3月〜2005年3月までに治療した苺状血管腫55例63部位(男児21部位、女児42部位)で、治療は短パルス色素レーザーを照射し、6ヵ月以上経過時点で、その成績を判断した。1)照射エネルギー密度は平均6.37J/m2(4.5〜7.5J/m2)で、治療回数は平均4.0回(1〜23回)、治療面積は数mm〜上肢全体、治療時間は最長5分前後、治療頻度は2〜4週に1度であった。2)治療結果はExcellent:17部位、Good:15部位、Fair:20部位、Poor:11部位であり、有効率は51%(32/63部位)であった。3)治療開始時期や治療部位による有効率に差は認められなかった。だが、照射エネルギー密度別の有効率では6.0J/m2以上:61%(30/49部位)、6.0J/m2未満:14%(2/14部位)であり、また治療回数別では3回以上:64%(29/45部位)、3回未満:17%(3/18部位)と差がみられ、更に照射エネルギー密度6.0J/m2以上でかつ3回以上照射:77%(27/35部位)、それ以外:28%(5/18部位)といずれも有意差が確認された。

A 41-year-old man noticed painful erythema on his right lower abdomen. Similar lesions, discharging pus, appeared subsequently over his bilateral lower extremities and axilla during the last two years since his initial onset. Therefore, we diagnosed this patient as ectopic pyoderma chronica. Consequent surgical resection of those lesions successfully introduced a significant improvement towards this patient. However, a differential diagnosis of pyoderma gangrenosum, which can be comparable both by clinically and histologically with pyoderma chronica, initially led us to the standard regimens for pyoderma gangrenosum : i.e. oral betamethasone, salazosulfapyridine, cyclosporine
those medications failed to provide remarkable benefit for this patient.

49歳女.交通事故で右足を巻き込まれ,皮下組織にまで至る挫創を受傷した.保存的処置で4週間後にはほぼ上皮化したが,同部が肥厚性瘢痕となった.痒みと痛みが著明であった.初期からアンテベート軟膏の外用を行い,外的刺激の除去と患部の圧迫目的にサポーターの装着を行った.6週間後には瘢痕はやや平坦化・白色化し,自覚症状も改善し,ストロングランクのステロイド軟膏に変更した

A 79-year-old Japanese man visited our hospital on October 1, 2002, because of a right axillar ulcer. In 1989 he had noticed a small nodule on the right axillar region. Cryosurgery with liquid nitrogen had been performed several times for the nodule, resulting in the formation of an ulcer. Three months prior to this visit, the growth rate of the lesion had increased. The ulcer was 6 × 5 cm in size and biopsy revealed basal cell carcinoma (BBC). There were regional lymph node metastasis and skin metastasis in his axilla. The primary tumor and metastasis were excised and covered with a split-thickness skin graft. The histological appearance of both the lymph nodes and cutaneous metastasis was the same as that of the primary tumor. Based on pathological examination, a diagnosis of metastasizing BCC was made. Four months after treatment, local recurrence was observed. However, 14 months after re-excision, there has been no recurrence.

過去21年間に11例の筋層間及び筋肉内脂肪腫を経験した.筋層間脂肪腫は広背筋下3例,僧帽筋下2例及び三角筋下,足底腱膜下,小指球筋層間の各1例に見られた.1例においては,3個の筋層間脂肪腫が棘下筋下に見られ,大きな皮下脂肪腫を合併していた.11個の筋層間脂肪腫のうち,5個は多葉性の発育を示した.一方,筋肉内脂肪腫は2例に見られた.即ち,左大腿内側広筋内の境界明瞭な1例と左下肢全体に及ぶdiffuse lipomatosisの1例(巨趾症を合併)を経験した.多くの筋層間或いは筋肉内脂肪腫は緩徐な発育を示す,無痛性の腫瘤で,しばしば筋肉の収縮時に明瞭になる.CT,MRIはこれらの脂肪腫の診断や局在部位の評価には極めて有益な検査である

新鮮瘢痕,肥厚性瘢痕,成熟瘢痕組織の真皮線維芽細胞の増殖活性からみた創傷治癒過程を検討した.瘢痕組織は,いずれにおいても真皮線維芽細胞密度は高かったが,その増殖活性は正常組織と同等もしくはそれより低値であった.つまり,瘢痕組織は,一時的に皮下硬結,隆起性病変を形成しても,その増殖活性が低い為に,やがては柔軟で扁平な正常皮膚に類似した状態に落ちつくという臨床経過とかなり一致すると考えられた.更に,新鮮瘢痕の時期に既に,肥厚性瘢痕を形成するものと,形成しないものが決定されている可能性も示唆された
