    A rectovaginal fistula (RVF) is an abnormal tract between the rectum and vagina, which requires surgical intervention in many cases. Although there are many different therapeutic approaches for RVF depending on the patient's' condition, there are no established guidelines for the care of RVF. This study aimed to evaluate the results of laparoscopic colostomy in advanced cancer patients with RVF, and the safety and efficacy of this surgery. In this study, seven female advanced cancer patients with RVF were hospitalized and successfully treated with laparoscopic colostomy from 2015 to 2018 at our university hospital. Their data were retrospectively evaluated from their medical records. The early use of diverting stomas facilitated timely resumption of cancer treatment and enabled early treatment with chemotherapy or radiotherapy. Although vaginal stool leakage affected three patients, all patients recovered, experiencing neither pain nor infection during their cancer treatment. While colostomy was physically and mentally taxing for the patients, it improved the infection and pain caused by the RVF. We conclude that the early use of diverting stomas had two effects: a significant improvement in infection management and facilitation of the rapid resumption of cancer treatment.

    There are many reports on the positional relationship between the ileocolic artery and superior mesenteric vein (SMV). However, there have been no reports of anomalous venous confluence in the ileocecal vessel area. A 69-year-old man was diagnosed with cecal cancer on a preoperative examination of a lung tumor. We planned to perform surgery for the cecal cancer. Computed tomography angiography revealed an anomalous vein confluence in the ileocolic region. We performed robot-assisted ileocecal resection. Although the small intestinal vein was misidentified as the SMV at first, we confirmed the misidentification, identified the SMV on the dorsal side of the ileocolic artery, and ligated the ileocolic vessels with precise forceps manipulation during robotic surgery. Especially for cases with vascular anomalies revealed by preoperative computed tomography angiography, robotic surgery may be useful, as flexible forceps manipulation prevents vascular injury.

    BACKGROUND: Complete mesocolic excision (CME) and central vascular detachment are very important procedures in surgery for colorectal cancer. Preoperative and intraoperative assessments of the anatomy of major colorectal vessels are necessary to avoid massive bleeding, especially in endoscopic surgery. A case with a rare anomaly in which the middle colic artery (MCA) and ileocolic artery (ICA) had a common trunk is reported. CASE PRESENTATION: The patient was a 73-year-old woman diagnosed with ascending colon cancer on colonoscopy. Preoperative abdominal contrast-enhanced computed tomography confirmed that the MCA and ICA had a common trunk. She underwent laparoscopic ileocecal resection for the ascending colon cancer with D3 lymph node dissection. Intraoperative indocyanine green fluorescence imaging was conducted. After confirming vessel bifurcation, the ICA was dissected at the distal end of the MCA bifurcation. The patient has been followed as an outpatient, with no signs of recurrence as of 2 years postoperatively. CONCLUSION: A case of an ascending colon cancer with a unique vascular bifurcation pattern was presented. Preoperative and intraoperative evaluations of the major colorectal vessels are very important for preventing perioperative and postoperative complications.

    BACKGROUND: Laparoscopic and robotic surgery for transverse colon cancer are difficult due to complex fusion of the foregut and midgut and variation of the vessels of the transverse colon. Although the vessels of the right colon have been investigated, middle colic artery (MCA) variation and the relationship with vessels around the transvers colon are unknown. We investigated variation of the MCA using computed tomography angiography (CTA) and cadaver specimen and the relationship between the superior mesenteric vein (SMV) and MCA using CTA. The classification of vessels around the transverse colon may lead to safer and reliable surgery. METHODS: This study included 505 consecutive patients who underwent CTA in our institution from 2014 to 2020 and 44 cadaver specimens. Vascular anatomical classifications and relationships were analyzed using CT images. RESULTS: The MCA was defined as the arteries arising from the superior mesenteric artery (SMA) that flowed into the transverse colon at the distal ends. The classifications were as follows: type I, branching right and left from common trunk; type II, the right and left branches bifurcated separately from the SMA; and type III, the MCA branched from a vessel other than the SMA. Type II was subclassified into two subtypes, type IIa with one left branch and type IIb with two or more left branches from SMA. In the CTA and cadaver studies, respectively, the classifications were as follows: type I, n = 290 and n = 31; type IIa, n = 211 and n = 13; type IIb, n = 3 and n = 0; and type III, n = 1 and n = 0. We classified the relationship between the MCA and left side of the SMV into three types: type A, a common trunk runs along the left edge of the SMV (n = 173; 59.7%); type B, a right branch of the MCA runs along the left edge of the SMV (n = 116; 40.0%); and type C, the MCA runs dorsal of the SMV (n = 1; 0.3%). CONCLUSIONS: This study revealed that The MCA branching classifications and relationship between the SMV and MCA. Preoperative CT angiography may be able to reliably identify vessel variation, which may be useful in clinical practice.

  • Efficacy of Coagulofibrinolytic Markers for Postoperative Prediction of Venous Thromboembolism in Colorectal Surgery Patients: A Retrospective Observational Study.

    OBJECTIVES: A low rate of the incidence of venous thromboembolism (VTE) after surgeries that are preoperatively classified as having high risk of VTE has been reported in recent years. We seek to identify the optimal cases to receive perioperative pharmacologic thromboprophylaxis. In this study, we evaluated the incidence rate of VTE among patients undergoing colorectal surgery who did not receive perioperative pharmacologic thromboprophylaxis, and the ability of coagulofibrinolytic markers to predict the postoperative development of VTE. METHODS: We retrospectively analyzed the rate of postoperative development of VTE in 70 patients undergoing elective colorectal surgery without perioperative pharmacologic thromboprophylaxis and the ability of coagulofibrinolytic markers to predict the development of VTE. RESULTS: The incidence of VTE was observed in 11 patients (15.7%); all cases were asymptomatic and distal-type deep vein thrombosis (DVT). Comparisons of time course changes in perioperative coagulofibrinolytic markers between patients with and without DVT revealed significant differences in soluble fibrin (SF), thrombin-antithrombin complex (TAT), fibrin/fibrinogen degradation product (FDP) and D-dimer. Dynamic postoperative physiological coagulofibrinolytic responses were shown, but all four markers at each postoperative point demonstrated moderate accuracy (median area under the curve [AUC]: 0.788, median sensitivity: 0.865, median specificity: 0.644) for predicting the development of DVT. CONCLUSIONS: The incidence of postoperative VTE was low in patients with colorectal surgery even in those who did not receive perioperative pharmacologic thromboprophylaxis. SF, TAT, FDP and D-dimer were useful for predicting the development of DVT when we set cut-off values taking the physiological perioperative coagulofibrinolytic responses into consideration.

    PURPOSE: Few studies have reported on the interactions between gastrectomy and antithrombotic therapy, especially the long-term prognosis. We aimed to clarify the short- and long-term prognosis of gastrectomy for patients on antithrombotic therapy. METHODS: We reviewed the perioperative data and survival rate of patients who underwent laparoscopic distal gastrectomy (LDG) at our institute between 2010 and 2013. RESULTS: There were 119 patients enrolled in this retrospective study: 31 who were taking antithrombotic drugs (antithrombotic therapy (ATT) group), and 88 who were not (non-ATT group). The mean age was significantly higher in the ATT group than in the non-ATT group. No significant differences were observed in the amount of intraoperative bleeding or blood hemoglobin level after surgery between the groups. Bleeding complications occurred in only one patient from the ATT group, and the postoperative complication rate was comparable between the groups. During follow-up, cerebrovascular or cardiovascular events developed in 19.4% of the ATT group patients and 4.5% of the non-ATT group patients; however, there were no significant differences in the 5-year overall survival rates between the groups (ATT group, 76.9%; non-ATT group, 82.9%). CONCLUSIONS: Antithrombotic therapy did not affect the short-term or long-term prognosis of patients after LDG.

    症例は71歳男性。胃潰瘍の既往歴があり、黒色便と貧血の精査で紹介受診した。来院時に嘔吐を認め、その後胸痛と呼吸苦が出現した。胸腹部造影CTで縦郭気腫を広範囲に認め、下縦郭で食物残渣の食道外への漏れを伴っていた。胃内には出血と思われる高濃度域を伴う液体貯留を認めた。特発性食道破裂、縦郭限局型に加えて上部消化管出血と診断し、経口内視鏡を併用した腹腔鏡下食道破裂修復術を施行した。経口内視鏡による観察で胃潰瘍と出血点を認め、クリップと高張ナトリウム・エピネフリン法で止血した。食道には左壁に縦走する裂創と筋層外膜に至る穿破を認めた。無送気で裂創部をクリッピングし、粘膜と粘膜下層を閉鎖した。その後腹腔鏡操作を行い、破裂した外膜筋層を縫合閉鎖した。縦郭内にドレーンを留置し閉創した。これらの治療により救命できた。

    PURPOSE: During the perioperative period, coagulofibrinolytic activation occurs, which occasionally results in thromboembolic complications. However, natural perioperative coagulofibrinolytic responses have not been well investigated. The present study examined perioperative coagulofibrinolytic changes and their association with the development of venous thromboembolism (VTE). METHODS: We retrospectively analyzed the changes in coagulofibrinolytic markers for 7 days in 70 patients undergoing elective colorectal surgery. To explore the natural coagulofibrinolytic response, we investigated patients not undergoing perioperative chemical thromboprophylaxis. RESULTS: Coagulation activation occurred from just after surgery to postoperative day (POD) 1, followed by a gradual decrease, but persisted to even POD 7. Fibrinolytic activity showed a tri-phasic response: activation, shutdown and reactivation. Consequently, fibrin/fibrinogen degradation product (FDP) and D-dimer levels continued to increase until POD 7. The development of deep vein thrombosis (DVT) was observed in 11 patients (15.7%). Postoperative sustained hyper-coagulation [soluble fibrin (SF) or thrombin-antithrombin complex (TAT) values on POD 7 > their normal limits] was significantly associated with the development of DVT (SF, p < 0.001; TAT, p = 0.001). CONCLUSION: We found initial coagulation activation and a tri-phasic response of fibrinolytic activity after colorectal surgery. Thus, physicians need to pay attention to these responses when attempting to prevent or treat VTE.

    Indocyanine green (ICG) fluorescence angiography has recently been reported useful as a method for predicting intestinal blood flow and may reduce anastomotic leakage. However, the quantification method for ICG fluorescence angiography has not been established. We usually measure the tissue oxygen saturation (StO2 ) in the intestinal tract via near-infrared spectroscopy, as it is able to measure the oxygen concentration accurately and immediately shows objective data. In this study, we propose that the time to reach the anastomotic site after intravenous ICG injection is an effective parameter for quantifying ICG fluorescence angiography from the comparison to the data of StO2 in the intestinal tract.

    BACKGROUND: Recently, aging in patients with severe motor and intellectual disabilities (SMID) has been causing serious problems. There have been few reports about treatment for tumors in patients with SMID. METHODS: The treatments performed for tumors of 12 SMID patients were examined. RESULTS: Blood tests and ultrasonography were useful in screening. With regard to treatment, the surgery for SMID patients was performed in the same manner as in healthy cases, and the results were generally satisfactory, without major complications. Typically, the patients were diagnosed in advanced stages; many metachronous double cancers were observed. CONCLUSIONS: We reported a tumor therapy to the patients with SMID. Our therapy provides satisfactory results for patients with SMID and their families. Further studies are required to clarify the clinical significance of the screening test and the tumor operative method for the patients with SMID.

    Local recurrence of rectal cancer is defined as any evidence of relapse within the small pelvis after surgical resection of the primary tumor. After removal of a rectal tumor, necrotic cellular debris and viable exfoliated cancer cells are present in the rectal lumen
    using saline or povidone-iodine solution, many surgeons perform rectal washout beyond an occlusive clamp to remove these exfoliated malignant cells, which may lead to local recurrence. A meta-Analysis showed that intraoperative rectal washout at the surgical anastomosis site can reduce the risk of local recurrence after rectal cancer resection. Therefore, intraluminal lavage is useful to prevent local recurrence. Although intraoperative rectal washing is considered to be important, there is probably room for improvement on this method. We described the idea of a new transanal irrigation and aspiration system for small rectal contents. The aim of this research was to investigate the effect of a new rectal washout system using a chemiluminescent imaging system. We attempted to evaluate and compare the washout efficiency between this new system and the conventional irrigation system using a cylindrical rectum model. Our results implied that, at the same volume of solution, the irrigation-Aspiration rectal washout system was more effective than the irrigation washout system. This new rectal washout system was easy to use and allowed a good washout. To the best of our knowledge, this was the first report that evaluated the efficacy of a rectal washout system using quantitative data.

    Wilms tumor (WT) is the most common malignant kidney tumor in children. High blood pressure is seen in up to 55% of children with WT. However, hypertensive cardiomyopathy with congestive heart failure due to WT is remarkably rare, with only several cases reported worldwide. In this report, a pediatric case of WT with hypertension causing hypertensive cardiomyopathy and congestive heart failure is presented. An 8-month-old male child with abdominal distension was seen by his primary physician. He was referred to our hospital for further examination and treatment. Abdominal contrast-enhanced computed tomography demonstrated a weakly enhancing, large abdominal mass, which was larger than 12 cm. Two-dimensional transthoracic echocardiography showed a diffuse hypokinetic left ventricle. The patient was diagnosed with cardiomyopathy caused by hypertension. Open surgical resection of the mass was successfully performed. His postoperative course was uncomplicated, and the patient was successfully discharged. The plasma renin activity was maintained at a high level even after left nephrectomy, suggesting that the right kidney was likely the source of renin secretion. Mechanical compression of the right renal blood vessels by a greatly enlarged left kidney can cause right renal ischemia, which activates renin excretion. Nephrectomy can be an effective treatment for a WT patient with hypertension causing hypertensive cardiomyopathy, and then cardiac function will be improved within several weeks. We recommend routine echocardiography surveillance in patients with WT. This report can help pediatric surgeons become more familiar with cardiomyopathy caused by WT.

    Rubinstein-Taybi syndrome is an extremely rare autosomal dominant genetic disorder that occurs in 1/125,000 and is characterized by distinctive facial appearance, short stature, mild to severe mental retardation, and higher risk for cancer. In addition, variable organ anomalies had been reported. Paraovarian cyst causing torsion of the ipsilateral fallopian tube is less common, with an estimated incidence of 1/1,500,000, but it can adversely affect tubal function. It occurs mainly in women in the reproductive age and is very rare in prepubescent girls. Here, we described the successful treatment of an extremely rare case of paraovarian cyst causing torsion of the ipsilateral fallopian tube in a patient with Rubinstein-Taybi syndrome. A 14-year-old girl with Rubinstein-Taybi syndrome was referred to our hospital for abdominal pain. Her medical history was unremarkable, except for moderate hirsutism and keloid scar. Physical examination revealed tenderness in the lower abdominal midline. The preoperative diagnosis was torsion of a left ovarian cyst. An exploratory laparoscopy was performed because of acute abdominal pain and revealed a left fallopian tube that was twisted twice due to an ipsilateral paraovarian cyst. The huge paraovarian cyst required laparotomy cystectomy, and the left ovary was preserved. Her postoperative course was uncomplicated. Preoperative diagnosis of paraovarian cysts can be difficult. The moderate hirsutism seen in our patient suggested the presence of a large paraovarian cyst due to androgen receptor-mediated effects. Therefore, Rubinstein-Taybi syndrome patients with hirsutism should be screened and assessed by pediatric surgeons for the presence of paraovarian cysts.

    Obstructive colitis (OC) is a nonspecific inflammatory condition that occurs at the proximal side of a completely or partially stenotic lesion typically caused by colorectal cancer. Impaired blood flow caused by these stenotic changes in the colon or rectum results in this condition. During surgery for sigmoid colon carcinoma with OC, complete surgical removal of the OC lesions is required. However, it is difficult to anticipate the range of OC before surgery. Diagnosing the potential ischemia during surgery would decrease the need for re-operation. This is the first report of HyperEye Medical System (HEMS) angiography for surgery of colon cancer with OC. We report a case of sigmoid colon carcinoma in which HEMS angiography was used and found to be useful for real-time detection of the OC lesion.

    Raman spectroscopy provides a wealth of diagnostic information to the surgeon with in situ cancer detection and label-free histopathology in clinical practice. However, to identify cancer diagnostic biomarkers in vivo is still challenging, because malignancy can be characterized not only by the cancer cells but also by the environmental factors. Here we investigate molecular dynamics in both cancer cells and their environment in xenograft models and spontaneous metastasis models using Raman spectroscopy and nonlinear optical imaging. We are constructing a custom-designed Raman spectral imaging system to reveal the metastasis process and to evaluate therapeutics toward the clinical application of the technique.

    Raman spectroscopy provides a wealth of diagnostic information to the surgeon with in situ cancer detection and label-free histopathology in intraoperative conditions. Raman spectroscopy is a promising optical technique which can analyze biological tissues with light scattering. The difference in frequencies between the incident light and the scattering light are called Raman shifts, which correspond to the vibrational energy of the molecular bonds. Raman spectrum gives information about the molecular structure and composition in biological specimens. We had been previously reported that Raman spectroscopy could distinguish various histological types of human lung cancer cells from normal cells in vitro, and also confirmed that Raman spectra obtained from cancer cells and their environment including other cells and extracellular matrix in xenograft models and spontaneous metastasis models were distinguishable using Raman spectroscopy combined with fluorescence microscopy and photoluminescence imaging. Malignancy can be characterized not only by the cancer cells but also by the environmental factors including immune cells, stroma cells, secretion vesicles and extracellular matrix, but to identify and detect cancer diagnostic biomarkers in vivo on Raman spectroscopy is still challenging. Here we investigate morphological and molecular dynamics in advanced cancer specimens obtained from patients. We are also constructing a custom-designed Raman spectral imaging system for both in vitro and in vivo assay of tumor tissues to reveal the metastasis process and to evaluate therapeutic effects of anti-cancer drugs and their drug delivery toward the clinical application of the technique.

    Background Elderly patients usually have concurrent ailments, and the safety and effectiveness of laparoscopy-assisted distal gastrectomy (LADG) for these patients have been controversial. This study aimed to evaluate whether laparoscopy-assisted distal gastrectomy is safe and effective for elderly patients aged 80 years and over, as well as to clarify their long-term prognosis.Methods A total of 31 patients aged 80 years and over who underwent LADG in our hospital were retrospectively reviewed. Peri- and postoperative data were compared with those of 38 patients aged 65 years and younger. The median follow-up period of the elderly and younger group was 56.0 and 63.0 months, respectively, and their prognosis was examined.Results There were significant differences between the two groups in preoperative respiratory and renal functions, hemoglobin, and nutritional index. Significant differences in postoperative complications were seen only in pneumonia and delirium. There were no hospital deaths, but the 3-year and 5-year overall survival rates were significantly lower in the elderly group than in the non-elderly group. However, in the elderly group, only one patient died of gastric cancer recurrence, whereas four died of cardiovascular disease and three died of pneumonia during follow-up. Therefore, the recurrence-free survival rate was not significantly different between the groups.Conclusions LADG seems to be safe and effective even for elderly patients, and their prognosis was satisfactory. However, careful monitoring for cardiovascular and pulmonary disease should be observed during the follow-up period.

    We developed a near infrared fluorophore-conjugated anti-Carcinoembryonic antigen (CEA) antibody for mice bearing tumor of CEA expressing cells, and demonstrated in vivo optical imaging by macro zoom microscopy. In the result, tumors of CEA-expressing cancer cells were specifically detected in vivo. Furthermore, cancer-specific fluorescence images were acquired at subcellular level in vivo by two-photon microscopy. In preclinical applications, the lymph node micrometastasis was also successfully visualized by two-photon microscopy. These results suggest that two-photon excitation microscopy in combination with an immunoconjugated probe could be widely adapted to cancer detection in clinical settings.

    Recently, there has been growing interest in applying fluorescence imaging techniques to the study of various disease processes and complex biological phenomena in vivo. To apply these methods to clinical settings, several groups have developed protocols for fluorescence imaging using antibodies against tumor markers conjugated to fluorescent substances. Although these probes have been useful in macroscopic imaging, the specificity and sensitivity of these methods must be improved to enable them to detect micro-lesions in the early phases of cancer, resulting in better treatment outcomes. To establish a sensitive and highly specific imaging method, we used a fluorophore-conjugated anti-carcinoembryonic antigen (CEA) antibody to perform macroscopic and microscopic in vivo imaging of inoculated cancer cells expressing GFP with or without CEA. Macroscopic imaging by fluorescence zoom microscopy revealed that bio-conjugation of Alexa Fluor 594 to the anti-CEA antibody allowed visualization of tumor mass consisting of CEA-expressing human cancer cells, but the background levels were unacceptably high. In contrast, microscopic imaging using a two-photon excitation microscope and the same fluorescent antibody resulted in subcellular-resolution imaging that was more specific and sensitive than conventional imaging using a fluorescence zoom microscope. These results suggest that two-photon excitation microscopy in conjunction with fluorophore-conjugated antibodies could be widely adapted to detection of cancer-specific cell-surface molecules, both in cancer research and in clinical applications.

    経肛門的に挿入された巨大なシリコン製玩具に対して、経肛門的に摘出困難であったため、開腹術を要した症例を経験したので報告する。症例は52歳男性。自慰行為にて肛門よりシリコン製玩具を挿入したが、自力で摘出できないため当院に救急搬送された。腹部症状は比較的軽度であり、反跳痛や筋性防御は認めなかった。単純腹部CTにて20cm×12cmの巨大な直腸内異物を認めたが、free airや腹水は認めなかった。外来での無麻酔下での抜去は困難で、全身麻酔下に摘出する方針とした。経肛門的に摘出を試みるも異物が骨盤腔につかえ摘出困難であった。開腹手術へと移行するも、異物は骨盤腔に挟まれており摘出に難渋した。腸管を半周切開し、切開部より異物を摘出した。術後は合併症など認めず、術後18日目に退院した。直腸異物では腹部症状も軽度にあらわれる可能性があり、治療方針決定においては病歴聴取、理学所見、画像診断が重要であることが認識された。(著者抄録)

    症例は73歳、女性。S状結腸癌術後にRS3PE症候群を発症した。当初は発熱および血液検査での炎症所見から感染症を疑っていたが、抗生剤に反応が乏しいこと、両側四肢末梢の浮腫に気づくことでRS3PE症候群と診断し得た。疾患概念があまり知られていないことから診断に時間を要する症例も散見され、診療の際は鑑別に注意が必要と考えた。また、RS3PE症候群を発症した場合、悪性腫瘍合併の可能性があり長期的なフォローが必要である。(著者抄録)

    <p>Most cases of Meckel's diverticulum are asymptomatic, but it can cause various symptoms. We report a rare case of Meckel's diverticulum penetrating the abdominal wall and forming a bladder anterior cavity abscess. A 6-year-old boy had a lower abdominal pain in the last 4 days. He complained of an increase in the severity of abdominal pain and fever, and he was brought to a clinic. Owing to suspicion of gastrointestinal perforation, he was brought to our hospital for emergency treatment and emergency surgery was conducted on the same day. Laparoscopic observation showed that Meckel's diverticulum penetrated the abdominal wall and formed a bladder anterior cavity abscess. We performed resection of Meckel's diverticulum and abscess drainage. His postoperative course was uneventful. Histopathological analysis revealed ectopic gastric mucosa at the tip of Meckel's diverticulum. Perforated ulcer was formed in the intestinal mucosa near the boundary between the ectopic gastric mucosa and the normal intestinal mucosa. Although preoperative diagnosis is difficult, exploratory laparoscopy enabled intraoperative diagnosis and a good postoperative course.</p>

    <p>A84－year－old man visited our hospital because of abdominal pain was diagnosed by abdominal computed tomography（CT）to have external fistula of stoma, eleven years after Mles'oparation. He underwent reconstruction of stoma. Regarding fistula development，Crohn，s disease, neoplastic lesion, and stool due to chronic constipation were not found. Furthermore, for this case, it was presumed that the thickness of the subcutaneous fat layer decreased and the internal pressure increased．In Japan, the ostomate is increasing year by year. On the other hand, the survival rate after rectal cancer surgery has also improved, and the long－term survival of the ostomate has not become rare. It is necessary to pay attention to external fistula as late complication of stoma fecal leakage. In conclusion, pouching systems should be exchanged while skin barriers remain during the swelling stage.</p>

    <p>Raman spectroscopy provides a wealth of diagnostic information to the surgeon with in situ cancer detection and label-free histopathology in intraoperative conditions. Raman spectroscopy is a promising optical technique which can analyze biological tissues with light scattering. We confirmed that Raman spectra obtained from cancer cells and their environment including other cells and extracellular matrix in xenograft models and spontaneous metastasis models were distinguishable using Raman spectroscopy combined with nonlinear optical imaging. Malignancy can be characterized not only by the cancer cells but also by the environmental factors including immune cells, stroma cells, secretion vesicles and extracellular matrix, but to identify and detect cancer diagnostic biomarkers in vivo on Raman spectroscopy is still challenging. Here we investigate morphological and molecular dynamics in advanced cancer specimens obtained from patients toward the clinical application of the technique.</p>

    Main advantages of the optical imaging technique achieving high spatio-temporal resolution, non-radiation, less cytotoxicity and low cost with small devices as compared to conventional clinical imaging techniques such as CT, MRI and PET. In particular, near infrared (NIR) fluorescence imaging enables us to visualize deep portion of tissues due to the excellent penetration <i>in vivo</i>. We developed a NIR fluorophore-conjugated anti-Carcinoembryonic antigen (CEA) antibody for mice bearing tumor of CEA expressing cells, and demonstrated <i>in vivo</i> optical imaging by two-photon excitation microscopy. In the result, CEA-expressing cancer cells were specifically detected <i>in vivo</i>. In preclinical applications, the lymph node micrometastasis was also successfully visualized by two-photon excitation microscopy. These results suggest that two-photon excitation microscopy in combination with the immunoconjugated NIR fluorescence probe could be widely adapted to cancer detection in clinical settings or various cancer metastasis model to reveal the mechanism of cancer metastasis and to visualize microenvironment which may consist of cancer cells and stroma cells.

    螢光イメージング技術を駆使して、生きている動物のなかで細胞のダイナミックな動態や機能を画像化・解析する生体螢光イメージング技術に注目が集まっている。この手法を癌研究に応用すると、生体内で癌細胞が転移する様子、細胞内シグナル伝達や細胞周期、さらに癌細胞と微小環境の相互作用などをリアルタイムで解析することが可能で、これまでは知りえなかった細胞の性質と周囲環境と相互作用が手に取るようにわかる。とくに、多光子励起顕微鏡を用いた非線形光学を応用すると、生体深部で高時間・空間分解能でのイメージングが可能で、実際の癌細胞やその周囲の免疫細胞などの挙動や変化を目の当たりにする。その結果、生命現象や病態の理解が深まり、新しい癌診断・治療開発につながると考えられる。本稿では、癌研究分野における生体螢光イメージングについて、とくに非線形光学に焦点を絞って実例を紹介し、その現状と今後について議論したい。(著者抄録)

    【目的】癌患者に対する化学療法時には、口内炎などの副作用のために治療を継続することが困難な場合が多い。今回それらに対して成分栄養剤が効果的であった症例を経験したので報告する。【症例1】75歳男性、平成15年1月直腸癌、前立腺癌に対して手術施行後平成21年1月局所再発認め化学療法開始。平成22年5月、PDにてレジメ変更した。化学療法の継続に伴い口内炎の悪化が認められた。成分栄養剤(エレンタール)を投与し口内炎の改善が認められた。【症例2】70歳代女性、平成22年10月左乳癌に対して乳房切除術施行された。外来で術後補助化学療法(TC)行ったが全身倦怠感および口内炎による口腔内痛のため経口摂取困難となり緊急入院し抗生剤投与と成分栄養剤の投与を行った。症状は改善し外来化学療法継続可能となった。【結語】成分栄養剤の投与により化学療法の副作用が軽減したと思われる例を経験した。私達の取り組みを含めて報告する。(著者抄録)

    We report a case of blunt aortic injury requiring treatment. A 22-year-old man fell down from a height of 1.5 m during work. He sustained an aortic injury and liver injury, as well as fractures of the mandible, lumbar vertebrae, and femur. An emergent operative repair of the aortic injury was performed and liver injury was conservatively treated. There was a massive hematoma around the distal aortic arch, a hemothorax was found, and the intima of the aorta was torn. A prosthetic graft replacement of the thoracic aorta was performed. The post-operative course was uneventful. After the fractures were repaired, the patient was discharged on the 119th day after the initial operation.

    平成15年4月〜20年9月にイレウスや穿孔性腹膜炎を合併せず、他臓器を合併切除しなかった大腸癌切除例185例(腹腔鏡(補助)下大腸切除術(LAC)78例・開腹下大腸切除術(OC)107例)を対象に、LAC、OCそれぞれの無再発生存期間、全生存期間を中心に比較検討した。郭清リンパ節個数に、LAC群、OC群間に有意差は認めなかった。結腸癌110例の短期成績は、LAC、OCそれぞれ出血量69.4±119.7ml、122.6±169.6ml、手術時間3h33m±41m、2h41m±44m、術後排ガス日2.3±0.8日、3.3±1.1日で両群間に有意差を認めた。LAC群はOC群に比べ出血量は少なく、手術時間は長く、術後排ガス日は早いといえた。直腸癌75例の短期成績は、LAC、OCそれぞれ出血量94.9±94.2ml、279±164ml、手術時間4h22m±1h17m、3h42m±1h21m、術後排ガス日2.2±0.9日、3.1±1.3日で両群間で、出血量、術後排ガス日に有意差を認めた。LACの長期予後をOCと比べた結果、短期成績はLACの方が回復が早いことが示され、無病生存期間、全生存期間には差がなかった。

    A 65-year-old man visited his family physician with the chief complaint of appetite loss, general fatigue, and melena on 15 March 2006, and then was referred to our hospital. At the time of admission to our hospital his body pressure was around 70 mmHg, and a blood examination revealed severe anemia and metabolic acidosis. Emergency gastroduodenoscopy revealed old bloody fluid and a black large bezoar. After his general condition improved we examined him, but there was no special mention except the bezoar, and on 6 June we incised the stomach and extracted the bezoar by open surgery. The bezoar measured 13&times;10 cm and presented as a stomach mold. The analysis of the stone revealed absorption similar to tannic acid in part.<br>We suspect that in this case the large bezoar injured the mucosa of the stomach, caused bleeding and severe anemia, and that disturbance of passage caused malnutrition, shock and metabolic acidosis. Gastric ulcer, ileus, and other complications of bezoar exist, and some of them progress seriously as in this case, so we must keep this disease in mind in case of bleeding of the digestive system.

    腹腔鏡補助下胃切除術(LAG)や腹腔鏡補助下大腸切除術LAC)は多くの利点があるが、まだまだ一般治療手段として普及しているとは言い難い。当科で施行しているLAG、LACについて検討した。(対象および方法)当科および愛媛大学第一外科で施行された腹腔鏡(補助)下胃、大腸切除術106例(LAG:58例、LAC:48例)。(結果)LAGは開腹による胃切除術に比べて、術後排ガス日は早く、体重減少は少なく、食事摂取量は多かった。また炎症性サイトカインのひとつであるIL-6の術中の変動は有意に低かった。(考察、結語)腹腔鏡(補助)下胃-大腸手術は、開腹手術に比べて術中のIL-6の上昇が低く、その結果術後の回復が早いと考えられた。腹腔鏡手術は良好な視野の下に、より確実な郭清を行うことができる可能性がある。3D-CT-Angiographyを行うことで、より安全に手術操作を施行することができた。(著者抄録)

    症例1(67歳男性).十二指腸潰瘍穿孔による汎発性腹膜炎から全身性炎症反応症候群(SIRS)状態に陥り,急性肺障害を惹起する侵襲として間接侵襲のうちの敗血症と考えられた.術直後から好中球エラスターゼ阻害薬であるエラスポールを投与し,良好な改善を得た.症例2(90歳男性).胸部外傷からSIRS状態に陥り,急性肺障害を発症した.誘因として直接侵襲の肺挫傷が考えられた.外傷によるDICを併発しており,超高齢であることから重症化が懸念されたが,受傷初期からのエラスポール使用開始と二次感染を併発しなかったことが,比較的早期の軽快となった.症例3(86歳女性).汎発性腹膜炎を誘因とする敗血症性SIRSに伴う急性肺障害と診断された.超高齢,下部消化管穿孔等の悪条件下にあったが,術直後からエラスポールを投与し,preDIC状態に対しても早期に蛋白分解酵素阻害剤を併用することで,比較的良好な術後経過が得られた

    平成15年度に腹腔鏡補助下大腸切除術施行した14例について検討した.郭清度はD3が12例,D1が2例であった.5例に開腹手術の既往があった.平均手術時間は3時間42分で,5例で他の疾患に対する同時手術を施行した.途中2例が開腹手術に移行した.術後経過について開腹手術と比較したが,対象例も少なく,特に有意差を提示することはできなかった.排ガスまでの日数も,開腹手術3.3日,腹腔鏡補助下手術3.5日となっていたが,これは後者症例に統合失調症に伴う腸管麻痺による排ガス遅延症例も混じっていたためであった.したがって,実際には術翌日に排ガスを認めた症例であった.尚,術後在院日数が長くなった症例もみられたが,術後補助化学療法施行や精神疾患による入院期間の延長の影響と考えられた

    62歳男.患者は心窩部不快感を主訴とし,内視鏡的胆管膵管造影,MRCPにより肝部胆管癌と診断された.肝右葉体積が大きいことから,左葉肝切除を選択し,拡大肝左葉切除,尾状葉切除,肝外胆管切除,リンパ節郭清術を施行したところ肝動脈および門脈には浸潤癌はなかった.しかし肝切離面に肝内胆管枝が計6本出現したため,胆管形成を行った後,肝管空腸吻合術を施行した.術後,肝管空腸縫合不全を約3週間認めたものの,良好なドレナージで発熱もなく,術後53日目に退院なった.現在,術後約12ヵ月が経過,無再発で外来通院中である.尚,1998〜2004年までの年次別肝臓切除症例の推移を確認したところ,近年増加傾向にあり,平成15年度には10例の肝切除を施行し,うち胆道癌症例は2例でであった

    70歳女.黄疸を主訴とした.眼球黄染を自覚し,近医で閉塞性黄疸を指摘された.転院後,経皮経肝的胆道ドレナージにより中部胆管癌と診断され,幽門輪温存膵頭十二指腸切除術,リンパ節郭清術を施行した.膵切離では,術中超音波を行い,主膵管の位置を十分確認の上,ハーモニックスカルペルを用い,膵下縁側から上縁方向へ膵実質を切離した.続いてメスを用いて主膵管部を切離した.膵腸吻合は膵管空腸粘膜吻合を施行し,膵被膜実質と空腸漿膜筋層の外列と,膵管壁は膵実質とともに針をかけて空腸全層の内列を吻合した.正常膵では主膵管径が1〜2mmと極めて細いが,6-0のモノフィラメント吸収糸を用いて6〜8針程度で縫合した.その結果,術後,ドレーン中のアミラーゼ値は経時的に低下し,一過性に胃排出遅延が認められたものの,その後は経過良好で,術後39日目に退院となった

    A 58-year-old woman admitted with abdomina pain and abdominal distension, had been diagnosed with ileus at an other hospital. Upper gastrointestinal tract examination showed a duodenum diverticulum and a jejunal diverticulum. We diagnosed ileus due to a jejunal diverticulum. Conservative therapy with an ileustube, and intravenous hyperlimentation achieved, but no symptomatic remission, necessitating adhesiotomy and wedge resection of the jejunal diverticulum. Resected jejunal diverticulum was a simple, true diverticulum with pathological inflammatory changes. The postoperative course was uneventful. This case is of interest due to its rarity.

    A 71-year-old man admitted with vomiting and abdominal pain has been diagnosed elsewhere with ileus and had a history of gastrectomy 34 years earlier due to a peptic ulcer.<br> He had a fist-sized tumor in the umbical area. We diagnosed intussusception based on abdominal ultrasonography and computed tomography and conducted emergency surgery.<br> Through Braun's anastomosis, the jejunum on anal side of Braun's anastomosis intussuscepted into the afferent jejunal loop and strangulation was reduced by manipulation.<br> The postoperative course was uneventfully. We found only 6 cases of jejunogastric intussusception at Braun's anastomosis after gastrectomy in the Japanese literature, indicating the great rarity of this condition.

    75歳男.主訴は腹痛である.激しい腹痛が出現し搬送された.入院時上腹部やや右側に強い自発痛はあったが,腹膜刺激症状はなかった.受診時腹部CTで門脈内にガス像があり,小腸の一部に軽度の浮腫が存在した.外来受診時,血圧が低下し,腹部CTで門脈ガス血症(HPVG)が認められたことより,緊急開腹手術の準備を開始し,腹部にプローブを強く当て腹部超音波検査を行っていた途中に突然腹痛は消失した.受診後3時間後の画像検査でも改善傾向が確認でき,入院の上で絶飲食,輸液,抗生剤投与のみで経過観察とした.第4病日目の腹部CTではHPVGは完全消失していたが,腹水が出現していた.第34病日に退院し以後再発していない

    A 70-year-old woman was referred to the hospital because of a mass on the right side of the neck. She had a 3-cm smooth, elastic soft mass in the right side of the neck. A CT scan dected a 4&times;3cm tumor with calcification in the right lobe of the thyroid. A 4&times;2cm smooth and solid mass was accidently found behind the thyroid tumor at the right side of the trachea during the operation. She underwent a right lobectomy with modified neck lymph node dissection and etipation of the pamatracheal tumor preserving the right recurrent laryngeal nerve. Histological examination revealed follicular carcinoma of the thyroid and concominant paratracheal neurinoma. Her postoperative course has been uneventful without any evidence of recurrence and hoarseness. This case of thyroid cancer with paratracheal neurinoma is very rare. So far only six cases have been reported in this country.

    Raman spectroscopy provides a wealth of diagnostic information to the surgeon with in situ cancer detection and label-free histopathology in intraoperative conditions. Raman spectroscopy is a promising optical technique which can analyze biological tissues with light scattering. We confirmed that Raman spectra obtained from cancer cells and their environment including other cells and extracellular matrix in xenograft models and spontaneous metastasis models were distinguishable using Raman spectroscopy combined with nonlinear optical imaging. Malignancy can be characterized not only by the cancer cells but also by the environmental factors including immune cells, stroma cells, secretion vesicles and extracellular matrix, but to identify and detect cancer diagnostic biomarkers in vivo on Raman spectroscopy is still challenging. Here we investigate morphological and molecular dynamics in advanced cancer specimens obtained from patients toward the clinical application of the technique.

    Optical imaging technology is a reliable tool for biomedical research filed and clinical diagnostics in various diseases. We developed and evaluated own microscopy system for biomedical applications in the field of orthopedic surgery. Osteoporosis is a major bone disease that connotes the risk of fragility fractures. Bone strength arises from both bone quantity and quality. Assessment of bone quality and bone quantity is important for prediction of fracture risk. Bone mineral density is used to determine the bone strength in the current diagnosis of osteoporosis. However, there is no practical method to measure chemical composition of bone tissue including hydroxyapatite and collagen non-invasively. Raman spectroscopy is a powerful technique to analyze chemical composition and material properties of bone matrix non-invasively. Here, we successfully obtained Raman spectral data of bone matrix to evaluate the bone quality in the osteoporosis model.
