BACKGROUND: Epidemiological evidence for the relationship between education and income and carotid intima-media thickness (CIMT) has been limited and inconsistent. The present cross-sectional study investigated this issue using baseline data from the Aidai Cohort Study. METHODS: Study subjects were 2012 Japanese men and women aged 34-88 years. Right and left CIMT were measured at the common carotid artery using an automated carotid ultrasonography device. Maximum CIMT was defined as the largest CIMT value in either the left or right common carotid artery. Carotid wall thickening was defined as a maximum CIMT value > 1.0 mm. RESULTS: The prevalence of carotid wall thickening was 13.0%. In participants under 60 years of age (n = 703) and in those aged 60 to 69 years (n = 837), neither education nor household income was associated with carotid wall thickening or with maximum CIMT. Among those aged 70 years or older (n = 472), however, higher educational level, but not household income, was independently related to a lower prevalence of carotid wall thickening: the multivariate-adjusted odds ratio for high vs. low educational level was 0.43 (95% confidence interval 0.21-0.83, p for trend = 0.01). A significant inverse association was observed between education, but not household income, and maximum CIMT (p for trend = 0.006). CONCLUSIONS: Higher educational level may be associated with a lower prevalence of carotid wall thickening and a decrease in maximum CIMT only in participants aged 70 years or older.

<p>Japan has adopted the universal health insurance system, in which a detailed, single list of fee points is used to evaluate every claim, regardless of type of insurance. The great success of “Rececom” (i.e., an abbreviation for “receipt computer”) in managing electronic medical claims, which was developed ahead of the world, may also have represented a significant factor in hindering the spread of electronic medical records systems in the personal computer era. Over time, departmental information systems became linked to computerized provider order entry systems, and data exchange over networks became practical. Nevertheless, interoperability between medical information systems became a problem that was impossible to ignore. In the 1980s, a non-profit organization, HL7 International, was established in the United States to create and disseminate international standards for medical information systems. Subsequently, the Japanese Association of Healthcare Information Systems Industry (JAHIS) was established in 1994, and the Association for Medical Informatics and JAHIS founded the HL7 Japan in 1998. JAHIS and HL7 Japan have since led the development of medical information standards in Japan. In 2001, the Healthcare Information System Review Committee announced a “Grand Design for computerization in the medical field,” and MHLW promoted the spread of electronic medical record systems and the development of medical information standards. HELICS, which is an incorporated association of academic societies and industries involved in the provision of health information, was established in 2007. HELICS evaluates proposal to determine whether they should be accepted as proposals for the national standards in Japan. Specific health checkups and specific health guidance are an initiative unique to Japan, drawing worldwide attention as a measure to counter lifestyle-related diseases that are increasing in developed countries. The HL7 CDA document specification is available regarding specific health checkups, and checkup institutions must create and submit checkup reports that conform to the defined specification. The Pharmaceuticals and Medical Devices Agency (PMDA) has three responsibilities: “Relief Service for Adverse Health Effects,” “Review,” and “Post-marketing Safety Measures.” To achieve efficient review and cross-sectional analysis, PMDA requires clinical trial data to be submitted in accordance with the Clinical Data Interchange Standards Consortium (CDISC) standard. Consistent with HELICS proposals, standardization will be promoted principally in accordance with HL7 V2.x and CDA. In addition, the Fast Healthcare Interoperability Resources may be used to develop new medical applications in areas not profiled in these HL7 2.x and CDA related standards in Japan.</p>

<p><b>Abstract </b>Swift data collection and response across borders become urgent issues in various fields, and the public health sector is no exception. It is necessary to develop and disseminate elemental technology to realize an environment that enables high-quality data collection and management conforming clinical research proto col; collection, integration, and analysis of data in various scenes, and can be released in real-time through standardization of information content and exchange methods. In this paper, we introduce the historical background and development philosophy of FHIR and CDISC standards as the main-stream medical infor mation standards and the use cases in the field of public health with these standards. A promising approach in the field of public health in the future is to operate FHIR for collecting information originating from healthcare settings and CDISC standards for surveillance in general.</p>

&lt;p&gt;Data health initiatives require that we are able to derive evidence from an observational database using Real World Data (RWD) collected exclusively. As the issues related to database design, RWD requires integration into the standard information model and controlled terminologies, inter-organization calibration on such as measurement results, identification and tracking of patient individuals, and bias problems related to data sources. Regarding these issues, standardization of ETL method from RWD, definition of standardized database structure, developing proper methodology to define cohorts, identification of the sources of RWD data and development of quality measures, and ensuring coverage on all cohorts and time series are the highest priority research topics of the medical big data era. We focused on methodologies for defining a common data model for an observational research database and proper phenotyping for the medical informatics concerned. We have investigated the PheKB repository that publishes phenotyping methods in the eMERGE project, and the OMOP Common Data Model and standard vocabulary developed by in OHDSI project. We also propose that rather than trying to implement international CDM in a single step, we should set up two projects. One is promoting the development of a CDM consortium so that electronic medical vendors properly integrate their EHR system and DWH regarding Japanese standard terminologies. The other is that the researcher should develop the mapping tools that map between domestic and international CDM and Terminologies leveraging the CDM.&lt;/p&gt;

&lt;p&gt; In order to share really useful information of the patient profile which describe a whole picture of a patient, we investigated how many items of the patient profile information (PPI) were implemented at three university hospitals and how highly data of the PPI were registered. Analyzing data of this investigation, we clarified the problems of the PPI management and considered those solutions. We used items of the PPI which were temporarily set by the Research Group of the Patient Profile Information Infrastructure. The items to enhance the reliability and to be related to the preventive behaviors and summarized medical information were low implemented. Only about 30% of implemented items were registered for more than 50% of the inpatients. The data registration was mainly operated by nurses in the wards and by physicians at the outpatient clinics. Since the number of nurses at the outpatient clinics is rather small, even the basic items of the PPI for the outpatients were not well registered. In order to improve the low registration, we propose to standardize the components of the PPI, to promote the medical institutions to implement the standardized PPI and to register data. In addition, we propose to develop a networking system to share the data of the PPI among the local medical institutions including the health checkup institutions.&lt;/p&gt;

We analyze the deterioration of clinical data quality due to anonymization. The result shows that data quality remained high with micro-aggregation and also verify the availability of noise addition to prevent illegal re-identification by matching another personal data.

Background: For more than 30 years, there has been close cooperation between Japanese and German scientists with regard to information systems in health care. Collaboration has been formalized by an agreement between the respective scientific associations. Following this agreement, two joint workshops took place to explore the similarities and differences of electronic health record systems (EHRS) against the background of the two national healthcare systems that share many commonalities.
Objectives: To establish a framework and requirements for the quality of EHRS that may also serve as a basis for comparing different EHRS.
Methods: Donabedian's three dimensions of quality of medical care were adapted to the outcome, process, and structural quality of EHRS and their management. These quality dimensions were proposed before the first workshop of EHRS experts and enriched during the discussions.
Results: The Quality Requirements Framework of EHRS (QRF-EHRS) was defined and complemented by requirements for high quality EHRS. The framework integrates three quality dimensions (outcome, process, and structural quality), three layers of information systems (processes and data, applications, and physical tools) and three dimensions of information management (strategic, tactical, and operational information management).
Conclusions: Describing and comparing the quality of EHRS is in fact a multidimensional problem as given by the QRF-EHRS framework. This framework will be utilized to compare Japanese and German EHRS, notably those that were presented at the second workshop.

大規模災害が発生しても医療サービスを継続すること、すなわちシステム障害発生下でも診療に必要最低限の医療情報にアクセスでき、かつ速やかに原状の運用体制に復帰できる事業継続計画を策定することが医療機関に望まれている。これまでは情報セキュリティの保全の観点から、電子カルテの遠隔バックアップには利用者に閉じたホスティングが好まれ、管理コストが増大する傾向があった。本研究では、医療機関が相互に計算機資源を提供して相互バックアップを行い、必要時にいずれの医療機関でも速やかに参照、原状復帰できる環境の実現を目指した。単独では復元が不可能な複数の分散片に分割・保存する秘密分散技術を利用して秘密分散バックアップシステムと、安全に医療情報が参照できるシステムを開発した。個人情報の安全性確保と、運用コストの抑制かつ単一障害点を有しない相互医療情報バックアップ環境の実現が可能であることが確認された。(著者抄録)

Due to rapid increase of network bandwidth, applications and systems working in long fat network (LFN) play more important roles. For effective development of them, precise measurements of network conditions are significant. Transmission Control Protocol (TCP) is the most commonly used protocol, but is essentially unable to achieve high throughput in LFNs with packet losses. For this reason, it is hard for conventional network measurement tools to show the maximum or available bandwidth in LFN, especially in high packet loss environments. To overcome this issue, we introduce a novel data transfer protocol on TCP/IP transport layer, namely high-performance and flexible protocol (HpFP). For high-precision pace control and retransmission control, the HpFP intermittently monitors network conditions such as packet loss and latency. We develop an application via the HpFP, named hperf, which measures end-to-end throughput as well as status of packet loss and latency in LFNs. We carry out experiments to examine the abilities of the hperf in high-throughput data transfer and measurement of network qualities in terms of packet loss and latency. The hperf achieves almost wire-rate throughput, 10 Gbps, on the international link between Japan and the USA with even 0.5% packet loss ratio (PLR). The measurements of packet loss and latency show good correspondence with the conventional methods via iperf and ping. These results are verified in our laboratory experiments on 10 Gbps link using a network simulator as well. We conclude that the HpFP has significant potential for a variety of network applications and the hperf is a good network quality measurement tool in LFNs, compared to the conventional TCPs.

Issues related to ensuring patient privacy and data ownership in clinical repositories prevent the growth of translational research. Previous studies have used an aggregator agent to obscure clinical repositories from the data user, and to ensure the privacy of output using statistical disclosure control. However, there remain several issues that must be considered. One such issue is that a data breach may occur when multiple nodes conspire. Another is that the agent may eavesdrop on or leak a user's queries and their results. We have implemented a secure computing method so that the data used by each party can be kept confidential even if all of the other parties conspire to crack the data. We deployed our implementation at three geographically distributed nodes connected to a high-speed layer two network. The performance of our method, with respect to processing times, suggests suitability for practical use.

With the tremendous development of remote sensing technologies, a large amount of observation data are generated from sensors. Since each sensor generates data periodically, e.g., every minute, a concurrent data processing using a cloud system plays an important role in the modern design process. This paper focuses on concurrent data processing techniques for an X-band phased array weather radar ( PAWR) using high-speed network, parallel data processing system, and large-scale storage system. The PAWR at National Institute of Information and Communications Technology (NICT), Japan rotates in 30 sec to capture a 3D structure of rainfalls within 60 km in radius and 15 km in altitude. In this paper, we develop a real-time 3D visualization system of the observation data of the PAWR. Our visualization is carried out from 54 sec to 69 sec ( depending on the weather conditions) after every observation period, which is in the same time scale with other conventional 2D visualization of X-band weather radars. In addition, we discuss a combination of cloud ecosystems for the concurrent processing at low cost. The methodology is considered as a pioneering case study to develop of a variety real-time data processing systems of big data via remote sensing.

For network communications using modern high throughput satellite (HTS) on geostationary orbits, network throughput of transmission control protocol (TCP), one of the most popular protocols, is limited due to the packet loss on the satellite link. The packet loss is mainly caused by the attenuation of signals in severe weather conditions like heavy rain. It is high time to develop novel network communication techniques on the transport layer in TCP/IP designed for the systems and applications in broadband communications. In this paper, we introduce a high-speed data transfer protocol, named high-performance and flexible protocol (HpFP), to achieve high throughput for the HTS even with packet loss. The HpFP, in comparison with TCP-Hybla and UDP-based data transfer (UDT) protocols, is evaluated on a laboratory experiment simulating a geostationary orbit satellite link of 10 Gbps. It is clarified that the HpFP outperforms both the TCP-Hybla and the UDT showing high throughputs (close to 10 Gbps) when the packet loss ratio (PLR) is 1%, and remains more than 1 Gbps under even 10% PLR condition. Moreover, in case of no packet loss, the HpFP exhibits a quick start-up time (6 sec) at the initial phase to achieve 10 Gbps, while the TCP-Hybla and the UDT take 9 sec and 16 sec to their maximum throughputs, respectively.

With the tremendous development of remote sensing technologies, large-scale data sets are collected from environmental and meteorological sensors. Edge computing for large-scale data sets is hard to be conducted in real time on the sensors, thus data transfer to cloud system and data processing play an important role. One of the barriers in such system is highspeed data transfer on long-distance networks. Even low packet loss condition leads to decrease in throughput by conventional transmission control protocol (TCP)-based tools. In this paper, we overcome this issue by proposing a high-performance data transfer tool to improve real-time data processing of phased array weather radar on long-distance networks. The proposed tool, called high-performance copy (hcp), is designed based on an open source tool for file copy operations and implemented using our socket library specialized for long-distance networks. We evaluate our tool with simulations and real data transfer scenarios. The results show that the hcp enables concurrent data transfer from radar sites to cloud system (data processing site) in real time. The throughput of data transfer from aradar site to a data processing site increases from 40 Mbps using the conventional TCP-based tool to 489.2 Mbps using the hcp.

Large-scale distributed file system is an important technology for big data sciences. Several works so far have succeeded in high-bandwidth data I/O on multi-point to multi-point remote storage systems. However, for easy use by many scientific researchers, high-bandwidth remote storage tools working on a single client computer are preferable. As for high-bandwidth multi-point to single-point remote storage system, there is a study to introduce a high-bandwidth virtual remote storage (HbVRS) tool using a distributed file system (Gfarm) and a UDP-based data transfer protocol (UDT) in long fat network (LFN). In this paper, we introduce a novel transport protocol, named high-performance and flexible protocol (HpFP), to show high throughputs for the HbVRS in LFNs with packet loss. We examine basic performances of multiple streams of the HpFP and the UDT on a laboratory experiment simulating an international LFN. In the presence of packet losses (0.5%), the HpFP shows more than 9.3 Gbps network throughput at any parallelization numbers from 1 to 12, while the maximum throughput by the UDT is limited in 1.4 Gbps with 12 parallelization.

Achieving the quality of service (QoS) is an important requirement in a communication network. Satellite communication is posing many challenges due to the limitation of transmission control protocol (TCP) over networks with high latency. To overcome these issues, the wide area network (WAN) optimization provides the data transfer on such long-distance networks. However, this optimization is not able to utilize the available bandwidth of provided network efficiently since it performs fixed bandwidth allocation. This paper proposes a technique to enhance the available bandwidth utilization for International Telecommunications Satellite Organization (ITSO, or INTELSAT) network. This technique adopts a high-speed data transfer protocol, named high-performance and flexible protocol (HpFP), to transfer data between the satellite and the ground station. The HpFP is a connection-oriented protocol to work on the top of user datagram protocol (UDP) and provides us with a stream-type of reliable data transfer even under high packet loss rate. One of the ingenious attempts in the HpFP is to set an internal target throughput for pace control of sending packets. Since this parameter setting is time-dependent, the target throughput is calculated based on network conditions monitored by the HpFP. The HpFP detects the unused bandwidth in the satellite bandwidth resources at every moment, then dynamically allocates HpFP data transfers. The results of laboratory experiments show how effectively the HpFP utilizes the available network bandwidth in the condition of the WAN optimization control on INTELSAT satellite network.

In communication systems using geostationary orbit satellites, throughput of transmission control protocol (TCP) is limited due to the impact of latency on network and packet loss caused by signal attenuation in severe weather conditions like heavy rain. It is high time to develop network techniques and applications in broadband communications over the gigabit satellite and the high throughput satellite (HTS). In this paper, we introduce a high-speed data transfer protocol, named high-performance and flexible protocol (HpFP), to achieve high throughput over a geostationary satellite link even in severe weather conditions. The HpFP is firstly evaluated on a laboratory experiment simulating a geostationary orbit satellite link. It is clarified that the HpFP shows high throughputs even when the packet loss ratio (PLR) is 0.01%. We next carry out a field experiment using the Wideband InterNetworking engineering test and Demonstration Satellite (WINDS). The performance of the HpFP over single, dual, and multiple connections are evaluated. The result shows that the aggregate throughput of dual connections of HpFP almost reaches to the maximum bandwidth, and the time to the maximum bandwidth is within 3 sec which is over 20 times faster than that by the TCP. For multiple connections, the HpFP shares the bandwidth equally among all 50 connections.

A model dataset of patient profile information was created based on the items used at five Japanese university hospitals, the patient information data elements in Health Level 7 (HL7) v2.5, and the standard datasets for medical information exchange used in Japan. In order to check the validity of the model dataset, a cross-sectional survey was performed. A preliminary analysis of 20 Japanese hospitals found that most items were implemented at some hospitals, but the number of items implemented at many hospitals was rather small. This result strongly shows the necessity for a standardized dataset of patient profile information.

The Standardized Structured Medical Information Exchange (SS-MIX) is intended to be the standard repository for HL7 messages that depend on a local file system. However, its scalability is limited. We implemented a virtual file system using NoSQL to incorporate modern computing technology into SS-MIX and allow the system to integrate local patient IDs from different healthcare systems into a universal system. We discuss its implementation using the database MongoDB and describe its performance in a case study.

We demonstrate the concept of Arden Syntax, which is like Domain Specific Language (DSL), for describing decision support rules that take advantage of modern programming language concepts. Using object relational mapper and meta programming enables the conceptualization of FHIR resources from various data sources, and reduces the complexity of knowledge retrieval in decision rules.

This paper is to propose a cloud system for science, which has been developed at NICT (National Institute of Information and Communications Technology), Japan. The NICT science cloud is an open cloud system for scientists who are going to carry out their informatics studies for their own science. The NICT science cloud is not for simple uses. Many functions are expected to the science cloud; such as data standardization, data collection and crawling, large and distributed data storage system, security and reliability, database and meta-database, data stewardship, long-term data preservation, data rescue and preservation, data mining, parallel processing, data publication and provision, semantic web, 3D and 4D visualization, out-reach and in-reach, and capacity buildings.

In 2008, the International Council for Science (ICSU) established the World Data System (WDS) to ensure the long-term stewardship and provision of quality-assessed data and data services to the international science community and other stakeholders. One of the objectives of the WDS is to support and make progress of interdisciplinary and cross-over studies data analyses covering a variety of research fields. In 2010, the NICT Science Cloud, which is one of the projects of the National Institute of Information and Communications Technology (NICT), has initiated a sub-project to realize the WDSs concept of the interdisciplinary data activity. In this paper, we demonstrate potential capability of our Web application for interdisciplinary data analysis, which is now operated on the NICT Science Cloud. In the initial stage of the system development, we constructed a basic concept and an initial design of the Web application, taking into account our long-term experiences in data-oriented activities under the WDC (World Data Center) system of the ICSU, which was an ICSUs data program operated from 1957 to 2008, preceded the WDS. Based on our design, we have implemented the Web application for data plots on the basis of the HTML5 and the Ajax technologies. Data plots can be previewed on the Web application with higher usability and better handleability than those of traditional data-plot tools developed for multiple-datasets. We finally discuss feasibility and potential usefulness of the Web application for future interdisciplinary data–analysis activities of the WDS.

Achieving interoperability between clinical electronic medical records (EMR) systems and cloud computing systems is challenging because of the lack of a universal reference method as a standard for information exchange with a secure connection. Here we describe an information exchange scheme using HTML5 microdata, where the standard semantic container is an HTML document. We embed HL7 messages describing laboratory test results in the microdata. We also annotate items in the clinical research report with the microdata. We mapped the laboratory test result data into the clinical research report using an HL7 selector specified in the microdata. This scheme can provide secure cooperation between the cloud-based service and the EMR system.

A variety of satellite missions are carried out every year. Most of the satellites yield big data, and high-performance data processing technologies are expected. We have been developing a cloud system (the NICT Science Cloud) for big data analyses of Earth and Space observations via spacecraft. In the present study, we propose a new technique to process big data considering the fact that high-speed I/O (data file read and write) is important compared with data processing speed. We adopt a task scheduler, the Pwrake, for easy development and management of parallel data processing. Using a set of long-time scientific satellite observation data (GEOTAIL satellite), we examine the performance of the system on the NICT Science Cloud. We successfully archived high-speed data processing more than 100 times faster than those on traditional data processing environments.

Cross-sectional studies have become important for an improved understanding of various Solar-Terrestrial Physics (STP) fields, given the great variety and different types of observations from the Sun to the Earth. In order to better combine, compare, and analyze different types of data together, a system named STARS (Solar-Terrestrial data Analysis and Reference System) has been developed. Cross-sectional study requires cooperative work. STARS has two functions for cooperative work, the "Stars Project List (SPL)" and the "Event Listing". The SPL is used for exchanges of plotting information by cooperating persons. The event list database provides all users of STARS hints for recognizing typical occurrences of STP phenomena.

In the face of a disaster hospitals are expected to be able to continue providing efficient and high-quality care to patients. It is therefore crucial for hospitals to develop business continuity plans (BCPs) that identify their vulnerabilities, and prepare procedures to overcome them. A key aspect of most hospitals' BCPs is creating the backup of the hospital information system (HIS) data at multiple remote sites. However, the need to keep the data confidential dramatically increases the costs of making such backups. Secret sharing is a method to split an original secret message so that individual pieces are meaningless, but putting sufficient number of pieces together reveals the original message. It allows creation of pseudo-redundant arrays of independent disks for privacy-sensitive data over the Internet. We developed a secret sharing environment for StarBED, a large-scale network experiment environment, and evaluated its potential and performance during disaster recovery. Simulation results showed that the entire main HIS database of Kyoto University Hospital could be retrieved within three days even if one of the distributed storage systems crashed during a disaster.

Objectives: This article describes the Web application framework for Electronic Health Records (EHRs) we have developed to reduce construction costs for EHR sytems. Methods: The openEHR project has developed clinical model driven architecture for future-proof interoperable EHR systems. This project provides the specifications to standardize clinical domain model implementations, upon which the ISO/CEN 13606 standards are based. The reference implementation has been formally described in Eiffel. Moreover C# and Java implementations have been developed as reference. While scripting languages had been more popular because of their higher efficiency and faster development in recent years, they had not been involved in the openEHR implementations. From 2007, we have used the Ruby language and Ruby on Rails (RoR) as an agile development platform to implement EHR systems, which is in conformity with the openEHR specifications. Results: We implemented almost all of the specifications, the Archetype Definition Language parser, and RoR scaffold generator from archetype. Although some problems have emerged, most of them have been resolved. Conclusions: We have provided an agile EHR Web framework, which can build up Web systems from archetype models using RoR. The feasibility of the archetype model to provide semantic interoperability of EHRs has been demonstrated and we have verified that that it is suitable for the construction of EHR systems. © 2013 The Korean Society of Medical Informatics.

Medical staff members should match blood products with patients using a barcode authentication system for blood transfusion to prevent medical accidents. However, our hospital only verifies the blood products of the Japanese Red Cross Society and the preserved autologous blood, not the autologous blood salvaged during the operation or from the oxygenator. In this study, we developed the barcode medication administration system and mobile device for verification. This system will prevent blood transfusion errors in the ward setting.

This paper evaluated the feasibility of secret sharing through simulation on a large-scale network experiment environment and on a commercial secret sharing service platform on the Internet. The simulation result confirms the feasibility of the existing secret sharing services for HIS data backup, although additional performance tuning of encryption and disk IO processe is required.

Using SSL-VPN requires special considerations for well-known issues such as attackers exploiting web browser vulnerabilities and phishing sites using man-in-the-middle attacks. We used leakage-resilient authenticated key exchange (LR-AKE) to develop a comprehensive solution to SSL-VPN issues. Our results show that the LR-AKE should contribute to building a robust infrastructure for personal health records.

It is often discussed that the fourth methodology for science research is "informatics". The first methodology is a theoretic approach, the second one is observation and/or experiment, and the third one is computer simulation. Informatics is a new methodology for data intensive science, which is a new concept based on the fact that most scientific data are digitalized and the amount of data is huge. The facilities to support informatics are cloud systems. Herein we propose a cloud system especially designed for science. The basic concepts, design, resources, implementation, and applications of the NICT science cloud are discussed.

The Act for the Protection of Personal Information in Japan considers as personal information any quasi-identifier that may be used to obtain information that identifies individuals through comparisons with datasets. Studies using health records are not widely conducted because of the concern regarding the safety of anonymized health records. To increase the safety of such records, we used the Pk-anonymity method. In this method, attributes are probabilistically randomized and then reconstructions are performed on the basis of statistical information from perturbed data. Hence, it is expected to provide more precise statistics and more reliably preserve privacy than the traditional "k-anonymity" method. We anonymized health records, performed cross tabulation, and assessed the error rate using original data. This study shows that the Pk-anonymity method can be used to perform safety statistical disclosures with low error rates, even in small cases.

Because intractable diseases result from unidentifiable causes and are very difficult to treat, they require a lifelong epidemiology database. Japan does not use global unique identifiers, such as social security numbers, so we conducted a feasibility study regarding an electronic health record (EHR). An EHR can be used as a lifelong database and reduce conventional administrative work. However, it will be necessary to develop additional tools to overcome issues specific to Japan before an EHR can be implemented.

The healthcare domain has a specific workflow for medical treatment. The introduction of information security measures that does not considers the behavioral principles of medical workers not only interferes with treatments but may also increase medical risk as a result of human errors due to attempts to bypass these security measures. This is because security measures may introduce a liability if they prevent smooth medical treatment. For this reason, we need to develop efficient security measures for healthcare that balance information security and usability. We propose a security framework that improves safety without disturbing the physicians' actions. The use of a device that has the ability of spatial perception and a suitable authentication system allows the user to freely interact with applications after the first login procedure is completed. By applying the security framework, we can simultaneously solve the following conflicting problems: information security, medical risk management, and sanitary issues. © 2011 IEEE.

Clinical research forms enable the obtaining of clinical information about diseases covered by the specifi c diseases treatment research program and are expected to be used for characterizing pathogenesis and drug-development. Recently, clinical research forms have been digitalized in all prefectures and have been utilized in intractable disease research groups. Clinical research forms should satisfy the reguirements of administrative applications for public fi nancing and research applications for characterizing pathogenesis; however greater emphasis has been placed on administrative applications. Therefore, it has been argued that the information obtained froms clinical research forms lacks credibility. In this study, we try to establish a novel system for extracting clinical data from electronic health records reliably and safely, as well as to review the contents of clinical research forms for the purposes of the specifi c diseases treatment program.

小・中学校にて行っている運動器障害の結果と検診体制について検討を加えた。対象は、平成20年では小学生699名、中学生470名であった。まず、アンケート調査票によるスクリーニングを行い、アンケート調査票は、Optical Character Reader(OCR)によりコンピューターに取り込み、要直接検診者の選定を行い、学校での直接検診を行った。学校での直接検診は、要検診者中98%が参加し、小学生アンケート回答者中152名(21.7%)、中学生161名(34.2%)であった。今年から、直接検診のときにも検診結果をコンピューター入力する電子カルテ化を行い、その場で結果を印刷し、手渡すこととした。疑い病名ありは、小学生ではアンケート回答者中10.3%であり、中学生では16.8%であった。スポーツによる障害は、小学生28%、中学生45%であった。学校における運動器検診を円滑かつ効率的に実施するために、コンピューターを活用し、解析することに工夫を行ったが、さらに改善に取り組みたい。(著者抄録)

The Japanese phrase Tokutei Shikkan refers to intractable diseases that result from unidentifiable causes and that are very difficult to treat, as there no treatment procedures have been established. Typically, the treatment of these diseases requires long-term care and medicines, which causes great financial and mental stress on the patient's family, so these treatments are subsidized at public expense. Given the lack of statistical information on these rare diseases, The Specified Disease Treatment Research Program was established in 1972, and 56 intractable diseases are currently included as Tokutei Shikkan. The program has been very successful, and many clinical research groups have gained new insight into intractable diseases. Unfortunately, no cross-sectional studies of these diseases have been conducted due to semantic differences among clinical research forms, which are designed mainly to apply for administrative funds and which contain little clinical information. We redesigned the forms as openEHR-based templates and mapped specific clinical notations for intractable diseases onto the archetypes in CKM (Clinical Knowledge Manager). The openEHR project is a current reference implementation of ISO/CEN 13606. We are now trying to bridge the gaps between international standards and criteria and those unique to Japan. This paper shows how we mapped the clinical information on intractable diseases onto the archetypes and how we implemented a clinical research form entry system that is coordinated with electronic medical records.

Results of bandwidth challenge contest at Super Computing (SC) 2007 by NICT are discussed. The target of this contest is to make full use of 10Gbps network for any scientific applications. We, NICT team, attempted to transfer large-scale numerical data at NICT (in Japan) to SC 2007 venue in Reno, USA. We optimized our data transfer system for the real-time computer simulation data, and finally obtained through-put from Japan to USA as high as 4Gbps.

It is well known that the Earth&#039;s magnetosphere often shows complex 3D configurations when IMF (Interplanetary Magnetic Field) By component is large. However, such complex magnetic fields have not been clarified yet. We performed a 3D Global MHD simulation with IMF By=/=0 to study interactions between solar wind and the Earth&#039;s magnetosphere. We found that complex 3D magnetic fields called magnetic flux rope in the Earth&#039;s magnetotail are generated and propagated tailward.

地球科学の推進には,国際共同による地球観測とデータの自由な交換が不可欠である.現在,Virtual ObservatoryやVirtual Organizationなど,世界規模での観測データの交換技術についての研究が進められている.本研究が対象とする太陽地球系物理(STP)観測分野は,他の地球観測分野と比較して観測領域が広いこと,観測データのタイプが多岐にわたるためデータ解析対象が複雑であること,データ公開機関が国内外に分散していること,データ公開ポリシが機関ごとに独立していることなどが分散データベースの実現を困難にしている.本研究では,このような疎結合型データベースによるVirtual Observatory(VO)を実現するため,RSS1.0を用いたメタ情報収集の仕組みを提案する.RSS1.0でSTP分野のメタ情報を記述し,データファイルやデータ観測日時情報を定期的に自動収集する.これにより,データ提供サイトがメタ情報提供を意識せずに,VOが必要とするメタ情報の収集が可能となる.設計したシステムを実装したところ,約3か月間で四つのデータ公開サイトから81種類約31万件の観測データファイルを自動収集できた.

聴覚障害者への情報保障手段として,パソコン要約筆記が多くの現場で利用されている.しかし要約筆記には独自の技術が必要であり,習熟した技術を有する要約筆記者の数は十分ではない.その結果,現在はまだサービスや支援が十分でないことが指摘されている.要約筆記者の不足を補うため,現在,様々な研究グループが遠隔支援による要約筆記の研究を進めている.本研究では,インターネット上で動画像ストリーミング配信を行い,それを用いた遠隔要約筆記システムを設計及び構築し,その有効性について検討する.これまでの遠隔パソコン要約筆記システムでは,要約筆記アプリケーションで文字情報送受信を行う際のセグメントの違いにより要約筆記者の参加制限が生じる問題点があった.本研究ではこれをVPNアプリケーションで補うことで,実用性の高い遠隔パソコン要約筆記を実現した.その結果,複数入力者へのネットワークに遅延がある場合でも,一宗時間内であればインターネット上からの遠隔パソコン要約筆記が可能であることが分かった.

In the Solar-Terrestrial Physics (STP) field, the Virtual Observatory (VO) project is difficult to realize because the data model of STP is wide-ranged and rich in diversity. To realize VO in STP, we have to develop the method that absorbs the gap between the data models in many data site. We have designed the light-weight meta-data delivery mechanism with RSS 1.0 which supports the extensibility from the RDF nature. We have designed the class and relationships of between Mission, Team, Group, User, and observatory data with OWL by the lesson of comparing the difference of models and web-services: CDAS and STARS. This RSS/RDF based meta-data proposes promising prospects which it shows capabilities of changing the specification without breaking forward application compatibility by making deduction from ontology information.

We have constructed a new research environment for geo-space science based on 3-D visualization tool and network database; Virtual Earth&#039;s Magnetosphere System (VEMS). With an interactive research environment researchers can visually understand structures of the Earth&#039;s magnetosphere using VEMS. On the VEMS, computer simulation results and observation data are simultaneously visualized, having a potential to data assimilation for geo-space studies in the future. Since the VEMS deals with time-dependent data, it also helps researchers to study dynamics of the Earth&#039;s magnetosphere. We found that immersive data analyses are possible using the VEMS on a virtual reality system.

We propose a Virtual Earth Magnetosphere System (VEMS), which is a new research environment for satellites observation data analysis and computer simulations. The VEMS is constructed on 3-D visualization tools, which are programmable and interactive. We can handle a variety of phenomena over the Earth magnetosphere at arbitrary period from arbitrary viewpoint. In the present study, we constructed the VEMS on a 3-dimensional visualization tool. Application Visualization System AVS/Express Developer. Recent version of the AVS supports time-dependent visualization functions using time-loop modules. We have applied the VEMS to aurora and AKR observation by satellites. In the VEMS, we could successfully visualized the locations of AKR (Auroral Kilometric Radiation) source regions on assumption that the AKR is excited on the magnetic field lines connected with aurora illumination regions on the Earth. As the possible future development, we propose two systems: combination with network database system and with virtual reality system. © 2004 IEEE.

We have developed medical information network and the ORCA network (Project Code "Online Receipt Computer Advantage'') with VPN (Virtual Private Network) technology. The ORCA project states to use IPv6 network to realize secure medical record backup to remote data center and collecting information for EBM (Evidence-based medicine). But as the spread of IPv6 network is still in early stage, and there are many problems with security related implementation, we designed to use the IPv4 based network, but also make the migration to the IPv6 network will be with minimum cost. Our approach is widely accepted and currently over 120 clinics and hospitals are connected with our virtual private network and exchange medical information.

&lt;p&gt; In Ehime, construction of the Virtual Private Network as the base of a medical information network and the ORCA network (Project Code &quot;Online Receipt Computer Advantage&quot;) is currently promoted. As the technology improvement on VPN related technology is so rapid, the fundamental definition for VPN is likely misled and confused. So we have classified VPN-related technology and network layer notion. We have inspected some inherent constraint and variance of IPv4 network architecture. Then we contrived the comprehensive methodology for provisioning VPN over any circumstance by introducing UDP encapsulation with IPsec, dynamic DNS server selection with DNS routing, and the avoidance of network address collision by applying NAPT with IPsec tunnel and distribution unique private network address. As a result, provisioning of medical information network is smoothly ongoing and gear up the prevalence of medical information network.&lt;/p&gt;

In VPN (virtual private networks), security policy and the services provided have hitherto been modeled separately in determining the specifications. The reason is that VPN is generally characterized as a technology for replacing backbone dedicated links, and is regarded as a still-developing means to provide a secure connection to the end user. One key to the widespread use of VPN in SOHO (small offices and home offices) is integrating the services offered by various service providers from the viewpoint of the user, and providing the services securely. As a model of SOHO, this paper considers the case of various medical services surrounding the physician and his office as the site of medical care actions. The present status of development is analyzed and problems are pointed out. A new combination of autonomous decentralized comparison environment architecture and VPN is proposed. © 2003 Wiley Periodicals, Inc.

We have developed a tele-diagnosis system of echography (tele-echography system) to control the echographic diagnosis robot. The examiner moves it by handling a control device apart from the patient and obtaining the echogram and the scene of the patient. The places of the examiner and the patient are connected by network. Remote control was realized by Object Request Broker technique. We verified possibility of our system for clinical use by the experiment. The robot was stable to slight body movement. Remote diagnosis of echogram was possible with sufficient image quality after a skilled examiner got used to control the robot. The adult patient never felt any discomfort to the robot. Since required bandwidth of this system was at most 1 Mbps, we confirmed the ability of this system for mobile robotic telemedicine. Copyright (C) 2003 IFAC

We have developed a wireless tele-echography system for controlling an echographic diagnosis robot (EDR) that was developed in our laboratory. The examiner can control the robot at a distance from the patient and obtains both an image of the patient's status and an echogram of the internal organs. The locations of the examiner and the patient are connected by a wireless network. Remote control was realized by means of the Object Request Broker technique. By this experiment, we have thus verified the usefulness of our system for clinical practice. Remote diagnosis of an echogram with sufficient image quality was possible after a skilled examiner had become used to controlling the robot. The adult patient never felt any discomfort or pain from the robot. Since the maximum bandwidth required by this system was 1Mbps, we have confirmed that wireless tele-echography can become a reality in telemedicine and telecare.

We have developed a remote echographic diagnosis system which consists of mechanism for echo probe to contact with suitable contact force on human body and software to control it via network. Probe movable mechanism is carefully designed not to damage human body. Miniature force sensors detect not only total contact force to body surface but also angle of probe from rectangular to surface. This mechanism is controlled via network by using ORB (Object Request Broker) technique which is recent majority of distribute object system programming. We applied to control the mechanism via 192K-1.5M ISDN (Integrated Services Digital Network). Motion of human heart can be observed in real-time with high quality through 1.5M bandwidth.

To Error is Human report brought to the healthcare domain the concept of Safety Management as a medical accident measure. The introduction of ICT into healthcare information systems should secure 'Patient Safety'. The development of BCMA (barcode medication administration) in the last decade has contributed to a reduction in medical errors. However, BCMA still has some problems that can lead to medical accidents. The major problems with BCMA are poorly designed user interfaces and limited computing resources. As smartphone performance equals or exceeds former high-end workstations, smartphone use will resolve BCMA issues. In this paper, we discuss the institutional and technological backgrounds of healthcare, and present a number of issues to be resolved before the introduction of smartphones in healthcare.

In many scientific fields, most data are digitized and handled on computers and networks. The data files are getting larger in size and numbers. Science clouds, which are equipped with both flexibility and specialty applicable for countless scientific studies, are called upon to play important roles as an infrastructure of big-data sciences and inter-disciplinary studies. The NICT Science Cloud is designed to work with functions of data crawling and transfer, data preservation and management, and data processing and visualization. We have developed applications, tools, systems and services designed for these three functions (especially for Earth and space environmental sciences) installed on the NICT Science Cloud. By applying these functions to each sensing project, researchers are able to advance their studies. In this paper, we first introduce the NICT Science Cloud. We next report our 2-years applications of the cloud system based on several long durational satellite data processing. Finally, I propose a design to make progress of various types of sensing projects; post-project simulator, real-time data processing and archived data processing.
