    本研究では、高齢者の生活の場の転移に伴う排泄ケアに関する施設間での情報共有・協働のために、①排泄に問題を生じやすい高齢者の特徴を明らかにする、②移転時に継続して個別ケアを実践するために必要な施設間での情報共有の内容と方法を明らかにする。特に、回復期リハビリテーション病棟において、急性期から回復期、回復期から慢性期（施設や在宅）へとケアの場が変化する時点に着目して研究をすすめる予定である。
    本年度は、入院・入所・退院後に排泄に関する問題が生じ、他施設との連携を必要としたケースについて、①どのようなケースであったか、②どのような情報共有・連携が必要であったかについて、回復期リハビリテーション病院、特別養護老人ホーム及び訪問看護ステーションの看護師等から情報収集う予定であった。現在、特別養護老人ホームの看護師、急性期病院の医師などからの情報で、尿道カテーテルを留置中の患者について、その適応の理由や抜去の可能性についての情報共有の必要性を確認している。特に、課題になる点は、抜去の際の方法、時期や判断であった。
