    目的:看護シミュレーション教育のファシリテーターの成長のプロセスを明らかにする。方法:質的帰納的研究デザインを用いた。対象は看護シミュレーション教育のファシリテーター経験がある看護師で、2020年8~9月にWebインタビューを行った。データ分析は逐語録から重要発言を抽出、コード化し、相違点、共通点について比較、分類し、集まったものの共通性を見出してカテゴリー化した。所属施設の倫理審査委員会の承認を得て実施した。結果:研究参加者は12名で、看護師平均経験年数は25±10年、ファシリテーター平均経験年数は8±4年であった。データ分析の結果30のテーマから9のサブカテゴリー、4のカテゴリーが得られた。4つのカテゴリーは、【仲間との実践と振り返り】【自分らしいファシリテーター役割の実践】【自律的なシミュレーション教育拡充に向けた取り組み】【ファシリテーター役割の内面化と応用】であった。結論:対象者は、機会を逃さずに実践を繰り返し、指導者から継続的な支援を受け、仲間との内省を行っていた。それにより、ファシリテーター役割を概念化し、さらにファシリテーションスキルを日常看護業務における新人看護師指導に役立てるなど応用もしていた。ファシリテーター支援には、基本的なスキルを教授するとともに、お互いに支えあえるコミュニティの形成が必要である。(著者抄録)

    医療現場へのAIの導入は容易にイメージできる。AIにより診断技術が大きく進歩し、医師の役割に変化が生じることも予測されている。この変化を見越した医学教育を構築する必要性も理解できる。一方で医学教育現場へのAI導入は現在は模索中である。シミュレータの機能充実に対するAI活用は解りやすい。臨床データからシミュレーション教材を作成することも可能と考える。教育方略としてのシミュレーション教育にAIを取り入れることは急速に進められるであろう。教育目標におけるニーズ分析へのAI活用も考えられる。新しい学習者評価の基準策定にもAIは活用できるであろう。積み重ねられた「膨大な量の間接証拠」をAIを用いて解き明かし、現状では曖昧な教育上の諸問題を解決に導けるかもしれまい。AI時代の到来により、「未来の医師を、現在育てている」という認識を、われわれ教育者はもたなければならない。(著者抄録)

    教室での多職種連携教育はCOVID-19のため困難となった。オンラインでの多職種連携教育のノウハウを共有するため、2大学で実施したオンラインの初年次学生を対象とした多職種連携教育の実践報告を行う。三重大学ではZoomを用いた授業を実施し、クイズやチャットなどで教員と学生とのインタラクションを促す仕掛けが機能した。北海道科学大学ではZoomとLearning Management System(LMS)を用いてチーム医療体験ゲームを行い、ゲーム性と明確な指示のもと学生間のインタラクションが促進された。どちらも既存の多職種連携教育をオンラインでうまく活用し、学生の職種役割の理解やメンバーシップ/チームシップの理解は達成されたが、教員の慣れや時間配分などの課題があった。(著者抄録)

Background: Although cardiopulmonary resuscitation (CPR) training is recommended in schools, there are few attempts to train all students at universities and no reports showing actual resuscitation actions at emergency settings after the training. We surveyed how many students encountered a collapsed person, whether they performed any resuscitation actions, and any reasons why they could not do any resuscitation actions. Methods: We have provided chest compression-only CPR and automated external defibrillator (AED) use training for 3000 new university students every April since 2015 and followed up on their subsequent emergency actions to collapsed persons in the real world. We carried out a questionnaire survey for 2nd through 4th-year students during the annual student health checkup period in 2018. Results: A total of 7595 students underwent the annual health checkup and 5549 of them (73.1%) responded to the survey. The rates of encountering collapsed persons and out-of-hospital cardiac arrest (OHCA) patients were 2.5 and 1.1 per 100 person-years, respectively. Of the 264 students who encountered a collapsed person, 82 (53.6%) who encountered non-OHCA collapsed persons and 54 (48.6%) who encountered OHCA persons performed at least one resuscitation action including either chest compression, AED use, or any other various resuscitation actions. Conclusions: The incidence rate of encountering OHCA patients was 1.1 per 100 person-years and half of them who encountered a collapsed person performed at least one resuscitation action. Hands-on mass training would encourage university students to perform any resuscitation actions on the emergency scene.

    BACKGROUND: Suicide is a crucial global health concern and effective suicide prevention has long been warranted. Mental illness, especially depression is the highest risk factor of suicide. Suicidal risk is increased in people not only with mental illness but also with physical illnesses, thus medical staff caring for physically-ill patients are also required to manage people with suicidal risk. In the present study, we evaluated our newly developed suicide intervention program among medical staff. METHODS: We developed a 2-h suicide intervention program for medical staff, based on the Mental Health First Aid (MHFA), which had originally been developed for the general population. We conducted this program for 74 medical staff members from 2 hospitals. Changes in knowledge, perceived skills, and confidence in early intervention of depression and suicide-prevention were evaluated using self-reported questionnaires at 3 points; pre-program, immediately after the program, and 1 month after program. RESULTS: This suicide prevention program had significant effects on improving perceived skills and confidence especially among nurses and medical residents. These significant effects lasted even 1 month after the program. LIMITATIONS: Design was a single-arm study with relatively small sample size and short-term follow up. CONCLUSIONS: The present study suggests that the major target of this effective program is nurses and medical residents. Future research is required to validate the effects of the program with control groups, and also to assess long-term effectiveness and actual reduction in suicide rates.

    日本医療機能評価機構が公表するデータを用いて、看護師の臨床経験年数と部署配属年数によるヒヤリ・ハットの発生傾向の特徴を明らかにするために、公開されたヒヤリ・ハット事例のうち、2015年4月から2016年3月までの5092件を分析した。職種経験年数は1年未満、1年以上3年未満、3年以上5年未満、5年以上とし、部署配属年数を1年未満、1年以上とし、これを組み合わせて臨床経験レベルを「新人N/A/B」「一人前A/B」「中堅以上A/B」の七つに分類した。この結果、部署転属1年未満の間は臨床経験年数が長い看護師であっても、ヒヤリ・ハットの発生傾向が大きく異なることが示され、留意が必要であることが明らかになった。新人レベルでは臨床経験が1年以上であれば部署配属年数による影響は見られないが、一人前、中堅以上レベルでは異なっていた。看護師のキャリアラダーは臨床経験年数と部署配属年数を組み合わせた「臨床経験レベル」を考慮する必要があることが示唆された。

    <p>This paper proposes a novel trainer for training of blood collection with an injector, an essential skill in medical and nursing fields. An optical motion capturing system and a projector are employed in the proposed system, in addition to the conventional blood collection trainer. The proposed system guides the injector to ideal position and posture until piercing the surface of the simulator. Then, after piercing, subsurface information including the position and the depth of the tip is indicated. Experimental results with subjects have demonstrated that the feedback provided by the proposed system is useful to improve the skill of the blood collection.</p>

    遠隔診療におけるコミュニケーションを理解することは遠隔診療のための情報システムやワークフローを検討する上で重要である.本研究では遠隔診療のコミュニケーション分析を目的とし,まず,初期的検討として聴診訓練シミュレータを使用した模擬遠隔聴診において医師と聴診経験がない介助者各1名を対象とする観察実験を実施した.実験は健常3例と呼吸器系疾患3例について実施した.分析では,被験者へのアンケートとインタビュー,被験者に提示する映像と音声の記録,実験環境の外観をカメラで撮影した映像・音声により,多角的な分析を試みた.結果より,本実験で設定した状況下における医師と介助者の視覚的・言語的行為連鎖のパターン化,コミュニケーションモダリティの遷移など,今後分析を進める上での基礎的な知見が得られた.(著者抄録)

    In tele-auscultation, since a doctor cannot operate an auscultator by herself, she must refer a target point by a marker and have a helper in a distant place move an auscultator by proxy. This article analyzes a simulated tele-auscultation experiment and proposes an interactional pattern observed in the process of multimodal communication from pointing by marker by a doctor, operation of an auscultator by a helper to auscultation by the doctor. This pattern is then considered in terms of division of transmission between multi-channels of a system for tele-auscultation and a tele-conference system for conversation. At the end, problems on the system environment found in the experiment are addressed.

    Purpose: The purpose of this study was to understand the reality shock experienced by recent nursing school graduates and to explore ideal nursing educational methods that allow for a smoother transition from schools to clinics. Methods: The participants were 10 nurses who worked for university hospital "A" following their graduation from university "A". The Phenomenological qualitative method was conducted using depth-interview after the ethical approval of IRB of university "A" and university hospital "A". Results: Interview analyses shows that recent nursing school graduates believed themselves to be inadequately prepared for the unexpectedly high demands of clinical work, including both medical practices and the ability to effectively interact with senior colleagues. It has become clear that nursing students need to have real high-risk care clinical practice and to brush up non-technical skills. Discussion: It is suggested that to support high-quality clinical practice, there needs to be a coherent educational system that includes scenario-based high-risk nursing care simulation learning methods as practiced in industrialized countries.

    This paper discusses evaluation of a vision-based remote navigation system for tele-auscultation. In tele-auscultation, the doctor needs to navigate a non-clinical proxy handler of the stethoscope. The system provides doctor's navigation of the stethoscope as a graphics marker overlaid on a view of the target's chest. The system is evaluated in a simulated tele-auscultation experiment with a lung auscultation simulator. Auscultation time was reduced when the system was used. The results of conversation analysis show that the system has reduced the indication of auscultation position and position tuning. The subjects commented in the questionnaire that the system smoothened navigation and handling of the stethoscope.

    Objective: Development of a clinical sensor network system that automatically collects vital sign and its supplemental data, and evaluation the effect of automatic vital sensor value assignment to patients based on locations of sensors.
    Methods: The sensor network estimates the data-source, a target patient, from the position of a vital sign sensor obtained from a newly developed proximity sensing system. The proximity sensing system estimates the positions of the devices using a Bluetooth inquiry process. Using Bluetooth access points and the positioning system newly developed in this project, the sensor network collects vital sign and its 4W (who, where, what, and when) supplemental data from any Bluetooth ready vital sign sensors such as Continua-ready devices. The prototype was evaluated in a pseudo clinical setting at Kyoto University Hospital using a cyclic, paired comparison and statistical analysis.
    Results: The result of the cyclic paired analysis shows the subjects evaluated the proposed system is more effective and safer than POCS as well as paper-based operation. It halves the times for vital signs input and eliminates input errors. On the other hand, the prototype failed in its position estimation for 12.6% of all attempts, and the nurses overlooked half of the errors. A detailed investigation clears that an advanced interface to show the system's "confidence", i.e. the probability of estimation error, must be effective to reduce the oversights.
    Conclusions: This paper proposed a clinical sensor network system that relieves nurses from vital signs input tasks. The result clearly shows that the proposed system increases the efficiency and safety of the nursing process both subjectively and objectively. It is a, step toward new generation of point of nursing care systems where sensors take over the, tasks of data input from the nurses.

    The aim of this research is to develop an information support system for tele-auscultation. In auscultation, a doctor requires to understand condition of applying a stethoscope, in addition to auscultatory sounds. The proposed system includes intuitive navigation system of stethoscope operation, in addition to conventional audio streaming system of auscultatory sounds and conventional video conferencing system for telecommunication. Mixed reality technology is applied for intuitive navigation of the stethoscope. Information, such as position, contact condition and breath, is overlaid on a view of the patient's chest. The contact condition of the stethoscope is measured by e-textile contact sensors. The breath is measured by a band type breath sensor. In a simulated tele-auscultation experiment, the stethoscope with the contact sensors and the breath sensor were evaluated. The results show that the presentation of the contact condition was not understandable enough for navigating the stethoscope handling. The time series of the breath phases was usable for the remote doctor to understand the breath condition of the patient.

    Proton pump inhibitor (PPI) is an effective and safe medication for the elderly people for the treatment of peptic ulcer disease. However, some PPIs have been reported that they have metabolic interactions with some drugs. Therefore, drug-interactions should be considered when the PPI is prescribed to the elderly people. The number of NSAIDs ulcer patients is thought to increase along with the increase of those who take NSAIDs in the elderly. Although PPI is indispensable for the prevention of the NSAIDs ulcer, PPI has not obtained authorization for the purpose of prevention in Japan. PPIs are strongly expected to be approved for prevention of NSAIDs ulcer by the Japanese government in the near future.

    豪雪地域における退院患者の特徴を明らかにすることを目的として,新潟県内病院の一内科系病棟の退院患者を対象に,診療録からの調査を行った.対象患者317名の平均年齢は71.7歳(SD17.5)で, 65歳以上が全体の8割を占めた. 1月と8月の入院患者数および冬季の退院患者数が多い傾向にあった.ほとんどの患者が基礎疾患を持ち,半数の患者が肺炎などの呼吸器疾患やポリープ,胃腸炎などの消化器疾患をきっかけに入院していた.在院日数は平均19.4日(SD24.6)であった.退院時の状況では, 8割の患者が自宅へと退院していた.退院時の食事・排泄・清潔・移動・整容のADLの全項目において, 7割の患者が自立していた. 4割の患者は退院後も内服などの継続処置を必要としていた.家族背景では,全体の8割が配偶者や息子などと同居していた.ほとんどの患者が配偶者,息子配偶者などの介護者を有し,半数以上の患者に介護協力者が存在した.社会的サポート体制では,介護保険適用患者のうち7割が介護保険制度を使用中または申請済みであった.ホームドクターのいる患者は46名,他17名が訪問看護サービスを, 9名が医療ソーシャルワーカーやホームヘルパーによるサービスを利用していた.人口の減少と高齢化の進行に伴い,今後も予想される家族の介護力の脆弱化を見据え,介護者の健康と安寧を支える社会的サポートの充実が必要であると示唆された.

    豪雪地域で生活する療養者とその家族の療養生活の特徴を明らかにすることを目的とし,9事例の訪問看護場面の参加観察と面接調査を行った.得られたデータは,&lt;療養環境に関すること&gt;&lt;療養の日常生活に関すること&gt;&lt;療養の健康管理に関すること&gt;&lt;介護者に関すること&gt;&lt;在宅ケアへの思い&gt;の5つの側面から特徴を分析した.結果,介護者は療養者の「食べることの維持」と「規則的な排泄の維持」を介護の大きな課題として認識し,試行錯誤しながら独自の方法を確立していた.また,療養者と家族のセルフケア向上のためには,《口腔ケアの必要性と方法の指導》《家庭での良肢位とリハビリテーション指導》が課題であった.《移動・外出を支援するサービスの充実》《「来てくれる」専門家の配備》が大きなニードとしてあり,これらのニードは外出や療養者の健康管理が困難となる冬季においてさらに高まっていた.その他,《療養者の視点にたったサービスの柔軟性》を検討することや,介護者が自らの行っている介護の価値に気づき,要介護者とよりよい信頼関係を築いていくことができるような《介護者を理解し,話しを聞く存在とサポートシステムの構築》も重要であると考えられた.

    本研究は、思考力・判断力を兼ね備えた助産師の育成を目指し、アクティブラーニング(AL)型の様々なシミュレーションシナリオの体験を通して、助産実践能力の習得を可能とする助産師育成の基盤となるプログラムを開発することを目的としている。対象者は、助産師学生から卒後2年目までの助産師である。今年度は、プレテストとして、専攻科で学ぶ助産師学生を対象に、入学時から臨床実習前,臨床実習後,卒業時の3つの時期に、3場面(a:妊娠健康診査,b:分娩進行,c:産褥期と新生児)でシミュレーション学習を実施した。入学時~臨床実習前の学習目標は、周産期の場面イメージすること、円滑なコミュニケーションはかることで、学生間で知識や情報を共有することでコミュニケーションスキルの向上ができた。臨床実習後では、助産実習で実施した助産ケアを振り返ることを目的に、助産診断および助産ケアの実施計画の立案を行った。学生は、実習での体験も振り返りながら、明確な医学的根拠をもって助産診断や助産ケアの実施計画ができていた。卒業時は1年間の学習を統合するために、異常の早期発見および妊娠期から産褥期までの継続的な視点で専門職としての自立を目指す目的で実施した。助産師学生は、継続的に対象に関わることで、対象のニーズに沿った助産ケアの重要性を感じていた。学生が主体的に演習を実施することで、学生は積極的に意見を述べるだけでなく、互いの意見も傾聴するなど、仲間を大切にしながら能動的に演習に取り組む様子が見られていた。また、実習前に行う演習は、臨床場面のイメージ化を可能にし、瞬時に行動するための訓練に繋がっていた。さらに、学生は実習後の演習を通して助産ケアの根拠を明確にし、継続的な視点を忘れることなく対象のニーズに沿った助産ケアを展開できることが明らかになった。なお、県立広島大学研究倫理審査の承認を得ている。

    We interviewed facilitators of nursing simulation education about their process of growth. The subjects did not miss opportunities to repeat the practice, received continuous support from their instructors, and engaged in reflection with their peers. They conceptualized the facilitator role and applied their facilitation skills to teaching new nurses in their daily nursing work. It was suggested that ongoing involvement with a mentor and forming a mutually supportive community are necessary to support facilitators. The study also revealed that experienced facilitators utilize the following skills in debriefing situations: placemaking skills, facilitating dialogue and learning, and structuring skills.

    In this study, using virtual reality (VR) technology, we aimed to develop an education model for assessing high risk foot lesions, to use in training nurses who provide diabetes-related preventive foot care.
    Responsible for planning, coordination and generalization of the whole, three groups were created: VR training system development; education model velopment; diabetes preventive foot care experts.
    The VR training system was developed with continuous information exchange, the equipment maintenance and simulation carried out, and trials were completed by experts. At the same time, our team developed an educational model, packaging educational content and clinical ability evaluation that incorporated a VR training system. Created and evaluated models were ranked according to three categories of high, medium and low difficulty.

    The purpose of this study was the development of a new program for facilitators in simulation education.
    We conducted interviews between December 2016 and January 2017 with 16 instructors at Japanese medical institutions. The interviewers also conducted focus group discussions. We found five categories of difficulties for the facilitators. The facilitators also possessed their own skill sets, outlooks and personal beliefs.The future tasks are to strengthen the current facilitator development program and to create a self-assessment table to gain the confidence of facilitators.

    This study aims to develop an evaluation scale that can objectively measure nursing performance by CL for generalist nurse.For CL level to be even within the sample, stratified random sampling was used until there were 50 nurses of A university hospital in each ladder. Briefing sessions were held with potential participants, and the 249 nurses who were targeted agreed to participate in this study.
    To examine whether there is a difference in results depending on the order of the questions, simulations were conducted to change the order, and it was confirmed that there was no difference. The time required for judgement was convenient, as short as about 30 minutes.
    CL is a common concern of nursing managers not only in Japan but also worldwide.This system is an efficient and useful tool that can judge CL in a short time according to the respondent’s ability. Future research should further develop this system that can help busy nurses.

    The purpose of this study was to build the new educational system in cooperation with clinical side to improve the gaps of competency between the current clinical practice and the basic nursing education. We explored the gaps through the qualitative and quantitative studies on the competencies of nursing students before graduation and new graduates on 6 to 12 months. The simulation learning was chosen the best practice to improve the gaps. The attendant's satisfaction was high in each workshop so that it was clear that simulation learning was effective. During these three years, we had twelve presentations of our study at the international and national nursing conferences, published one paper, two reports for the journal, and now still three papers writing.

    In order to obtain information for the development of a discharge instruction program in elderly patients with femoral neck fracture, the medical records of 105 male and female patients, aged from 66 to 100 years old, were examined. Duration of hospitalization was affected by whether the patient had Long-Term Care Insurance or not, as well as the waiting time before surgery. Waiting time of 3 or more days was associated with an average of 6 days longer admission. The BI score on discharge was significantly lower for those patients who had a low BI score on admission for the parameters[eating],[micturition],[defecation], and for those patients where postoperative confusion was observed.
