思春期・青年期1型糖尿病患者と保護者のトランジションにおけるレディネス(準備状態)の認識を明らかにした.12〜20歳の患者と保護者27組に対し,患者へはトランジションにおけるレディネス,内科への転科の意志とその理由,保護者へは保護者が認識する患者のレディネス,転科の意志とその理由を調査した.その結果,レディネスの知識の総得点の中央値(範囲)は患者34(16-44)点,保護者33(25-41)点,行動は患者29(19-45)点,保護者30(21-42)点で両者に有意差は認められず,各項目得点でも有意差は認められなかった.よって,保護者は患者のレディネスを適切に把握していると考えられた.しかし,レディネスが形成されている患者は5名(18.5%),内科への転科の意志がある患者は6名(22.2%),保護者は8名(29.6%)のみのため,早期から移行の準備を進める支援が必要である.(著者抄録)

＜文献概要＞本研究の目的は,1型糖尿病をもつ年少児の糖尿病セルフケアに向けた親のかかわり尺度を開発し信頼性・妥当性を検討することである.文献検討および予備調査,専門家会議を経て58項目の試行版を作成し,1型糖尿病をもつ1歳〜小学校低学年の子どもの親34名のデータを得て,24項目から成る最終版を作成した.主因子法,プロマックス回転により因子抽出を行い,F1 糖尿病管理における負担,F2 子どもの低血糖対処能力の把握,F3 低血糖や血糖値に起因する余裕の欠如,F4 幼稚園や学校のサポート,F5 糖尿病管理の支えと子どもの将来を見すえたかかわり,F6 糖尿病管理と育児の自信,F7 子どもの意欲や関心の把握,を得た.24項目のCronbach's α係数は0.84,再テスト法でのPearsonの積率相関係数は0.93と許容範囲内であった.本尺度は,項目数が24項目と少なく,年少児に対する親の認識やかかわり,親のストレスやサポートを包括的に査定するために臨床での活用が期待できる.今後は,より大きなサンプルサイズでの分析を継続し,安定した構造をもつ尺度に洗練していく必要がある.

<p> This study's objective was to assess the readiness for transition to adult healthcare services among Japanese adolescents with type 1 diabetes. We conducted a self administered questionnaire survey in 27 patients with type 1 diabetes (aged 12-20 years). The survey assessed the patient's readiness to shift to adult healthcare services using the Am I ON TRAC for Adult Care Questionnaire (Saewyc, 2012), acceptance of disease, and self-efficacy for diabetes. The results of a questionnaire, only 5 patients (18.5%) answered that they were ready for transition. About the median total scores (by stage of growth) for knowledge and behavior about readiness, high school students scored the highest points, and scores of university students were lower than them. The results suggested that patients couldn't obtain knowledge and behavior of readiness without learning opportunities. This study also revealed that knowledge and behavior of readiness were correlated with acceptance of disease and self-efficacy for diabetes. Especially, self-efficacy for diabetes was correlated with behavior about consult and judgement.</p>

<p> The purpose of this study was to investigate the efficacy of continuous nursing intervention utilizing ICT for children with type 1 diabetes. We included nine children aged 8 to 15 years who were asked to input data, such as blood glucose and insulin levels, on tablet-style mobile devices, using a downloaded application (e-SMBG) for the self-management of diabetes. We reviewed the data input by the children and provided guidance in accordance with nursing intervention for about two months. HbA1c levels, the score of diabetes Self-Care Inventory-revised, and QOL scale before and after intervention were analyzed using <i>t</i>-test and Wilcoxon signed-rank test. Two months later, HbA1c levels significantly decreased and QOL scores increased. Utilizing ICT helped timely guidance and support matched to the children's lifestyle and improved their motivation and awareness toward independence of self-care behaviors. Supportive interventions utilizing ICT are effective for children who experience psychological problems requiring long-term intervention.</p>

The purpose of this study was to investigate the utilization of a childhood diabetes self-care support system at a diabetes camp. This study was designed to verify the timely sharing of information between health professionals during the camp and the efficacy of the system as an educational tool. The subjects of this study were 22 health professionals. Using a tablet assigned, the health professionals accessed a specialized homepage to view data portrayed in a graph format and to conduct educational intervention, such as input feedback comments about daily blood glucose control. The results of a questionnaire survey conducted after using the system suggested that the tool was useful for data sharing, utilization in meetings, and educational intervention. The reasons of positive appraisal included availability of enough information regarding blood glucose and insulin and visibility of the graph screen display. However, the system requires further improvement. Issues that still need to be addressed in the future include improvement for data transmission environment, access rapidity, securing a sufficient number of tablets, and verification of the effects of long-term use.

The purpose of this study was to investigate the utilization of a childhood diabetes self-care support system using tablet-type portable devices aimed to improve self-care ability and sense of self-efficacy in children with childhood diabetes. The subjects consisted of 15 children with type 1 diabetes, aged from 9 to 18 years. The subjects were asked to input data such as blood glucose levels during the diabetes camp on a specialized application that we developed for tablet devices. Changes in the summary of diabetes self-care activities (SDSCA) measure and self-efficacy scale scores were analyzed before using the system, immediately after using the system, and 1 month later. The system was evaluated based on the evaluation scores of the application and children's reactions while using the system. SDSCA measure scores significantly increased immediately after the camp. Enjoying the system to record their data and looking over their own data on a graph screen in an integrated fashion, children were able to easily link their behavior and insulin dosage to their blood glucose control. The results suggested that this system has the educational effects of encouraging children to be aware of their blood glucose levels and generate motivation for controlling it.

国立A大学看護学科の全卒業生706名を対象とし分析を行った。方法は、文科省の『学士課程で育成する看護実践能力I〜V群』を参考に作成した質問票で看護実践能力の測定を行うとともに、「卒後年数」「大学在学中の学習内容の有用性」「学会や研究会への参加」などについて質問紙調査し、看護実践能力との関連性を統計学的に検討した。結果、看護実践能力と強い相関を示したのは「大学在学中の学習内容の有用性」と「学会や研究会の参加」であり、特に「大学在学中の学習内容の有用性」を構成する要素の【科学的思考力】が看護実践能力に大きな影響を及ぼしていた。

The purpose of this study was to analyze the short-term and one-year outcomes of children with type 1 diabetes who participated in a diabetes camp. A national survey of the educational effects of diabetes camps (915 subjects) obtained valid answers from 247 subjects from 24 camps, which were analyzed according to the type of participation (continuous, experience, first-time, and non-participation) . The analysis revealed; (1) the appreciation of the camp was extremely high, and "making friends" was the most important goal of the camp; (2) the continuous-participation group showed a significant improvement in the score of the self-efficacy scale for diabetes after participating in the camp for one year. In addition, the participants created an environment in which they could readily ask for help when their symptoms became aggravated, by becoming more open about having diabetes and improving their relationships with people around them; and (3) the HbA1c levels continuously decreased after participating in the camp for one year in the continuous-participation and first-time participation groups. Participation in a diabetes camp is an important experience for adolescents living with type 1 diabetes which allows them the opportunity to gain confidence and an improved control of diabetes mellitus.<br>

The goal of this study was to determine the mentoring relationships and mentoring experiences of young adults with type 1 diabetes. Qualitative and quantitative data were obtained to assess the mentoring relationships and experiences of young adults. To assess the mentoring practice and mentoring experiences of seven young adults, a qualitative descriptive analysis was performed. Two different communication patterns were found in the mentoring relationship. Some mentors were successful at expressing sincere concerns for their mentees via active listening skills, while others were unable to maintain strong intimacy with their mentees, and the relationships ultimately ended. It was difficult to maintain strong intimacy between mentees and mentors over the long term. However, mentoring increases young adults' sense of responsibility and their positive roles as a mentor. Moreover, young adultsfound theirnew potentialities as they determined living with diabetes.

The purpose of this study was to develop and determine the effectiveness of a nursing mentoring intervention program for adolescents with type 1 diabetes. A coping skills training program using a group intervention session and online mentoring was providedvia e-mail or telephone over a 12-month period. Ten adolescents and 7 young adults with type 1 diabetes completed this program. The findings suggest that the use of the group-based coping skills training session and mentoring intervention improved the metabolic control of diabetes in adolescents over a 12-month period and facilitated their self-efficacy of diabetes management over a 6-month period. Overall, the adolescents enjoyed the relationship with their mentors during the intervention period. However, there was a significant decrease in the mentoring relationship at 12 months post-session. The online mentoring is not ideal for establishing and maintaining strong connections between mentees and mentors over the long term. Additional research is needed to increase the beneficial effects of mentoring for mentees. The basic structure (e.g., ongoing guidance for mentors and training them to develop strong communication skills, early support for relationship building, and supervision of mentoring relationships) of mentoring programs must be strengthened if they are to be successful.

第4回全国ヤングDMカンファレンスで実施したグループディスカッションの内容を分析し、青年期以降の1型糖尿病患者が抱える課題を明らかにした。患者の抱える課題には、【自己の身体的コントロールに対する課題】、【成長とともに顕在の可能性がある課題】、【社会生活を営む上で生じる課題】、【社会資源・保障における課題】、【社会的理解に対する課題】、【活動する仲間や後継者育成に対する課題】の6つの課題が見いだされた。発症年齢・罹病期間によって抱える課題には特徴がみられ、小児期発症患者では、進路や結婚といった青年期特有の発達課題が示された。同時にサマーキャンプにおける自己の役割や、後継者の育成方法の模索といった社会へ向かう力への意識が強かった。一方で思春期以降に発症した患者では、疾患の自己管理に直結する課題に関心が高い傾向がみられた。今後、発症時期の異なる患者同士の交流をすすめ、体験の共有による問題解決を導くシステム構築の必要性が示唆された。(著者抄録)

本研究の目的は、小児看護学実習における学生の体験から、学生が描いた関連図の特徴や学びを分析し、関連図を活用した実習での学習効果を明らかにすることである。実習を終了した4年制大学の3年生42名を対象に、関連図を用いた実習展開の学びについて自由記述式のアンケート調査を実施し、内容分析を行った。学生の自由記述の分析結果から、関連図を用いた学習効果に関する内容として230のコードと8つのカテゴリーを抽出した。カテゴリーは、さらに学生の実習展開を発展させた要因と、逆に実習展開を困難にした要因の二群に分類できた。学生の実習展開を発展させた要因は、【看護過程における論理的思考の強化】、【対象理解】、【情報の整理統合】、【関連図の効果的活用】であった。一方、実習展開を困難にした要因は、【描き方の難しさによる混乱】、【修正の繰り返し】、【枠組みのない情報収集とアセスメント不足に対する不安】、【情報収集・整理の難しさ】であった。関連図を活用した実習は、看護過程における学生の論理的思考を強化し、情報を整理統合する能力を育成するだけでなく、対象に応じた個別性のある看護計画の立案や、根拠に基づいた介入方法やケアの実践を促進する効果があることが示唆された。(著者抄録)

1型糖尿病に罹患した子どもの血糖自己測定に伴う痛みと,それに関連する特徴的な思いについて,その時間的変化と影響要因を明らかにすることを目的に,44名(男子21名,女子23名.6〜26歳,平均年齢13.7±4.5歳.平均血糖測定回数3.2±1.4回/日)を対象に半構成的面接を実施した.その結果,以下のことが分かった.1)血糖自己測定に伴う痛みについて,最初は痛かったが調査時には痛くない,と表現しており,VASによる調査でも痛みは発症時から時間の経過とともに有意に低下していた.また,「痛み」に影響する要因として,「女子」「高校生」「思春期発症群」が有意に高いことが示された(p&lt;0.05).2)血糖自己測定に対して「めんどう」「いや」という思いが特徴的であり,この思いは年齢が高くなるほど強いことが示された(p&lt;0.05).これらの結果から,血糖自己測定に対する子どもの思いに対する支援では,発症時には痛みを軽減するための支援が重要であり,思春期を中心に「めんどう」「いや」といった思いを軽減するための支援を継続する必要性が示唆された

This study explored the regeneration process of families who faced difficulties in dealing with children's psychosomatic behavioral disorders and described the characteristics of family hardiness.<BR>Data were collected through semi-structured interviews with 16 families and/or participant observations during counseling sessions. Interviews were audiotaped with permissions and transcribed verbatim and analyzed according to the grounded theory approach. The findings suggested that&ldquo;<I>synchronization</I>&rdquo; was a core category that described the process of how families of children with behavioral disorders coped with the frightening and discouraging events surrounding their children and how new family functions were regenerated through the child-parent interactions.<BR>This family regeneration process consisted of three phases:&ldquo;<I>in-limbo</I>,&rdquo;&ldquo;<I>stagnation</I>,&rdquo; and &ldquo;<I>getting out of chaos</I>.&rdquo; Each phase was constructed with the family interactions of&ldquo;<I>conflicting and relinquishing</I>,&rdquo;&ldquo;<I>groping in the dark and being involved with</I>,&rdquo;and&ldquo;<I>confirming the emotional distances and reconstructing the relationships of trust</I>,&rdquo; respectively. The family members got deeply involved in both their children's psychological and behavioral difficulties in these phases; however, they came to terms with their children's outlasting behaviors. While the families and children retraced these three phases repeatedly, new family ties and the best emotional distance between child and family members were established. Family hardiness was the most important strength that accelerated the family regeneration and psychological stability in families.

愛媛県下の精神保健指定医のいる病院及び診療所53施設,公的保健機関15施設を対象に,郵送によるアンケート調査を行った.また,精神保健福祉センター酒害相談員2名と御荘支所アルコール教室担当保健婦1名を対象に,インタビューを行った.アルコール依存症者を支援する専門医有り6施設,専従スタッフ有り5施設,専門病床有り2施設であった.スタッフが学習するための研修会への参加有り6施設であった.公的保健機関での,アルコール依存症者への支援は個別相談がほとんどで,集団教育は南予のみで開催されていた.酒害相談を行っている断酒会会員は,公的保健機関に対して,啓発や支援を望んでいた.以上より,今後の課題として,啓発活動,患者と家族が身近に通える集団教育の提供,システムの整備,スタッフ等の研修の充実が示唆された
