    BACKGROUND: We conducted a randomized controlled trial to investigate the effects of consumption of yogurt fermented with Lactobacillus delbrueckii ssp. bulgaricus OLL1073R-1 in women healthcare workers. In a previous study we used these data to investigate hypothesized preventive effects against flu, however any effects on improving mental quality of life were not analyzed at that time. In the present study, we focus on that aspect. METHODS: The participants (961 women; mainly nurses, aged 20-71 years) were randomly allocated to either the yogurt group (n = 479) or the control group (n = 482). Participants in the yogurt group drank 112 mL of OLL1073R-1 yogurt for 16 weeks, while those in the control group did not consume any yogurt. All participants were prohibited from consuming other yogurt or fermented dairy products during the study period. The participants answered the Pittsburgh Sleep Quality Index (PSQI), Short Form-8 Health Survey (SF-8), and Gastrointestinal Symptom Rating Scale (GSRS) questionnaires at baseline and after 16 weeks. RESULTS: The PSQI score showed significant improvement after the intake of yogurt (p < 0.01). SF-8 results showed significant intervention effects in the General Health and Vitality scores (p = 0.02 and p = 0.01, respectively). In other subscales of SF-8, we did not observe significant effects of the yogurt. In the GSRS, daily intake of yogurt exerted a preventive effect on constipation (p = 0.03). CONCLUSIONS: Consumption of yogurt fermented with Lactobacillus delbrueckii ssp. bulgaricus OLL1073R-1 enhances subjective psychological quality of life by improving quality of sleep and gastrointestinal condition among women healthcare workers.

    <p><b>Aim: </b>The effects of alcohol consumption on Mild Cognitive Impairment (MCI) among the Japanese population had not been fully examined. Therefore, the objective of this study was to examine the association between alcohol consumption and MCI among the Japanese elderly population.</p><p><b>Methods: </b>In total, 421 men and 700 women aged 60-84 years participated in this cross-sectional study. Alcohol consumption was estimated according to frequency and amount of major alcoholic beverages (i.e., beer, Japanese sake, shochu, and wine) consumed by each individual using a self-administered questionnaire. MCI was assessed using the Japanese version of the Montreal Cognitive Assessment. Multivariable odds ratio (OR) and 95% confidence intervals (CIs) of MCI according to alcohol consumption were calculated using logistic models. We further analyzed the associations of the major alcoholic beverages with MCI.</p><p><b>Results: </b>The prevalence of MCI was 50.4% among the male participants and 31.4% among the females. A positive association between alcohol consumption and MCI was observed in men, but not in women. The multivariable OR (95% CI) of MCI for ≥ 2 go (46 g ethanol) /day vs. non-drinkers was 1.78 (0.93-3.40, p for trend = 0.045) in men and for ≥ 1 go (23 g ethanol) /day was 0.96 (0.39-2.38, p for trend = 0.92) in women, respectively. We also observed an association between shochu consumption and MCI in men, whereby the multivariable OR (95% CI) of MCI for each 1 go increment was 1.57 (1.18-2.07).</p><p><b>Conclusion: </b>Our findings suggest that alcohol consumption in moderation may contribute to the prevention of MCI development in men.</p>

    <p>  This study aimed to examine social activity in elderly persons requiring support and identify factors that exert an influence on their social activities. The subjects were elderly persons age ≥65 years who been certified as requiring support and who lived in an area managed by A community General Support Center, located within a designated core city. Excluding care facility residents, the total number of subjects was 788. Survey content included social activity and the individual, physical, and social factors that likely exert an influence on it. The survey was conducted through anonymous, self-administered questionnaires. Valid responses were obtained from 411 subjects, and factors related to social activity were identified through multiple regression analysis. The results showed that social activities of elderly persons requiring support often included interaction with the community or with neighbors. The following five factors were found to be related to social activity: score on the Tokyo Metropolitan Institute of Gerontology Index of Competence, certification category, provision of social support, non-family support network, and use of public transportation. Maintaining physical function, as well as supporting relationships with friends or acquaintances in the community and role-players that can provide support are suggested to be crucial for promoting social activity in elderly persons requiring support.</p>

    <p>  The objective of the present study was to elucidate the actual conditions of cooperation between nurses and other professionals and the support for confirmation of intention regarding end-of-life care, as well as whether these factors are related to end-of-life care, at special nursing homes in Ehime prefecture. A questionnaire survey was conducted on nurses working at 94 special nursing homes in Ehime prefecture, and responses were obtained from 37 facilities. The most common timing of confirmation of intention regarding end-of-life care was following worsening of condition (91.9%), followed by the time of admission (70.3%). Nurses participated in the confirmation of intention at the time of admission at only 24.3% of facilities. As for relationships to end-of-life care, significant differences were observed based on physicians' attitude toward end-of-life care, cooperation among nurses, and working arrangements of nurses. Specifically, at facilities with a high rate of end-of-life care, physicians had a positive attitude toward end-of-life care, nurses actively engaged in exchange of opinions among each other, and nurses worked late shifts. These results suggest that in order to promote end-of-life care at special nursing homes, it is important for nurses to cooperate with physicians and to support night-time care workers.</p>

    <b>Objective: </b>This study aimed to elucidate the urination status among elderly people with incontinence who require at-home nursing care as well as the status of their family caregivers, and to identify the factors related to their urination methods. <b>Methods: </b>In the present study, questionnaires were distributed to 101 elderly subjects (n=101) and their caregivers. <b>Results: </b>The most prevalent urination method was the collaboration of toilet and adult diapers (69, 68.8%).There was a significantly larger number of patients who used a toilet <4 times and who had a "moderate" amount of incontinence and patients who used toilet 4-8 times and had a "slight" amount of incontinence (p<0.05). We performed a multivariate logistic regression analysis using the stepwise method. We used 8 items from the responses of the elderly subjects and their caregivers that were identified as being independently associated with the urination method using the chi-squared test (or Fisher's exact probability test), with the urination method as the dependent variable. Among elderly people, the ability to use a toilet properly (p=0.004) and independent mobility (p=0.028) strongly influenced their use of a toilet for urination. Among caregivers, the influencing factor was not thinking that they have to use the toilet even though they are unable to urinate (p=0.027). <b>Conclusion: </b>The use of a toilet for urination by elderly people was influenced by their physical functions and by the caregivers' attitudes toward excretion-related nursing care. When providing urination assistance, it is important for caregivers to maintain the physical functions of the elderly by providing support when necessary. It is also important to assess the bladder function by, methods such, as measuring the amount of residual urine and cooperate with a physician.

    Objective: The objective of the present study was to clarify nurses' decisions and the factors affecting their decisions regarding the use of 4-point bed-railing restraints for post-operative acute patients diagnosed with dementia. Methods: An anonymous, self-administered questionnaire survey was conducted on nurses working in the surgical wards of hospitals, regarding their understanding of the three major kinds of restraints and the main factors affecting their decisions to use 4-point bed-railing restraints to prevent falls. Results: Responses from 547 participants were analyzed. A total of 170 respondents (31.1%) were able to implement the use of 4-point restraints for patients diagnosed with dementia. About 40% replied that they could not decide and could not be sure when to use 4-point restraints for those diagnosed with dementia who exhibited states of excitement or restless behavior. Factors influencing decisions regarding the use of 4-point restraints for post-operative acute phase patients diagnosed with dementia were "employment in an ICU or HCU", "three-shift system", and "completion of nursing ethics training", with a significantly higher proportion of respondents in these groups agreeing to the use of 4-point restraints. Conclusion: These results suggest that use of 4-point restraints for post-operative acute phase patients diagnosed with dementia varies according to each nurse's work environment.

    The purpose of this study was to clarify the impact of lifestyle factors on nighttime sleep by conducting a survey on the state of sleep among long-term care facility residents. Targeting residents who use wheelchairs, we measured nocturnal waking duration, sleep efficiency, and total sleep duration using Actiwatch<sup>®</sup> and compared the daytime to the nighttime lifestyle factors. The subjects of analysis were 25 persons with an average age of 83.5±8.7. The total sleep duration of those who got out of bed twice or more during the daytime was 410.6 minutes (median), which was longer than that of those who got out of bed once (p=.034). The sleep efficiency of those who spent one hour or more in bed before sleeping was 66.5% (median), which was less than that of those who spent less than one hour (p=.038), showing a significant difference. Moreover, the sleep efficiency of those who had a preferred recreation was 80.4% (median), which was higher than that of those with no preference (p = .043), thus showing a significant difference. These results suggest that, by choosing the time in bed and lifestyle habits that suit each elderly person and by incorporating their preferences in lifestyle, the sleep and wake rhythm can be adjusted, which will lead to an improvement of the nighttime sleep of residents.

    The objective of the current study was to elucidate the defecation status and the factors relating to diarrhea in elderly patients receiving tube feeding. We conducted a survey on a total of 140 elderly individuals hospitalized in a recuperation ward who had been receiving tube feeding for &ge;6 months. A total of 113 patients (80.7%) had diarrhea and 138 (98.6%) had fecal incontinence. Items significantly related to diarrhea were the ability to maintain a seated position, form of nutritional supplements, and infusion volume per hour in patients receiving liquid-form nutritional supplements. Patients with diarrhea had a lower infusion volume per hour than those who did not have diarrhea, indicating that it would be difficult to treat diarrhea by reducing the infusion rate. In addition, many of the patients who did not have diarrhea were receiving semisolid nutritional supplements, confirming that use of semi-solid nutritional supplements is effective in treating diarrhea. Furthermore, diarrhea was associated with an inability to maintain a seated position, suggesting that physical function is an important factor affecting defecation.

    【はじめに、目的】 臨床においては踵部の褥瘡予防を目的としてクッション等を用いて下肢挙上を行うことがあるが、過度な下肢挙上により仙骨部に褥瘡を発症する事例が存在する。また先行研究において膝関節拘縮は仙骨部や踵部に対する褥瘡発生リスクを高める要因の一つとされている。以前から褥瘡予防を目的とした背臥位や30°側臥位などの体位と仙骨部接触圧との関係や膝関節の拘縮が仙骨部接触圧にどのような影響を及ぼすか調査した研究はよく行われている。しかし膝関節屈曲拘縮及び下肢挙上の高さが仙骨部接触圧に与える影響については検討されていない。本研究の目的は膝関節屈曲角度と下肢挙上の高さが、仙骨部接触圧にどのような関連が生じるのか明らかにすることである。【方法】 対象者は20歳から35歳までの健常な成人男性で、BMIが18.5以上25未満の者を対象とした。測定期間は平成23年5月1日~10月31日、測定項目は対象者に対して膝関節角度(0°、30°、50°)と下肢挙上の高さ(0cm、5cm、10cm、15cm、20cm)を変化させ、背臥位におけるそれぞれの仙骨部接触圧を測定した。また対象者の背景(年齢、身長、体重)を調査した。測定方法は仙骨部接触圧をニッタ社製Body Pressure Measurement System(以下BPMSと略す)を用いてベットにマットレス(ケープ社製 アイリス2)を置き、その上にBPMSのセンサーを設置し測定を行った。そして対象者は病衣を着用し、膝関節角度(0°、30°、50°)いずれかに設定したダイアルロック式膝装具 (中村ブレイス社製ラックニリガACL)を装着後、センサー上に背臥位となり、1分間安静を保持した後に全ての膝関節角度と下肢挙上の高さについて、仙骨部最大接触圧を20秒間に1回、計3回測定し平均値を仙骨部接触圧とした。下肢挙上の高さはマットレスから踵部までの距離とし、高さの設定は体圧分散能力のない高さ5cmの足枕とニシスポーツ社製バランスパッド(以下バランスパッド)を用いて設定した。なお下肢挙上時は両下肢を挙上した。 測定において順序効果を相殺するため、膝関節屈曲角度と下肢挙上の高さの順番はランダムに設定した。統計分析はExcel統計2006を用い、膝関節屈曲角度それぞれにおける下肢挙上の高さと仙骨部接触圧との関係をSpearmanの順位相関係数によって求めた。【倫理的配慮、説明と同意】 本研究は愛媛大学大学院医学系研究科看護学専攻研究倫理審査委員会の承認を受け、研究への参加は対象者の自由意志にて行い、書面による同意を得て行った。また個人情報の取り扱いについては氏名についてはコード化し外部に情報流出がないよう十分に留意した。【結果】 本研究に参加した対象者は15名であった。平均年齢は28.6±4.56歳、平均BMIは22.4±1.98であった。それぞれの膝関節屈曲角度における下肢挙上の高さと仙骨部接触圧との関係は、膝関節屈曲0°はr=0.41(p<0.001)、膝関節屈曲30°はr=0.35(p<0.001)、膝関節屈曲50°はr=0.41(p<0.001)であった。 【考察】 膝関節0°、30°、50°それぞれにおいて下肢挙上の高さと仙骨部接触圧に有意な正の相関関係が認められた。理由として下肢挙上により大腿部や下腿部後面とマットレスとの接触面積が減少し、大腿部後面や下腿部後面に係る接触圧が仙骨部へ移動したと考えた。また先行研究において大腿挙上運動によって骨盤は後傾方向へ運動するといわれており、下肢挙上による骨盤の後傾運動が生じ、仙骨部接触圧が増加した可能性が考えられる。【理学療法学研究としての意義】 下肢挙上は血圧低下時や整形外科手術前後などで行われる姿勢であり、臨床においてよく行われる姿勢である。また高齢化を迎えるにあたって膝関節屈曲拘縮を有する患者は今後増加することが考えられる。膝関節屈曲角度及び下肢挙上の高さと仙骨部接触圧との関連を明らかにすることで、仙骨部における褥瘡発生及び予防につながる知見が得られる可能性があり、本研究を行う意義は大きいと考える。

    <p>  In the present study, we implemented an educational program for providing excretion care through collaboration between care workers and nursing staff, and elucidated changes in the awareness of care worker regarding work, collaboration, and excretion with the objective of determining the effects of the program. The program consisted of lectures on basic knowledge of excretion and collaboration as well as a 12-week conference on excretion. Subjects were 26 care workers who provided valid responses among the 52 care workers working at two long-term care health facilities in Prefecture A. Subjects were divided into three groups based on 3 points in time, before intervention, after lectures, and after conferences, to make a comparison between groups. The results showed that in terms of beliefs in excretion care, participants' beliefs in the potentiality of lessening the usage of diapers through care became significantly higher through time. However, no significant changes were found in job satisfaction or sense of control over work. These findings suggest that the present program is effective for enhancing excretion care, and clarifying issues related to promoting collaboration.</p>

    <p><b>Abstract</b></p><p>The aims of the present study were to elucidate the factors affecting QOL in patients receiving outpatient chemotherapy and investigate nursing support that maintains and improves patient QOL. We conducted a questionnaire survey on patients receiving outpatient chemotherapy and received 95 valid responses. The survey investigated attribute and physical factors (severity of side effects), social factors (conditions surrounding support, going out, executing role and tasks at work and home, and interacting with others, including local people, friends and relatives), psychological factors (financial burden and time-constraints imposed by outpatient chemotherapy, and concern about lifestyle while receiving outpatient chemotherapy) and QOL. The Japanese version of FACT-G was used to measure QOL. Nausea, diarrhea, interaction with others, role execution, advice from healthcare professionals and concern about lifestyle while receiving outpatient chemotherapy were found to be significantly correlated with QOL. Multiple regression analysis was then conducted with these six items serving as independent variables and QOL as the dependent variable. A relationship with QOL was observed for interaction with others, role execution and concern regarding lifestyle while receiving outpatient chemotherapy. The more patients interacted with others and performed a useful role in society and the lower the concern regarding lifestyle while receiving outpatient chemotherapy, the higher the QOL. These findings suggest that it is important for nurses to pay attention to and provide support for not only physical aspects but also patients' social and psychological aspects.</p>

    The purpose of this survey is to identify and analyze the factors related to positive appraisal on the part of the family caregivers engaged in in-home medical care by examining their characteristics. The survey has been conducted by interviewing 190 primary caregivers (117 of them are giving in-home medical care, "medical group", and 73 are not, "non-medical group") by the use of questionnaires. <br>The survey shows that the "medical group" has the characteristics of long care-giving hours and strong in-home care motives. Compared to the "non-medical group", the "medical group" is shown to give higher responses, as positive appraisal, to "self-developing feeling through care-giving" and "willing acceptance of caregiver's role". As for the factors related to the positive appraisal, the "medical group" is shown to have strong motives for in-home care and "emotional approach style" as the coping process. <br>The above results suggest that it is essential for the caregivers to recognize care-giving practices as their own roles and that the good relationships between caregivers and care-receivers are fundamentally important. Nurses need to be involved so as to help caregivers develop the role recognition and to assist in forming better relationships between givers and receivers.

    In this study, falling self-efficacy (FSE) was surveyed by self-entering questionnaire to obtain basic information to utilize for the falls prevention of healthy aged persons. The subjects were 158 healthy elderly persons in the age of 65 or above who take part in community-based companionship activities on a regular basis, 57 males and 101 females. The relations between FSE and age-related somatic symptoms, physical activity, fear of falls, consciousness of decline in walking ability and pride against acting to prevent falls were examined by chi-square test and one-way analysis of variance. Followings were found; 1) Sex, age, age-related somatic symptoms, The Index of Competence development by Tokyo Metropolitan Institute of Gerontology, self or familiar persons' experience of falls or fractures, fear of falls, consciousness of decline in walking ability and pride against acting to prevent falls were related with FSE of healthy elderlies. 2) Around the age of 75 would be the turning point of FSE. The data suggests FSE would fall after the age of 75. 3) FSE could be basic information for the assessment of falls prevention of aged persons, if you understand not only objective physical abilities including physical activities, consciousness of walking ability, fear of falls and experience giving up what the subject want to do due to the fear of falls but also how the subjects face with the decline of these abilities and whether that motivates them to act to prevent falls. 4) Self or others' enactive mastery experiences and vicarious experiences elevated FSE. Self or others' experience of falls and whether having opportunities to communicate with others are important for FSE and to motivate elderlies to prevent falls. That implies they can be utilized for falls prevention campaign.

    本研究では、高齢者の生活の場の転移に伴う排泄ケアに関する施設間での情報共有・協働のために、①排泄に問題を生じやすい高齢者の特徴を明らかにする、②移転時に継続して個別ケアを実践するために必要な施設間での情報共有の内容と方法を明らかにする。特に、回復期リハビリテーション病棟において、急性期から回復期、回復期から慢性期(施設や在宅)へとケアの場が変化する時点に着目して研究をすすめる予定である。
    本年度は、入院・入所・退院後に排泄に関する問題が生じ、他施設との連携を必要としたケースについて、①どのようなケースであったか、②どのような情報共有・連携が必要であったかについて、回復期リハビリテーション病院、特別養護老人ホーム及び訪問看護ステーションの看護師等から情報収集う予定であった。現在、特別養護老人ホームの看護師、急性期病院の医師などからの情報で、尿道カテーテルを留置中の患者について、その適応の理由や抜去の可能性についての情報共有の必要性を確認している。特に、課題になる点は、抜去の際の方法、時期や判断であった。

    本研究では, 高齢者が罹患する可能性が高い誤嚥性肺炎に影響を与えるオーラルフレイルのうち,機能低下レベルであるとみなされる第1レベルから第3レベルまでを対象とし,地域在宅高齢者に対する簡便かつ効果的なオーラルフレイルへの予防的看護介入プログラムの開発を目的としている。
    本年度は,オーラルフレイルの第1レベルに対応した「地域在宅高齢者へのオーラルヘルスリテラシー向上介入プログラムの開発」を目的とし,まずは地域で行われているオーラルヘルスリテラシー向上のための保健活動に関する文献検討を行った。その結果,地域の通いの場などにおいて,高齢者を対象に口腔保健に対する意識を高めるための取り組み(口腔体操を含めた健康教育)が行われていることが明らかとなったが,効果をどのように評価しているかについては明確に述べている文献はほとんど見られなかった。
    次に,オーラルフレイル予防に対応している専門家(医師,歯科医,歯科衛生士,保健師,訪問看護師,呼吸サポートチームに属する専門看護師,認定看護師,理学療法士等)を対象に実際の活動内容とその評価方法についてのヒアリング調査のための研究倫理審査委員会における受審の手続きを行い,現在ヒアリング調査を実施しているところである。
    今年度はCovid-19感染拡大の状況下にあったため,ヒアリングの延長が生じた。現在ヒアリングとともに分析を行っている。

    2020年度は、2018年度までの収集データの整理と新型コロナウイルス感染対策を講じた上で、約90名に対して追跡調査を実施した。分析面では、自記式質問票によって歯の本数(15本未満・15-24本・25本以上)やかみ合わせ(よい・悪い)、特定健康診査の標準的な質問票の身体活動に関する3項目(「1回30分以上の軽く汗をかく運動を週2日以上、1年以上実施」「日常生活において歩行又は同等の身体活動を1日1時間以上実施」「ほぼ同じ年齢の同性と比較して歩く速度が速い」)と軽度認知障害との関連を分析した。その結果、歯の本数が15本未満、噛み合わせが悪いと軽度認知障害の多変量調整オッズ比が有意に高く、一方で身体活動に関する3項目について「はい」と回答した数が多いほど軽度認知障害の多変量調整オッズ比が有意に低かった。食行動との関連も分析したが明確な関連は得られなかった。また、飲酒量が多い男性ほど軽度認知障害の多変量調整オッズ比が高いことを学術誌に報告した。

    本研究は、在宅要介護高齢者(以下、高齢者)のアセスメントに基いた効果的な排尿ケアプログラムを開発し、その効果を検証することを目的としている。本研究でいうところの効果的な排尿ケアプログラムとは、①排泄ケアが必要な高齢者と家族が選定されていること、②膀胱機能、および排泄動作自立度がアセスメントされ高齢者の安全が守られること、③高齢者と家族のニーズに基づき実施されること、④家族の介護状況がアセスメントされ介護負担が増強しないことを目標としている。
    2019年度は、在宅排尿ケアプログラムの原案を作成した。2020年度は、作成したプログラムの原案を基に、研究メンバーで意見交換し、在宅排尿ケアプログラム(案)として修正した。排尿ケアプログラム(案)は、①対象者の選定:排泄に関するニーズがある高齢者と家族について、家族のもつ排泄の情報をアセスメントしたうえで選定する、②ベースライン期:膀胱用超音波画像診断装置を用いて残尿測定し排泄状態のアセスメントを実施したうえで排尿ケアを実施する(3日間)、③介入期間:日中8時間を目安に、ベースライン期で決定した排尿ケアを実施する(約8週間)、④評価期間:評価指標を基にした評価、および膀胱用超音波画像診断装置を用いて残尿量を測定し排泄アセスメントを実施し、排尿ケアの評価を行う(約1週間)、とした。2021年度は、作成したプログラム(案)を用いて高齢者と家族を対象とした排尿ケアプログラム(案)を実施実施する予定であったが、コロナ禍でフィールドに出向くことが困難な状況となり、プログラム(案)の実施を延期した。
    2022年度は、協力施設の施設長、およびスタッフと、コロナ禍においてどのように排尿ケアプログラム(案)を進めていくかについて再調整し、実施に向けて準備している。

    研究の目的は、介護医療院における重度要介護者に対する快適な排便ケアの実践の実態を明らかにし、実践に影響する要因との関連性を明らかにすることである。これにより、医療と生活の両方の視点から排便ケアに取り組む必要性のある介護医療院において、どのようなケア実践がされているのかが明らかになることによって、現状の課題が明らかとなる。さらにその関連する要因を明らかにすることで、排便ケア向上のために必要な、組織に求められる課題が明らかになる。
    重度要介護高齢者に対する快適な排便ケアの実践は、アセスメントに基づく便秘改善のためのケア、苦痛と羞恥心に配慮したケア、チームアプローチの3つの要素から構成される。この重度要介護高齢者に対する快適な排便ケアの実践は、施設の属性に関する要因、看護師個人の属性に関する要因、組織環境に関する要因、看護師の排便ケアに対する信念、看護師の生活を支える看護の態度、看護師の排便および排便ケアに関する知識の6つの要因から影響を受けると予測した。
    本調査において従属変数として扱う看護実践は、快便を目指すケアプロトコール、国内外の高齢者の便失禁および便秘に対する介入研究等、便失禁ガイドライン、高齢者の薬剤使用に関する文献等を参考に、重度要介護高齢者の快適な排便ケアの実践項目を独自に作成した。アセスメントに基づく便秘改善の為のケア33項目、苦痛と羞恥心に配慮したケア9項目、チームアプローチ9項目の計51項目からなる。事前に特別養護老人ホーム、介護療養型病棟、介護医療院に勤務する看護・介護職にプレテストを行い、内容妥当性は確認した。
    研究対象となる介護医療院に勤務する看護師・准看護師に調査票を配布し、現在調査を行なっている。

    本研究は、看護・介護職が高齢者の潜在する排泄機能に気づき、排泄ケアに積極的な態度を形成するために、①事例を通して知識を習得する教材を用いた研修を行い、その後、②継続した事例検討会でフォローアップする教育プログラムを開発する。教育プログラムの前後で、介護・看護職の排泄ケアに関する知識と態度の評価に加え排泄ケア実践に関する評価も実施し、プログラムの効果を明らかにする予定である。
    昨年度作成した評価指標となる態度・知識・排泄ケアの実践の測定項目を使用して、回復期リハビリテーション病院で働く看護師を対象に調査を実施した。態度に関する測定項目を10項目作成したが、天井効果が認められ4項目のみで主成分分析を行い1成分として分析に用いた。個別の排泄ケア実施との関連は認められ態度を測定するために有用であると考えるが、再度、内容を再検討する必要がある。排泄ケアの実践の評価項目に関しては、21項目作成し、天井効果のみられた6項目を除外し因子分析を行った。「アセスメントに基づくケア」、「排尿日誌の活用」、「高齢者の思いを尊重したケア」の3因子が抽出され、本研究に活用できることが確認された。
    教育教材作成のために、えひめ排泄ケア研究会で実施している事例検討会で検討した43事例について、分析を行った。その結果、看護・介護職が対応困難と考える原因は、頻尿が15例と最も多く、その他の排尿に関する問題は夜間多尿や排出障害があがっていた。排便に関しても、便秘(7例)、水様便の便失禁(5例)、頻回な便意(3例)と相談内容の多くを占めた。さらに、認知機能の低下によって生じる放尿、排泄介助拒否、弄便や、身体機能の低下によるトイレでの座位姿勢が取れないこと等の相談がみられた。この結果に基づき、典型的な事例を作成し、事例のアセスメントと援助方法、評価項目を基礎知識と関連付けて理解できるようなマニュアルを作成中である。

    本研究の目的は、施設高齢者に対する効果的な排尿誘導プログラムを作成することであり、機能性尿失禁を有する高齢者に作成した排尿誘導プログラムを実施し有用性を検証することである。文献レビューの結果では、日本の看護・介護実践の現場において、①看護職・介護職に対する尿失禁の治療やケアに対する教育的介入の必要性、②①の専門的な知識に基づいた看護・介護協働体制による排尿誘導の実施、③高齢者の心理的負担の軽減に考慮する必要性を確認したことから、これらの枠組みを活用した「排尿誘導プログラム(案)」を作成した。さらに、詳細な内容について検討のため予備調査を実施した結果、日常的に尿の性状の観察は実施していたが、下部尿路症状を把握するために必要な残尿量のチェックや尿量の測定は実施している割合が高くないことが明らかになった。介入前の下部尿路症状のアセスメントの必要性や具体的な方法の説明は不可欠だと考えた。現在、「排尿誘導プログラム」のアウトカム指標および測定ツールを検討している段階である。アウトカム指標および測定ツール決定後、作成した排尿誘導プログラムについては、排尿ケアの専門家や実践者からの意見を聴取し、可能な限りわかりやすく使いやすいものになるよう工夫する。また、プログラムを確実に実施してもらえるようにパンフレットも作成する予定である。2019年度から介入を開始できるよう、倫理審査申請およびフィールドの確保など準備を進めることを計画している。

    Toileting assistances are behavioral therapy and lower urinary tract rehabilitation in the treatment of urinary incontinence: UI. Especially, Prompted Voiding have used in particular in North America, mainly in long term facilities for people with cognitive impairment or dementia or not. In recently years, PV have conducted nursing homes in other countries. Since 2018, We have tried the systematic review for the effectiveness of Prompted Voiding for elderly with UI. In results, the significantly decrease of urinary incontinence rate/day was indicated. Development of the toileting assistance guideline will be continued.

    The feelings of difficulty of family caregivers concerning excretion assistance for an elderly person requiring long-term care and experiencing incontinence after they started using diapers were classified into nine categories. The family caregivers considered urinary disturbance during the nighttime, cleaning after incontinence, and a high number of bathroom visits as difficulties, which were believed to originate from the elderly person’s urination status. Furthermore, the family caregivers also experienced their own trial and errors by imitating others and the physical burden when giving assistance, which were believed to originate from the caregivers’ lack of knowledge. Instructions by nurses on proper assistance methods after assessing the urination status of the elderly patient and family caregivers’ knowledge and skills are believed to be important.

    Elderly dementia patients living in elderly care facilities engage in frequent voluntary excretion behavior, in other words the “behavior of going to the bathroom alone while risking falling down”. The excretion status of elderly patients that frequently had night-time bathroom visits was investigated using voiding diaries. The results showed normal voided volume per void and hardly any residual urine, indicating that the behavior was motivated by the desire to void. As the cause of nocturia is night-time polyuria, the present stud revealed the need for assistance in addressing night-time polyuria

    We clarified the evaluation of bladder function of the elderly who entered the facility, the actual condition of the toileting assistance, and the basis and judgment of the caregiver when determining the Toileting assistance .I attempted to prepare a protocol for effectively practicing the toileting assistance in elderly facilities.Associated with Toileting assistance, N-ADL, VI, appropriate toileting behavior, when changing from timed voiding to Habit trainig, it is necessary to evaluate the potential of the elderly from N-ADL and VI.It was important to support the time when changes in urinary condition of elderly people stagnated.The aid providers was recording voiding records, wavering in the stage which I began to record. However, when the point of involvement became clear from the voiding record, they had confidence and reflected each judgment in care.We only extracted voiding assessment indices for practicing effective toileting assistance.

    We investigated the associations between 1. Sleep Quality and night, morning and office hypertension, 2. Sleep disordered breathing and night, morning and office hypertension, and 3. supra-additive impact of sleep disordered breathing (SDB) and blood pressure on carotid arterial wall thickness (CAWT). Subjects were 2,033 men and women aged 30-79 who participated in the community health check-up in Japan between 2009 and 2012. The results suggested that 1. Deteriorated sleep quality was associated with the variation in morning blood pressure, 2. Severe sleep disordered breathing was associated with the higher morning blood pressure and larger difference between night and morning blood pressure. 3. SDB together with night hypertension might be associated with carotid atherosclerosis more strongly.

    The present study investigated the urinary status and urination practices of community-dwelling elderly people with incontinence who are receiving home care, as well as the caregiving status of their family caregivers, in order to clarify factors related to urination practices and develop support measures . Regarding factors directly related to elderly patients, the proportion of patients who perceived the urge to urinate, who had a high degree of independence in transfers and in the sequence of movements accompanying transfers, and who could recognize and correctly use the toilet was higher among those who used a combination of the toilet and adult diapers compared to those who used only adult diapers. No significant correlations were observed for caregiver factors.

    This study aims to clarify the characteristics of excretion troubles among the elderly who dwells in a long-term care facility, as well as to examine the efficacy of assistance program based on causes of their excretion troubles. The urinary incontinence is related to no urge to urinate rather than the malfunction of the bladder. Study participants were the persons who claimed no urge to urinate and had urinary incontinence, although their bladder functions were maintained. Researchers conducted urination assistance based on participants' desire to urinate after the regular confirmation of their urge. As a result, they were able to claim their desire to urinate and the number of their urinary incontinence was declined. Moreover, researchers decreased the amount of the laxative for the participants who were incontinent of muddy or watery stools. After the aid, their symptoms of diarrhea were improved and their stool incontinence had disappeared.

    In order to investigate an appropriate care method for defecation problems, an actual condition survey on defecation problems as well as the causes and care methods of these problems were conducted for elderly residents in nursing homes. Nursing care for elderly individuals suffering from diarrhea and incontinence due to laxative use involved scheduled toileting and use of diapers, and did not include assessment of desire to defecate or the ability to maintain a sitting position.
    Moreover, we provided nursing care based on the assessment of the state of fecal incontinence among nursing home residents and evaluated the effects of the nursing care. The subjects were aged nursing home residents who habitually took laxatives to ease constipation.
    For the first 4 weeks, we observed to assess the states of subjects' defecation troubles and formulated treatment plans based on the individual assessment. For the next 4 weeks, we guided the subjects to a toilet to provide opportunity of defecation within 30 minutes after meals, monitored and confirmed the desire to defecate, helped increase abdominal pressure and stabilize sitting position by handrails and other aid equipment. We also encouraged to take more dietary fiber, gave hot-pad, and conducted abdomen massage to facilitate transit of stools whenever appropriate.
    We provided the nursing care to 5 subjects with severe dementia (1man, 4 women). They had ability to maintain sitting position with wheelchair, and they needed aides in daily life. Three subjects had never passed stools on a toilet seat. During the nursing care, most increased the number of defecation on a toilet seat, and one could do most cases successfully, avoiding incontinence of feces. All increased the number of defecation desire. They also showed some improvement over the ability of maintaining their posture and easier transfer in arising from and returning to wheelchair.

    Background : The purpose of this study is to clarify the actual condition of home health care in Japan, China and USA, and to examine the gain of caregivers from a care-affirmative point of view.
    Targets and Methods : The study involved 215 groups of the elderly in need of home care and their family caregivers in Japan, China and USA. Interviews using questionnaires were performed to investigate the actual condition of their home care and their psychological and social condition concerning home care. All groups provided informed consent for the purpose of the study, their privacy guarantee and participation in the study.
    Results : As for the gender of the family caregivers, all in Japan, China and USA, female caregivers were more than those of male, and the ratio of male to female were higher in China compared with the other two countries.
    China also had more caregivers in the family and better family condition than Japan and USA. USA had the highest average age of the elderly as well as caregivers. Japan had the worst physical condition of the elderly and caregivers and the longest care period. As for caregiver's coping process, remedial skills were mainly used in China and USA : cognitive and remedial skills, in Japan. Caregiver's life satisfaction in each country generally went less owing to caregiving life, bat there were some whose life satisfaction stayed same before and after their caregiving lives. Both in Japan and China, "whether they have their own free time or not" was a common factor affecting their life satisfaction. "Care-affirmative feeling" was also another factor in China.

    Purpose : The purposes of this study are to examine the QOL and self-efficacy of family caregivers and to investigate the connection between life-style and the circumstances of care giving. A support model for family caregivers is constructed.
    Method : The participants in the study were 191 primary caregivers of elderly people being cared for at home in Ehime prefecture (45 islanders, 50 rural inhabitants and 96 city dwellers). The questionnaire included items related to the care-recipient's background, physical circumstances, Activity of Daily Living (ADL), Life Satisfaction Index K(LSIK), level of care need, as well as items relating to the caregiver's background, physical circumstances, family APGAR, period of care-giving, daily care-giving time, use of community care services, Visual Analogue Scale of happiness (VAS), and self-efficacy.
    Results and discussion :
    1. The primary caregivers in the three areas were predominately female. While they were usually the daughter-in-laws in island communities, they were the spouses in rural and urban areas.
    2. The most common level of care need was level 4 for islanders, level 1 for rural inhabitants, and level 5 for city residents.
    3. In terms of use of services, caregivers in island communities frequently used day-care services and short stays, while those in rural areas frequently used day-care services, and those living in the cities frequently used home-help services, community visiting nursing and day-care services.
    4. Overall, the VAS scores for the primary caregivers in all three areas had declined, compared to before they started caring for the elderly family member. However, this had risen or remained the same for 30% of the caregivers.
    5. A factor analysis of the self-efficacy scale identified three factors : the first factor is "will to continue care giving", the second factor is "self-control towards the care giving situation", and the third factor is "use of information concerning care giving".
    6. Correlations were found between VAS, self-efficacy and family APGAR, respectively.
    7. Factors influencing VAS and self-efficacy were physical circumstances and number of family members.
    These results indicate that it is necessary for caregiver support to consider the characteristics of family needs depending on area, and the physical and mental states of the caregivers.
