【目的】一般急性期病院の消化器外科,消化器内科を退院した20歳以上の患者を対象に,管理栄養士による早期栄養介入体制が入院日数に与える影響を検討した.【方法】一般急性期病院6施設の消化器外科,消化器内科を退院した20歳以上の患者345名を対象とした.診療録を後方視的に調査し,主要アウトカムは入院日数とした.管理栄養士が栄養介入を開始するまでの日数の中央値は1(四分位範囲0-2)日であったことから,1日以下の施設を早期栄養介入体制群(以下,早期体制群;3施設209名),2日以上であった施設を非早期栄養介入体制群(以下,非早期体制群;3施設136名)に分類し,群間比較を行った.【結果】入院日数の中央値(四分位範囲)は,早期体制群10(7-15)日,非早期体制群11(8-21)日であった.早期栄養介入体制の有無と入院日数の関連について,Cox比例ハザード回帰分析の結果,非早期体制群と比較した早期体制群の退院ハザード比は有意に高かった.【結論】管理栄養士による早期栄養介入体制は,一般急性期病院の消化器外科,消化器内科入院患者の入院日数を短縮する可能性が示唆された.(著者抄録)

【Background】 The purpose of the present study was to　examine the association between the timing of nutritional intervention by registered dietitians and the length of hospital stay for gastroenterological surgery and gastroenterology patients in general acute care hospitals.
【Methods】 This retrospective observational study was based on data from six acute care hospitals. We included 345 inpatients discharged from the gastroenterological surgery and gastroenterology department. The primary outcome was the length of hospital stay （number of days from admission date to discharge）． Participants were categorized into two groups by the number of elapsed days from hospital admission to the nutritional intervention
of each hospital ：≤1 day （early intervention group） and ≥2 days （non-early intervention group）. The Cox hazard
model was used to calculate hazard ratios （HRs） for discharge.
【Results】 The median length of hospital stay were 10（interquartile range ［IQR］，7 - 15） days in the early intervention group, and 11（IQR, 8 - 21） days in the non-early intervention group. We found that the early intervention group had a hazard ratio of 1.336（1.064 - 1.677） for hospital discharge.
【Conclusion】 Results of the present study implied that early nutritional intervention by registered dietitians contributes to preventing prolonged hospital stays.
